Experience of medical support organization of the tank biathlon competition

Abstract

Authors presented information about medical support of the tank biathlon competition taking part on 12—17 August 2013 at the Alabino. Crews from Belarus, Kazakhstan and Armenia (The Collective Security Treaty Organization) were invited for the contest. On the basis of the idea of the contest and location of the Alabino, the Main military-medical board of the Ministry of Defence of the Russian Federation developed and passed the Programme of medical support of the contest, gave the word to medical service of the Western Military District and central military-medical facilities about appropriation of funds. Personnel, providing the contest, was training every day during the period of preparation. Over a period of field ambulance station 73 people sought medical advice, 12 of them were sent to hospital. Authors came to conclusion the set tasks were completely fulfilled.

Abstract

Authors analyzed and generalized experience of 2910 anaesthetic supports, performed at the Branch 6 of Vishnevskiy 3rd Central Military Clinical Hospital of Russian Federation Defense Ministry during 2010—2012 in case of outpatient surgery such as endoscopic examinations (gastrofibroscopy, colonoscopy), gynecological support, dental care, ORL-surgery, urologic surgery, diagnostic and treatment arthroscopy. Authors gave information about preoperative, intraoperative and postoperative aspects of outpatient anaesthesia.

Peculiarities of occupational esophagogastroduodenal pathology in personnel serving at CW disposal and storage facilities

Abstract

In a process of medical care and supervision on staff and military personnel, eliminating the chemical weapons, it was revealed that they are more susceptible to esophagogastroduodenal pathology in comparison with the control group. Moreover, the given pathology has an asymptomatic disease course but associated with high contamination of Helicobacter pylori. Marked inflammation changes, atrophic and fibrosis manifestation and microcirculation dysfunction have been confirmed by histological analysis of gastric mucosa. We supposed that the complex of occupational health risk factors, including possible subliminal toxic influence of eliminated CW components determines this pathological dysfunction. Negative influence of shift work and irregular nutrition rhythm are not excluded. The findings of this research are dictating the necessary of revision of military medical expertise principles and the development of the more effective treatment and prophylactic methods.

Use of functional packages of medical stuff by military level of medical service of the Armed Forces

Abstract

Authors submitted results of recent developments made by The Kirov Military-Medical Academy and OOO «Special medical equipment»in accordance with State defence order in the area of modernization of the system of organizational equipment of military level of medical service of the Armed Forces of the Russian Federation. Along with other samples of organizational equipment, new functional equipment of medical stuff was developed and approved as supply. New equipment of medical stuff meets modern requirements and is highly valuated by medical services of foreign countries. Authors came to conclusion that functional equipment which is approved as supply and included into the Supply rate provides operational flexibility of set-up/tear-down stages of medical evacuation under the conditions of battlefield, allows to deliver medical aid on the basis of innovative medical technologies.

The use of local hemostatic agent «Celox» in experimental model of grade IV liver injury

Abstract

Ongoing intra-abdominal bleeding is the main life-threatening sequence of blunt abdominal trauma. The use of local hemostatic agents allows achieving fast and reliable hemorrhage control in case of parenchymatous organs injury. We investigated the efficacy of a dressing «Celox Gauze» in an experimental model of grade IV liver injury in large animals in the setting of coagulopathy. We used the following experimental model: 30—35% of blood volume was withdrawn followed by severe liver injury and hemostatic agent application. Laboratory tests were controlled in three endpoints. Hemostasis was evaluated in 2 days. «Celox Gauze» was effective for hemorrhage control in 100% of cases. All animals survived until the end of the experiment. Perihepatic packing with «Celox Gauze» applied is more effective to control hemorrhage than packing only. It has been noted lower volume of blood loss, lower decrease of red blood cells in animals of «Celox Gauze» group. We noted no adverse effects. Histological analysis showed the absence of any significant local hepatic damage. In whole the local hemostatic agent «Celox Gauze» has been shown highly effective in the model of parenchymatous bleeding. Additional studies with more number of animals are warranted to confirm our results.

