ENDOSCOPIC TREATMENT OF TRACHEAL CICATRICIAL STENOSES
Abstract
Treatment of tracheal cicatricial stenoses is one of the most difficult problems of modern medicine. There are two main therapeutic approaches - endoscopic (bronchoscopic) and surgical (open). This paper describes the methods of endoscopic treatment of stenoses allowing achieving complete recovery of tracheal lumen in some cases. The scheme of treatment for tracheal cicatricial stenoses is also written.

CURRENT POSSIBILITIES OF ANTI-INFLAMMATORY THERAPY IN PULMONOLOGY
Abstract
The paper gives updates on a basically new direction in anti-inflammatory therapy with the phosphodiesterase 4 inhibitor roflumilast in patients with chronic obstructive pulmonary disease. It describes the mechanisms of action of the drug and indications for its use. The results of clinical trials and the possibility of using roflumilast for other lung diseases are described.

SSHIFTS IN THE HEMOSTATIC SYSTEM ARE A COMPONENT OF SYSTEMIC INFLAMMATORY RESPONSE SYNDROME IN PULMONARY TUBERCULOSIS
Abstract
Patients with pulmonary tuberculosis have been found to have significant systemic inflammatory response syndrome (SIRS) and its associated hypercoagulation shift in the hemostatic system. In the cellular component, it appears as thrombocytosis and increased platelet responsiveness. In the plasma component, there are increasing concentrations of thrombogenic proteins in inadequate fibrinolysis. The degree of hypercoagulation syndrome is directly related to the severity of SIRS, but usually fails to result in multiple organ dysfunction and/or hemorrhage due to consumption coagulopathy.

NEW INHALED GLUCOCORTICOSTEROIDS FOR ASTMA
Abstract
Moscow Inhaled glucocorticosteroids (ICS) are the therapy basis for persistent asthma of any degree for the long-term control of inflammatory processes. Ciclesonide (Alvesco) is a new ICS for Russian practice is a pro-drug that is activated by the lung esterases and characterized by high anti-inflammatory properties and excellent safety profile because of low oral bioavailability, rapid elimination, high plasma protein binding. The drug affects the whole bronchial tree due to small inhaled particles.

FLUOROQUINOLONES IN THE TREATMENT OF URINARY TRACT INFECTIONS: WHAT CHANGES THE ANTIBIOTIC RESISTANCE OF UROPATHOGENS?
Abstract
The clinical and microbiological efficacy of fluoroquinolones at a <20% resistance can be maintained by producing their high concentration (well above the serum ones). Fluoroquinolones (mainly levofloxacin) in high doses have been shown to be effective in the majority of patients with urinary tract infections caused by fluoroquinolone-resistant bacterial strains if their minimal inhibitory concentration is not greater than 32 mg/l.

All-Russian Institute of Medicinal and Aromatic Plants, Moscow Sanguiritrin has a broad-spectrum antimicrobial activity and is active against antibiotic-resistant microbial strains. Clinical trials has demonstrated the high efficacy and safety of Sanguiritrin used as a topical (aqueous-alcoholic solution, ligament) and systemic resorptive (enteric-coated tablets) agent.

ESTIMATION OF ALTERED QUALITY OF LIFE IN PATIENTS WITH ASTHMA DURING LONG-TERM TREATMENT WITH TRAINING
Abstract
Long-term treatment for asthma requires continuous monitoring of its course and the efficiency of therapy performed in trained patients. A patients quality of life (QL) is one of the criteria for evaluating the effectiveness of therapeutic educational programs. A follow-up study of a set of criteria (the values of all scales) of the Saint George’s respiratory questionnaire shows a substantial QL improvement in the trained patients.

CAPACITIES OF JET VENTILATION IN THE ANESTHETIC MAINTENANCE OF ENDOSCOPIC SURGERY OF THE LARYNX
Abstract
The paper presents the capacities of using jet ventilation as a component of anesthetic maintenance during endoscopic microsurgical interventions into the laryngeal lumen. It shows the dissimilarity of jetwise ventilation from traditional one and gives the characteristics of major settings, as well as explanations of their impact on pulmonary gas exchange. The capacities of current monitoring methods, probable complications, and preventive measures are considered to assure a patients safety when different modes of jetwise ventilation are applied.

ENDOSCOPIC ENDONASAL TRANSSPHENOIDAL SURGERY FOR PITUITARY ADENOMAS
Abstract
The analysis of the results of treating 1700 patients with pituitary adenomas, operated on at the Acad. N.N. Burdenko Neurosurgery Research Institute in the past 7 years, by using an endoscopic endonasal adenomectomy procedure, has shown that the latter has some advantages over a microscopic access. Practical recommendations are given and algorithms for a surgeon’s actions in different clinical situations are described.

ENDOPROSTHETIC REPLACEMENT OF THE KNEE JOINT IN ITS MASSIVE DESTRUCTIONS IN ORTHOPEDIC PATIENTS
Abstract
The paper considers rather rare cases of prosthetic knee joint replacement with oncoprosthetic devices. The indications for such surgical interventions are severe juxta-articular bone destructions concurrent with the clinical and X-ray pattern of Stage III gonarthrosis.
