In order to improve the methods of primary surgical treatment of craniocerebral gunshot wounds with active drainage methods analysed the experience of treating 127 wounded with acute brain and skull gunshot injuries in a specialized army hospital from May 1980 to July 1982, that was located in Afghanistan during the conduct of hostilities. It is concluded that the active drainage of cranial wounds enables to carry out the removal of detritus accumulating in the wound, which promotes the favourable course of wound healing. The use of non-epileptogenic antibiotics during wound irrigation in conjunction with intramuscular, intravenous, intracarotid and endolumbal injections can significantly reduce the number of infectious complications of traumatic brain injuries. Active drainage method allows you to complete primary surgical treatment of the weld overlay deaf on stage and prevent secondary infections in these surgical wounds
