The paper summarizes data on the most basic features of the pathomorphogenesis of nonalcoholic fatty liver disease in patients with diabetes mellitus (DM). The author provides his view of the possibilities of current hepatoprotective therapy for fatty hepatosis and steatohepatitis in DM. There is evidence that there is a need for the combined use of hepatoprotective drugs, prominent among which are thioctic (а-lipoic) acid preparations in combination with other medicaments.

Abstract

The paper describes the physiological mechanisms of magnesium involvement in the function of different human body systems at the level of a cell and subcellular structures. Magnesium deficiency, the incidence of which in clinical practice is as high as 70% and which appears as cardiovascular, cerebral, visceral, and muscular-tetanic syndromes, shows a clinical diversity. The pathophysiological mechanisms responsible for the implication of magnesium deficiency in the formation of insulin resistance (IR) as a key component of metabolic syndrome (MS) are discussed in detail. The authors present the results of combination treatment incorporating magnesium preparations (Magnerot 3 g/day) in 69 patients with MS. They also give the data of an analysis of the time course of changes in IR values, carbohydrate and lipid metabolic parameters, and blood rheological spectra.

Abstract

Forty-three patients (27 men and 16 women; mean age 63.24 years) with coronary heart disease (CHD) were examined to assess the clinical and pharmacoeconomic aspects of the use of the If-channel inhibitor ivabradine as part of combination therapy from the data of ECG monitoring, treadmill test, and quality of life (QL) questionnaires. Ivabradine produces anti-ischemic activity, increases the volume and time of fulfilled exercise, reduces heart rate, and improves QL in patients with CHD and stable angina on exertion. The combination therapy using ivabradine is economically more advantageous than the standard therapy without this drug.

ANTIOXIDANTS IN THE TREATMENT PROGRAM FOR POLYMORBID PATIENTS WITH METABOLIC SYNDROME

Abstract

Using the data of a review of the medical research literature and their own results of the examination and treatment of patients with a body mass index of ≥2 5 kg/m 2 as the basis, the authors show the negative role of oxidative stress in forming insulin resistance (IR) in early metabolic syndrome (MS). The combination treatment of 54 patients with MS incorporated the antioxidants: 60 mg coenzyme Q 10 + 9 mg vitamin E daily. The study has shown that there is a close correlation between the levels of IR and the markers of oxidative stress. The restored capacity of the antioxidant system (coenzyme Q 10 + vitamin E) promotes the effective recovery of sensitivity of insulin receptors to insulin in the early phases of obesity.

ВОЗ определяет ситуацию с сахарным диабетом (СД) как эпидемию неинфекционного заболевания. В настоящее время в мире насчитывается 130 млн больных СД и каждые 25 лет их число удваивается. В статье обсуждается проблема диабетической полинейропатии у больных СД типа 2 (СД2). Обследованы 130 пациентов с СД2 без признаков полинейропатии. При наблюдении в течение 3 лет у 47% больных развилась полинейропатия. Проведен дискриминантный анализ, выявлены прогностически значимые факторы риска, выведена формула прогноза развития заболевания.
