About

The Military Medical Journal is the oldest national periodical devoted to medical issues; it has been published since January 1823.

The initiative to establish the Military Medical Journal belongs to a prominent figure in military medicine in the first half of the 19th century, Chief Military Medical Inspector of the Russian Army, President of the Medical and Surgical Academy Yakov Vasilievich Villie (1768–1854).

For many years, the Military Medical Journal was the only representative of Russia's periodical scientific medical literature. Over the years, he has turned into a priceless treasury of domestic medicine, which has absorbed many generations of physicians' vast experience. Until 1917, the magazine was headed, as a rule, by well-known military doctors, professors, and prominent organizers of the military medical service: Ya.V.Villie, P.A.Charukovskii, S.F.Khotovitskii, P.A.Naranovich, E.V.Pelikan, Ya.A.Chistovich, N.I.Kozlov, S.P.Lovtsov, N.P.Ivanov, A.I.Belyaev, A.S.Tauber, I.F.Rapchevskii.

In the 20th century until 1947, the responsible editors of the magazine were M.I.Baranov, E.I.Smirnov, N.I.Zavalishin. Subsequently, the Military Medical Journal was headed by full-time editors: in 1949–1955. - V.V.Skvortsov, in 1955-1960. - D.I.Troitskii, in 1960-1968. - I.E.Karpov, in 1968-1978. - A.V.Voropai, in 1978-1992. - V.V.Belozerov, in 1992-2007. - L.L. Galin. From May 2007 to the present, the journal is headed by M.V. Poddubny.

Since 1992, the journal is an organ of the Ministry of Defense of the Russian Federation. Frequency - 12 issues per year, volume - six printer’s sheets. 

Subject area description. Among the regular headings of the journal «Organization of medical support of the Armed forces,» «Medicine of emergencies,» «Military medicine,» «Medical and preventive issues,» «Epidemiology and infectious diseases,» «Hygiene and physiology of military labor,» «Through the pages of foreign medical Press,» «News Feed,» etc. In recent years, the subject area of publications, as a rule, has been focused on the issues of providing medical care in the context of local wars, anti-terrorist operations, technological disasters, and natural disasters. The problems of rehabilitation of the wounded and injured in the areas of conflict are widely covered. Themed issues of the journal are regularly published.

Modern information and digital technologies: prospects for application in the context of the mental health of military personnel

Abstract

Information and digital technologies have a significant impact on all aspects of human life, occupying a special place in various branches of medicine. Their use allows for solving the problems of diagnosis and treatment of mental illness. In recent years, artificial neural networks have been actively developed, capable of replacing the logic of medical thinking with specialized algorithms. The use of information and digital technologies to objectify mental disorders is of particular importance for military psychiatry, given the need to systematically address expert issues. It should be noted that these technologies have their limitations and currently cannot completely replace the existing forms of diagnostics. At the same time, they can be successfully used as auxiliary methods not only for syndromic but also for nosological diagnostics.

Prospects to treat wound infections in the spread of bacterial multidrug resistance

Abstract

The article reveals the problem of global antibiotic resistance and ways to overcome it in military healthcare. The authors provide historical background for the fight against antibiotic resistance based on breakthrough discoveries in the first half of the 20th century. The leading role of Soviet scientists in the development and production of the first antibiotics and bacteriophages is emphasized, thanks to which during the Great Patriotic War the medical service returned thousands of soldiers to duty and prevented the development of a cholera epidemic during the Battle of Stalingrad. In subsequent decades, the search for additional groups of antibiotics capable of suppressing multidrug-resistant microflora has become a priority throughout the world. However, the biological variability and adaptability of bacteria constantly outpaces the capabilities of the pharmaceutical industry. Bacteriophages are regaining their lost strategic importance, as evidenced by research and publications conducted by NATO countries on the emergence of «superinfection» in modern military conflicts. Therefore, the use of bacteriophages in the complex treatment of victims with infectious complications of wounds becomes an urgent need.

