Abstract

The current article deals with the main methods of development of the rehabilitation system for wounded during the late hospital stage. The problems of specialized rehabilitation for injured, patients of cardiology and neurology and the application of the newest rehabilitation techniques used to optimize medical aid in order to promote professional qualities recovery experience are also considered.

Improvement of medical service administration for wounded in road accidents in Moscow

Abstract

The article is devoted to the problem of reducing the severity of consequences of the road traffic. This problem refers to the actual intersectoral problems including medical service. The author showed that the main way to improve the medical service administration for the wounded in road accidents is to reduce time of the on-the-scene care and arrival of the wounded at the hospitals. A few ways of improvement of work of First Aid Teams are suggested. The author came to conclusion that every of the incorporated organizational forms improves the effectiveness of emergency medical service for the wounded in road accidents to some extent, but only joint introduction of these forms and creation of united organization-management system are the real ways of getting optimum results.

Psychophysiologi-cal monitoring of professionally important qualities of military physicians

Abstract

Due to reforming of a system of military-medical education, percent of female soldiers in medical service has increased. Psychophysiological peculiarities of this military personnel class have an impact on a military service quality. It has been established that professional success of military physician is governed by mentality, social and psychological adaptation and military-professional motivation. For this reason, medical service recruiting should be realized on the basis of scientifically grounded series of measures of professionally psychological selection. It is shown that it is necessary to elaborate measures for selection and evaluation of professionally important qualities for female military physicians recruiting. Authors suggested the system of ranking of professional activity success, which can be a methodological basis for acceptance of scientifically grounded solutions about the administration of current medical personnel.

Modern paradigm of reconstruction of anterior cruciate ligament in military personnel

Abstract

Analyzed data from annual reports of major trauma districts, chiefs of departments of hospitals and centers and reviewed the frequency and structure of surgical interventions on the knee, made in the last 5 years (2005—2010). In health care facilities the Defense Ministry. The frequency of the most typical injuries and diseases of the knee joint in different categories of servicemen and revealed the leading role the anterior cruciate ligament rupture. Analyzed stages of the evolution of anterior cruciate ligament reconstruction techniques in a specialized hospital department artrologicheskom Military Traumatology and Orthopedics MMA and a comparative characteristic of modern surgical approaches, providing anatomical and isometric position of the graft, as well as the minimal invasiveness of intervention with preservation of the sources of blood supply and potential revascularization autograft. Makes proposals for improving the organization of treatment of surgical pathology at the knee joint military operation and orthopedic trauma and military departments of medical institutions.

Abstract

On the basis of 42 analyzed sources of literature was performed a study about the problem of comprehensive training of patients with ischemic heart disease combined with type 2 diabetes mellitus which undergo interventional coronary intervention. It was noted that disease management programs for patients with associated pathology and comprehensive training of patients with ischemic heart disease combined with type 2 diabetes mellitus, which undergo interventional coronary intervention, are not developed. Therefore, in recent decades there emerged a need for comprehensive training of patients with associated pathology development of relevant structured programs of training, estimation of near and late results of training.

Application of device for local compression of injured magistral arteries of extremities

Abstract

External hemorrhage of extremities wounds is the leading cause of soldiers’ death on the battlefield. In these cases control of massive arterial bleeding require not only prompt and effective measures, but also safety procedure. We assessed on volunteers the effectiveness, application time and pain intensity during the use of construction powerful quick-grip one-handed bar clamp. In results we found that the use of improvised quick-grip clamp for hemorrhage control in axillary and popliteal areas stops arterial blood flow in an extremity in all cases proven by Doppler ultrasound examination. Application time in axillary zone was 15,3±5,2 sec, in popliteal area — 27,3±8,0 sec. In the groin area, the use of this improvised device was not effective due to technical characteristics (small distance between the main frame and the pressure surfaces). There do appear to be sufficient reasons to design the universal clamp for hemorrhage control from the wounds of junctional zones as well as proximal segments of extremities on Advanced Trauma Management stage and also for civilian prehospital emergency care.

Hypertensive disease: diagnostics and differential methods of antihypertensive therapy

Abstract

Diagnostics of hypertensive disease is performed according two-stage scheme recommended by Society of cardiology of Russian Federation (2010) and adapted by us for examination in Central Military Clinical Hospital n. a. N.N.Burdenko. First stage, as possible, should be carried out in out-patient departments, second stage, as it necessary, in specialized medical treatment facilities. Indications for admission of patient with hypertensive disease are abstrusity of diagnosis and necessity of carrying out of special, often invasive study, for updating of information about the form of arterial hypertension, difficulties in adjustment of antihypertensive therapy. Major criterion for adjustment of antihypertensive therapy is degree of cardiovascular risk. In the beginning of treatment monotherapy is preferable for patients with low and average risk of progression of cardiovascular disease, use of combination of two or three medications in small doses is reasonable for patients with high and very high risk of complications. Three medications were prescribed for 21% of patients, four or five for 14% of patients.

About non-specific prophylaxis of infections typical for organized groups

Abstract

Risk of progression of these infections is connected with activation of carry-over of causative agents among military personnel during replacement and decrease in immunity of conscripts. Usage of medication «Karmolis Kapli» for the purpose of prophylaxis allows to reduce cases of respiratory tract infections among the military personnel. Among the military servicemen who had take «Karmolis Kapli» was noted reduction of community-acquired pneumonia, tonsillitis and pyodermia morbidity. Non-specific protective effect of medication «Karmolis Kapli» is conditioned by increase of the common resistance of the body. For the purpose of prophylaxis it is necessary to use this medication during the personnel formation before the beginning of seasonal morbidity.

The following units are successively presented: the Defense Medical Research and Development Program, Uniformed Services University of the Health Sciences, Center for the Study of Traumatic Stress, Armed Forces Radiobiology Research Institute. The particular research programs conducting in the above mentioned institutions are presented.
