The lecture presents modern concepts of diagnosis and treatment of atrial fibrillation; principles of choosing the tactics of management of atrial fibrillation in general practice. The epidemiology of atrial fibrillation, risk factors, causes and mechanisms of its development are considered. The classification of atrial fibrillation is given. The tactic of diagnosis of atrial fibrillation is described. The principles of treatment of atrial fibrillation, including the anticoagulant therapy and pharmacological cardioversion, are presented.

Current issues of diagnosis and treatment of osteoporosis in men in general practice

Abstract

The author revises the latest evidence in the literature regarding managing of osteoporosis in men. Osteoporosis is an important under-recognized problem threatening men. Among people over 50 years of age, a fifth of men will suffer a fragility fracture during their remaining lifetime. The leading risk factors of male osteoporosis are genetic predisposition, low sex hormones and secondary osteoporosis. The latter comprises up to 40% of all male osteoporosis cases. The most common causes of secondary osteoporosis in men are alcohol abuse, endogenic and exogenic hypercortisolism and hypogonadism. Treatment of the underlying disease may result in BMD increase and probably fracture risk improvement. Bisphosphonates (alendronate, risedronate and zoledronic acid), denosumab and teriparatide are approved for treatment of male osteoporosis. Non-pharmacological treatment including avoiding smoking and excessive drinking, health lifestyle and a diet rich in bone-healthy nutrients should also be considered.

The role of a nurse in geriatric practice

Abstract

This review accentuates the problems of geriatric nursing care - areas of activities, which has not received to date widespread development in Russia. Meanwhile, the experience of countries, having the developed geriatric services, indicates that these activities are standardized and well structured, which allows to judge about the quality of geriatric nursing. The review deals with foreign resources that serve the objectives of obtaining special education of nurses as well as the substantial support in the practice of geriatric nurses.
