Russia’s first experience with GLE/PIB for the treatment of HCV infection genotype 3A with cryoglobulinemia and concomitant HBV infection in a hemodialysis patient with CKD C5d awaiting kidney transplantation

Abstract

The paper presents Russia’s first case of successful use of a 8-week therapy course using an original interferon-free combination of the third-generation NS3/4A protease inhibitor glecaprevir (GLE) and the second-generation NS5A-replication enzyme inhibitor pibrentasvir (PIB) in clinical practice to treat HCV infection genotype 3A with mixed cryoglobulinemia syndrome and concomitant HBV infection in a patient with chronic kidney disease C5d, who received chronic dialysis after elective bilateral nephrectomy for renal polycystosis and awaited donor kidney transplantation.

Abstract

The colon microbiota was investigated in patients with chronic kidney disease (CKD) and manifestations of renal failure. Colon dysbiosis was detected in all the patients, which was manifested predominantly by the proteolytic flora of enteropathogenic Escherichia coli, Enterobacter spp/Citrobacter spp., and clinically by the symptoms of intestinal dyspepsia (symptoms of tympanism and constipation). The use of Laminolact in CKD patients normalizes the colon microbiota and reduces the manifestations of renal failure.

Abstract

One of the common causes of worse quality of life and disability in the active population of almost all ages is dry eye syndrome (DES), a set of corneal and conjunctival changes caused by systematic tear film destabilization. In some cases DES causes severe changes in the cornea - its irreversible opacities, ulcerations, and even perforation. DES treatment and prevention with drugs that moisturize the ocular surface is the therapy of choice. There are a significant amount of tear substitutes on the market, but when making a choice, it is important to remember about the need for long-term treatment of the ocular surface and the safety of the components of an eye medication.

Abstract

Clinical and endoscopic comparisons were carried out in 156 patients with identified atelectasis. Bronchoscopy showed that 68.6% of cases had atelectasis due to bronchial obturation with a tumor, secretory plug or foreign body; 16.67% of cases were noted to have bronchial compression caused by external compression due to (as brought to light later) peribronchial tumor growth or to have bronchial compression due to enlarged lymph nodes. Bronchoscopy revealed no abnormalities in 14.7% of cases; during treatment these patients were found to have community-acquired pneumonia (12.2%) and infiltrative pulmonary tuberculosis (2.56%).

Oligosecretory monoclonal gammopathy is the result of the hyperproduction of a small proliferating cell clone, which, after various periods, can transform into overt secreting hematological tumors, but may be a cause of non-tumor organ and tissue damage, including kidney disease. The frequency, severity, and spectrum of monoclonal gammopathy in patients from different departments of a multidisciplinary therapeutic hospital have been studied. The nosological characteristics of diseases associated with monoclonal gamma of undetermined significance (MGUS) are given; special attention is paid to kidney diseases, including immune-mediated diseases, the association of which to MGUS has been least studied.

Today, recombinant prourokinase is an indispensable Russian enzyme preparation in ophthalmology. Gemase, the active ingredient of which is a human recombinant prourokinase, is capable of quickly eliminating massive hemorrhages into the vitreous body, retina and anterior chamber of the eye. The drug is widely used in hemorrhagic forms of diabetic retinopathy, hypertensive retinopathy, and age-related macular degeneration with a hemorrhagic component. It is effective in treating eye injuries, postoperative fibrinoid syndromes, thrombosis of the central retinal vein and its branches, occlusions of the central retinal artery and its branches.