On gunshot wounds of the rectum (Literature review)

Abstract

Rectal trauma continues to be a pressing problem in both peacetime and wartime. The results of an analysis of the literature on gunshot wounds of the rectum are presented. The frequency of rectal injuries among gunshot wounds of the abdomen is 5.1–20.5%, extraperitoneal injuries range from 20 to 72%, and intraperitoneal injuries range from 25 to 80%. Complications after colon surgery reach 69.6%. Mortality ranges from 3 to 51.4%. Existing classifications of rectal injuries do not allow comparison of research results because of different approaches to naming injuries. Gunshot wounds of the rectum are almost always combined with other injuries to organs and pelvic bones, which complicates their diagnosis and treatment. Therapeutic tactics for intraperitoneal wounds of the rectum involve performing a Hartmann-type operation; for extraperitoneal wounds, it is necessary to carry out primary surgical treatment of the perineal wound with drainage, the application of a double-barrel unnatural anus to the sigmoid colon with a spur, and washing the caudal part of the rectum.

Unilateral «hypertransparent» lung in a soldier (clinical observation)

Abstract

A contract serviceman, 32 years old, was sent to the district clinical hospital with an unproductive cough, shortness of breath, pain in the right side of the chest, «right-sided emphysema» of the lungs, and obstructive syndrome. A chest x-ray revealed a decrease in the volume of the right half of the chest, depletion of the pulmonary pattern, «high» transparency of the right lung, and mediastinal shift. CT angiography of the chest revealed hypertransparency of the right lung, hypoplasia of the main right pulmonary artery and its branches, and multiple bronchiectasis. Thus, a rare diagnosis was made – McLeod syndrome. We have identified no description of such pathology in military personnel in the domestic literature.

The use of neurogymnastics in the process of medical and psychological rehabilitation of combatants

Abstract

The practice of medical support for troops in a special military operation indicates a high frequency of blast wounds and head injuries among combatants. In this regard, the problem of correcting the central regulation of integrative functions of the brain and restoring neuromotor skills after traumatic brain injury is extremely relevant. The article analyzes the experience of using neurogymnastics methods in a rehabilitation institution, structuring them according to the main areas of correction of the functions of the central nervous system that are relevant for the rehabilitation of combatants. The developed complex of neurogymnastics techniques allows, besides standard treatment and preventive measures, to more successfully restore impaired cognitive functions and psycho-emotional state in military personnel after returning from complex service assignments associated with psycho-emotional stress and traumatic influences. Neurogymnastics methods are also effective at the stage of restoration of the body of military personnel in this category during the period of sanatorium-resort treatment and complex rehabilitation.

Minimally invasive methods of treating patients with recurrent varicose veins of the lower extremities

Abstract

This work is based on a comparison of the results of treatment of patients with recurrent varicose veins, in 68 of whom minimally invasive methods were used and in 93, traditional interventions were performed through an open surgical approach. The safety, technical success, and clinical effectiveness of minimally invasive treatment methods were assessed. The results obtained indicate that minimally invasive techniques, in comparison with open surgical interventions, are more effective, safe, and less traumatic. Their use requires less time, makes it possible to carry out treatment outside the area of scar tissue, reduce postoperative pain, significantly reduce the number of early postoperative complications and the level of neurological disorders, and improve the cosmetic result.

Method of allograft fixation in pre-peritoneal repair of ventral hernias

Abstract

The aim of the study was to evaluate the reliability of allograft fixation and the effectiveness of postoperative purulent-inflammatory complications using the proposed new method for fixing a synthetic graft in pre-peritoneal ventral hernia repair. The proposed method was tested by us in 32 patients aged 29–75 years (main group). The dimensions of the hernial orifice did not exceed 3 cm in diameter, the dimensions of the hernial sac were 4–5 cm. 28 patients aged 31–74 years (control group) were operated on with transaponeurotic sutures in the standard way. According to the main clinical characteristics, both groups were comparable. Among the evaluated indicators, the most significant were: the absence of hernia recurrence within 1–5 years in both groups; absence of postoperative complications in the main group, in the control group – 2 cases (7.1%) of hematoma of the postoperative wound (differences are statistically significant); a significant decrease in the average length of stay in the hospital after surgery in the main group to 7±0.2 days (in the control group, 8±0.1 days).