Cerebral dysfunction after coronary artery bypass graft surgery with cardiopulmonary bypass

Abstract

The cardiac surgery is accompanied by multifactorial alteration of the brain, which can present in different clinical types of postoperative cerebral dysfunction. A stroke was diagnosed only after coronary artery bypass graft surgery with cardiopulmonary bypass. Postoperative cognitive dysfunction (including acute cognitive dysfunction and deferred cognitive impairment) was significantly more frequent after surgery with cardiopulmonary bypass. The preoperative use of complex cytoprotective medication Сytoflavin decreased the severity of the deferred cognitive impairment. The data show the prospects of the pharmacological neuroprotection in cardiac surgery with cardiopulmonary bypass.

According to numerous researches the authors emphasize the importance of the quantification of C-reactive protein in patients with community-acquired pneumonia. We gave an analysis of different publications on the given issue. We came to conclusion that available laboratory kits for the quantification of C-reactive protein in blood serum, including instant diagnosis, allow considering the given method as a method of common use for military-medical facilities.

C-peptide and its role as a predicator of the cardiovascular complications (Literature review)

Abstract

C-peptide is secreted from proinsulin and over the recent decade is considered as an active peptide that leads to different capillary actions and macrovascular complications in patients with type 2 diabetes mellitus. Results of the recent researches showed the C-peptide uptake in the blood vessel walls, in the juxtaglomerular apparatus of malpighian tuft, induction of local inflammation, proliferative effect on mesangial cell. The given effects showed an atherogenic role of C-peptide. Authors consider C-peptide as a possible predicator of cardiovascular complications and mortality in patients with type 2 diabetes mellitus and without diabetes.

Epidemiology and prophylaxis of community-acquired pneumonia in servicemen

Abstract

Community-acquired pneumonia is one of the most pressing issues for military medicine in connection with a high incidence among conscripts. After the vaccination beginning in 2002, the uninterrupted increase of the pneumonia incidence that had been observed in 90-th years was changed to the tendency of decrease. But in 2009 and 2010 the incidence of pneumonia increased significantly in all military districts because of crowded conditions of accommodation and activation of «mixing» of personnel in connection with reduction of military service period from 2 and 1,5 years to 1 year. The improvement of accommodation conditions and prevention of excessive cooling are the bases of prevention of pneumonia. It is recommended to use bactericidal air recycling apparatus in barracks. Medicament prophylaxis consists of vaccination with polysaccharide pneumococcal vaccine (Pneumo 23) together with influenza vaccine. It is necessary to approve in the troops conjugate pneumococcal vaccine (Prevenar 13), which was registered in Russia 2 years ago. Vaccination should be combined with the use of immunotropical and antivirus medication, vitamins.

Scoliotic spinal deformity in pilot personnel from aviation physical examination’s point of view

Abstract

According to results of performed examinations scoliotic spinal deformity in flight personnel has a low impact on professional health. This is proved by: oligosymptomatic course of disease — lack of complaints of pain, moderate pain, which is revealed only in case of loading tests and palpation; preservation of supporting and movement spinal function; lack of worsening of deformity during the flight service. At the same time in flight personnel suffering from scoliosis was registered a low tolerance to ergometric probe, which point to insufficient muscle reserve of lower extremities, abdominals and dorsum. This insufficient may have an adverse effect on G-tolerance of pilots serving in maneuvering aviation. According to this fact authors came to conclusion that first-degree scoliotic deformity is of importance for expert examination of pilots of high-performance aircraft. Scoliotic deformity in pilots of other branches of aviation is of importance only in case of clinical implications (pain syndrome, restraint of movement). From there, it is not necessary to make a record in regulatory documents of flight medical board about functional-compensatory spinal deformity (first- and second degree scoliosis) in flight personnel, except flight personnel of high-performance aircraft.

Experience of medical assistance in the hosital of Plesetsk Cosmodrome under conditions of large patient load after explosion of a launch vehicle

Abstract

Military hospital of Plesetsk Cosmodrome was founded on 20 December 1958. The aims of the hospital were always connected with medical support of the cosmodrome, including emergency situations. On 18 March 1980 a Vostok-2M rocket exploded on its launch pad during a fuelling operation. Experience of medical assistance under conditions of large patient load showed the necessity of constant readiness to medical assistance to patients with combined pathology (burn injury, orthopedic trauma and thermochemical injury of the upper respiratory tract), expediency of compact patient accommodation along with the modern anaesthetic machine and readiness to frequent suction bronchoscopy.
