Aging and liver pathology: molecular and cell aspects

Abstract

Age-associated liver disease is one of the leading causes of reduced ability to work and the development of polymorbidity syndrome. The incidence of nonalcoholic fatty liver disease (NAFLD) and hepatitis increases with age and is about 20% in the working-age population of developed countries. The purpose of the review is to analyze the molecular mechanisms of development of age-related liver involution in normal and pathological conditions and to search for promising methods of differential diagnosis. The review describes the role of cell aging proteins and apoptosis (p16lnk4a, p21, p53) and factors that regulate the hepatocyte cell cycle (Cdk1, Skp2, Ccne2, pRb, survivin, Ssu72) in the development of liver pathology. Blood plasma proteins (CYP450, GLDH, SDH, K18, GST), sulfitoxidase, cytokeratin 18, etc. are considered as new markers of liver damage. Serum triglycerides, miRNA-122, miRNA-10b, miRNA-33 are specific biomarkers of NAFLD also methylation status MT-ND6. A possible way of steatohepatitis diagnostic to influence the signaling of FXR, FGF19, PPARα, YAP / TAZ. It is assumed that the targets for the action of new generation hepatoprotectors in case of age-related liver pathology may be cytokines (TGF, TNF, IL-6) and vascular endothelial growth factor (VEGF).

What conclusions should be drawn from the association between liver diseases and cholelithiasis?

Abstract

The authors reviewed current data, which could establish a pronounced association between the prevalence of cholelithiasis and liver diseases. A century-and-a-half discussion about the prevalence of the basic concepts of the pathogenesis of cholelithiasis led to the unification of metabolic and infectious theories. Currently, surgical tactics involving cholecystectomy in individuals with symptomatic or complicated cholelithiasis predominates in an approach to treating gallstones. A multidisciplinary view on the diagnosis, pathogenesis, treatment, and prevention of cholelithiasis and the recognition of a close relationship between the functional state and pathology of the liver and biliary tract justifies the feasibility of a thorough diagnosis of pathology of the liver and correction of its functional state in patients with cholelithiasis. Ursodeoxycholic acid is a good example of therapy that improves both the functional state of the liver and that of the biliary tract.

Chronic kidney disease as a proinflammatory syndrome

Abstract

There is a steady rise in the number of patients with chronic kidney disease (CKD) in the general population, which determines the relevance of studying its pathogenetic mechanisms. The proinflammatory syndrome in CKD is now recognized as a factor for high cardiovascular morbidity and mortality rates in this category of patients. Systemic inflammation is one of the nontraditional cardiac risk factors; however, its role in CKD has not been fully studied; investigations in this area are being continued. The effect of uremic toxins in the initiation and progression of inflammation at various stages of CKD has been inadequately studied. The authors consider the pathophysiological mechanisms of inflammation and its role in complications in patients with CKD and set forth a modern view of the problem associated with the development of CKD complications that result from chronic systemic inflammation. Studying the mechanisms of development and diagnosis of systemic inflammation will be able to apply a differentiated approach to assessing the cardiac risk and treating patients with CKD. Investigations in this direction seem extremely promising.

The aging process affects the structure and function of various organs and systems. The adrenal glands are also subject to age-related changes. Questions: hormones and aging, what is primary and what is secondary, where is the cause, and where is the consequence - scientists have been worried for a long time. The lack of answers to these questions makes it relevant to study age-related changes in the adrenal glands and their impact on the work of various organs and systems Purpose: to study the effect of age-related changes in the adrenal glands on the work of various organs and systems. Materials and methods: analysis of literature data on search words - aging, hormones, endocrine system, old age, senile age, adrenal glands, hypothalamus, pituitary gland glucocorticoids, dehydroepiandrosterone for 2000-2019 in computer databases: PubMed, Scopus, Medical-Science, Elibrary, Web of Science, Ceeol. Results. Normal aging leads to changes in the activity of the axis of the hypothalamus-pituitary-adrenal gland (HPA) and the production of adrenal hormones. Significant is an increase in the average daily levels of cortisol in the serum of older people with changes in the circadian secretion rhythm. Excess glucocorticoids in the elderly can affect the structural integrity and function of various areas of the brain, associated with muscle loss, hypertension, osteopenia, visceral obesity, and diabetes mellitus. In contrast, levels of other adrenal cortical hormones (aldosterone, dehydroepiandrosterone) in the elderly have been proven to be reduced. The main mechanisms for their reduction remain unclear. Catecholamine secretion is also subject to age-related changes. Conclusion. In the process of aging, numerous structural and functional changes in the adrenal glands occur, which affects the functioning of many organs and systems.