Features of the course and therapy of urolithiasis in elderly patients

Abstract

The results of the analysis of data from domestic and foreign literature on the risk of development, the characteristics of the course, modern methods of diagnosis, and treatment of urolithiasis in elderly patients are presented. When choosing the tactics of treating urolithiasis in elderly patients, preference should be given to conservative methods of therapy or external shock wave lithotripsy. If surgical treatment is necessary for elderly patients taking anticoagulants and/or suffering from chronic renal failure, ureterorenoscopy with contact lithotripsy and lithoextraction may be the method of choice. Consideration should be given to the possibility of medical or surgical correction of urodynamic disorders of the lower urinary tract before using minimally invasive methods of treating urolithiasis. When choosing a method of treatment, it is necessary to take into account the concomitant pathology.

Abstract

The historical evolution of preventive vaccination calendars in the Russian Federation is analyzed. The article provides vaccination schedules and outlines prospects for improving vaccination calendars. An important achievement within the framework of military-civil cooperation is the inclusion in the vaccination calendars of the civilian population of vaccination of persons subject to conscription for military service against influenza, chicken pox, meningococcal, pneumococcal and new coronavirus infections. The system of immunoprophylaxis for military personnel is considered, which is closely related to the vaccination of the civilian population and is its continuation. A list of vaccinations given to military personnel when they are sent to provide assistance in zones of emergency situations and armed conflicts is provided. The organizational basis for carrying out immunoprophylaxis in the Armed Forces of the Russian Federation is presented. The main directions for improving the system of immunization of the population and military personnel have been identified.

Social and hygienic factors influencing the recruitment of female military personnel into the Armed Forces

Abstract

The implementation of the legally enshrined right of women to professional guidance as a military personnel is a complex issue due to such an unconditional circumstance as the need to participate in combat operations (elimination of emergency situations) to protect the country. There is every reason to position female military personnel as a de facto personnel resource for the Armed Forces, which will have to find the most rational application. To do this, it is necessary, on the one hand, to remove all restrictions on the military-professional orientation of females, on the other hand, the number of female military personnel in the total number of the army and navy should be limited, in particular, by introducing initially higher requirements for their physical fitness. It is advisable to determine the replacement of military positions by women depending on the conditions of military service at specific workplaces, as well as on the effectiveness of the implementation of additional preventive measures developed to minimize the effect of harmful and (or) dangerous factors of military service on women’s health (social guarantees).

Mastering the method of intense diving in marine conditions: medical and physiological studies

Abstract

The article provides information on the performance of diving work using the method of intense dives in marine conditions and defines the role and place of medical and physiological studies of the body functions of aquanauts during a long stay at great depths. Particular attention is paid to the prevention of diving diseases, measures to prevent infectious and inflammatory pathologies are reflected. The work and rest regime of aquanauts in marine conditions has been tested. Based on the experience gained, recommendations are given for the rational organization of the conduct and medical support of diving work at depths that are extreme for humans. The results of experimental diving descents show the reliability and high efficiency of the saturation diving method and the possibility of introducing it into the defense sector, including for rescuing personnel of damaged submarines and carrying out emergency work at emergency facilities.

Efficiency of a biomechanical robot used in complex treatment of humeroscapular periarthritis

Abstract

The use of combined physiotherapy and a robotic system in the outpatient treatment of glenohumeral periarthritis is considered. According to the results of a study of 45 men and 15 women aged 18–60 years after combined physiotherapy, positive dynamics in the form of an increase in range of motion was observed in the 7–9th session. With a robotic complex for rehabilitation – by the 3-4th session. The inclusion of physiotherapeutic treatment using a biomechanical robot in the complex therapy of glenohumeral periarthritis more effectively reduced the severity of pain and contributed to an early increase in the range of motion in the shoulder joint to a greater extent compared with combined physiotherapy.

From the history of the military holiday home «Podmoskovye»

Abstract

The branch «Rest House “Podmoskovye”» of the sanatorium and resort complex «Podmoskovye» of the Russian Ministry of Defense celebrated its 55th anniversary in 2023. Today it is an actively developing health resort for recreation and rehabilitation of military personnel, military pensioners and youth with 800 beds. The holiday home is located in the near Moscow region. On an area of more than 14 hectares, 22 buildings and structures were erected - two five-story dormitory buildings and a one-story building housing a dining room, banquet halls, a cafe, a cinema, concert and conference halls, and a medical center. The holiday home has its own equipped beach with a boat station on the shore of the Klyazma Reservoir. More than 7,000 people take organized holidays here every year. First, these are military personnel, persons discharged from military service, members of their families, and civilian personnel from the Armed Forces of the Russian Federation.
