DISCIRCGLATORY ENCEPHALOPATHY: HETEROGENEITY OF THEDEVELOPMENT OF CHRONIC BRAIN ISCHEMIA, MODERN APPROACHES TO THERAPY
Abstract
Discirculatory encephalopathy is the most common type of chronic cerebrovascular diseases and representsslowly progressing insufficiency of brain blood circulation, leading to lacunar infarction and leukoaraiosis, diffusesigns of brain atrophy with infringement of its functions. The modern concept of the heterogeneity cerebrovas-cular insufficiency is complemented by new facts and expands our understanding about pathogenesis chronicbrain ischemia. Treatment of chronic brain ischemia and vascular dementia is one of the most controversial sec-tions modern vascular neurology. Improvement of the efficiency discirculatory encephalopathy treatment pos-sible with the combination of etiopathogenetic principle in the treatment and use of drugs, whose effectivenesshas been confirmed in randomized controlled trials.

THE PROBLEMS OF COMPLIANCE WITH THE ORDERS OF MEDICAL CAREIN THE PRIMARY HEALTH CARE SECTOR
Abstract
The article considers the features of work of primary care physicians in the requirements prescribed by theorders of medical care for therapeutic pathology. Specified contradictions that exist in the legal documents, bearingthe risk of impossibility of implementation of the requirements of the considered orders of magnitude in medicalpractice. There are some ways of overcoming the difficulties of the implementation of the orders of medical care.

DIFFUSE TOXIC GOITER IN PATIENT FROM NORTH REGIONOF SAUDI ARABIAN
Abstract
The article presents the results of treatment of 54 patients — the indigenous inhabitants of the northern re-gion of Saudi Arabia with newly diagnosed diffuse toxic goiter (Graves' disease). Peculiarities of the diagnosis,treatment and therapy of Graves' disease in this ethnic group are present. The authors have identified a combi-nation of Graves' disease with other autoimmune diseases and the high rate of relapse, despite adequate medi-cation thyrostatic therapy. Shows the results use of I131 in patients with Graves' disease.

PREVALENCE OF RESPIRATORY CHLAMYDIOSIS IN CHILDREN,ATTENDING KINDERGARTENS
Abstract
The article represents the data on the prevalence of Chlamydia contamination of upper respiratory tract inchildren, who attend kindergartens. The authors show species composition of identified Chlamydiae structuredby gender and their predominance in children with respiratory chlamydiosis in acute and chronic pathology ascompared to non-contaminated children. The obtained results are based on examination of 121 children. Chla-mydiae diagnosis was carried out by implementing direct immune fluorescence.

RATE OF HARMFUL AND HAZARDOUS ALCOHOL CONSUMPTION ESTIMATEDBY LATENT CLASS ANALYSIS
Abstract
The rate of harmful and hazardous alcohol consumption at the point of first contact with health care is esti-mated with help of AUDIT questionnaire and consumption recall diaries. The correction for imperfect diagnosticinformation is made. Highest prevalence of harmful and hazardous consumption is found among males visitingdermatologists, lowest-among young female students. After correction for imperfect information prevalence wasestimated to be 45,9% (95% CI = 43,1-48,7%). It is shown that there are four different consumption groupsamong respondents — reasonable consumers, persons that consumed harmful and hazardous amounts in thepast, persons that continue hazardous consumption and those with harmful continuing consumption. The needfor preventive measures in primary care is stressed.

COMPARISON OF AUDIT VERSIONS FOR ALCOHOL CONSUMPTION
Abstract
The reliability of the full and abridged version of AUDIT questionnaire was studied in a sample of respondents from Moscow and St. Petersburg (n = 1538). It was shown that Cronbach's а for the full version is equal to 0,888 and for abridged, 3-item version (AUDIT-C) — 0,788. Standard error of measurement (SEM) was relatively high for AUDIT-C and comprised 21% of the full range of values. In comparison for the full AUDIT SEMwas only 12% of the range. Comparison of the dichotomized versions of the questionnaires showd that agreement was good but not excellent (Cohen's к = 0,736, 95%CI = 0,701-0,771). In the same time one question(AUDIT-3) has almost as good agreement with results of full questionnaire as AUDIT-C (к = 0,641, 95%CI = 0,600-0,681). taking into account lower reliability of AUDIT-C and possibility of significant disagreementit is recommended to use full version of AUDIT and in case of acute time shortage — one question AUDIT-3.