Age features of collagen exchange markers in patients with heart failure with intermediate emission faction on the background of metabolic syndrome

Abstract

In pathological conditions, the activity of matrix metalloproteinases (MMP) increases, which leads to an imbalance between the synthesis and degradation of the extracellular matrix, which provokes the development of fibrosis in the myocardium, which contributes to the occurrence of heart failure. The aim of the study. To evaluate the features of collagen metabolism in elderly patients with chronic heart failure, an intermediate ejection fraction of heart failure with mid-range ejection fraction (HFmrEF) against the background of metabolic syndrome (MS). Material and methods. The study included 180 patients with HFmrEF (40-49%), functional class II according to NYHA (NYHA class II) of ischemic origin. We assessed the age of patients, the presence of MS, and levels of tissue inhibitor metalloproteinases tissue inhibitor metalloproteinas (TIMP-1), MMP-1, MMP-9. Results. Depending on the presence of MS in patients with HFmrEF, there was a significant (p<0.05) increase in MMP-9 with increasing age. The level of MMP-1 in the elderly late adulthood and senile gerontic age significantly increased with age in the presence of MS (p<0.05). In senile patients, the level of TIMP-1 in the group with MS was 13% lower (p<0.05) than without MS. In all age groups, the ratio of MMP-9/TIMP-1 in the presence of MS was higher (p<0.05) than without MS. Conclusions. Increased expression of MMP-9 and decreased expression of TIMP-1 in old age contribute to myocardial remodeling and fibrosis myocardium fibrosis, thereby changing the extracellular matrix of myocardial tissue leading to progression of heart failure. Metabolic syndrome significantly worsens the course of heart failure in the elderly and senile age, a mismatch between the degree of increase in the level of MMP-9 and TIMP-1 is characteristic.

Plant sorbents in combination anthelmintic therapy

Abstract

The task has been completed by choosing and using sorbents in the dehelmintization process. Objective: to determine the efficiency of detoxification in patients diagnosed with helmin-thiases. To study the combined use of anthelmintic agents and plant sorbents for endotoxemia elimination and in the dehelmintization process. Subjects and methods. There were a total of 180 patients, including 41% with skin allergy and 59% with gastrointestinal tract diseases. The patients took nemozole tablets for dehelmintization. They used in parallel plant enterosorbents, such as bran, zosterin-ultra, beet root powder. Enterosgel was used as a control. Fecal changes were monitored by histological and coprological surveys. Results and discussion. Pectins have a fairly potent sorption activity and, unlike the control (enterosgel), antibacterial, therapeutic, and nutritional effects. The use of enterosorbents is mandatory in all patients before and during dehelmintization. Patients with a family history of allergy should additionally take antihistamines.

Abstract

The object of the following research is to study the incidence of the most widespread geriatric syndromes (frailty, syndrome malnutrition) advanced age and senility patients are suffering - in structure of rural dwellers and city dwellers population in Smolensk region. Health status of 340 patients was examined by the agency of Comprehensive geriatric assessment (CGA), case histories and outpatients cards were analyzed. Particular patients, supposedly suffering from frailty, were selected according to screening scale «Age is not a hindrance» (total score 3 and beyond). Nutrition status was examined by the agency of Mini nutritional assessment (MNA). In this article the geriatric syndromes and malnutrition (nutritional disorder) incidence research results in structure of advanced age and senility patients in Smolensk region population were represented, also the part of socio-economic status in conjunction with malnutrition syndrome genesis was studied and the comparative characteristics of Barthel Index figures of various age groups was accomplished.

The coronavirus pandemic: the life of elderly people under lockdown

Due to the spread of the novel coronavirus infection in Russia, people over 65 years of age are advised to observe a lockdown regime, as in the patients older than 65 years, the new coronavirus infection generally occurs severely and often leads to death. The complications are pneumonia, resulting in pulmonary edema. The article provides simple exercises to maintain physical activity for patients older than 65 years who are under lockdown. It gives recommendations (including affordable digital technologies - the World Health Organization Integrated Care for Older People (ICOPE) Handbook App) for primary care physicians working with these patients.
