RATIONAL CHOICE OF EYE DROPS FOR PUPIL DILATION IN NEONATES

Abstract

Background. The problem of a choice of eye drops with the minimum side effects for mydriasis for screening of retinopathy of prematurity is actual.

The purpose  to estimate clinical efficiency of the combined mydriatic eye drops Fenicamide in ophthalmoneonatal practice.

Materials and methods. Under supervision were 75 (150 eyes) newborns and babies aged from 1 day to 3 months (on average 38,2±32,2 days). Patients are distributed on 3 groups depending on gestational age: the group 1 – 25 full-term children; group 2 – 25 premature children and group 3 – 25 very premature children. For achievement of the mydriasis once on 1 drop buried in the combined drug of 5% phenylephrine and 0,8% tropicamide. Observed dynamics of change of diameter of a pupil within 4 hours in each group of patients and fixed side effects within 24 hours. By means of the pediatric retinal camera RetCam 3 estimated quality of visualization of the fundus.

Results. The maximum dilation of a pupil (on average 6,5±0,5 mm) was reached on average to 60±14 minute after an instillation, remained within 1 hour and was normalized in 4 hours. Mydriasis varied from 5,9 mm at very premature children to 7 mm at the full-term newborns (p>0,05) and allowed to investigate in full and qualitatively of the fundus of all patients, including visualization of peripheral zones of the retina when screening ROP. It is established that side effects on the drug were absent at 76% (57) children, were present – at 24% (18) patients, mainly in the form of local reaction.

Conclusion. Eye drops Fenicamide is an effective mydriatic, with the convenient regimen of instillation, have the minimum side effects and can be recommended for wide use in ophthalmoneonatal practice.
