TREATMENT FOR HYPOSPADIAS IN CHILDREN

Hodgson-I-III, Duckett, Duplay, onlay plasties, etc. are the most common procedures for repairing the urethra in children with hypospadias. The paper considers the epidemiology of this abnormality, indications for its treatment, by taking into account the forms of the disease, and analyzes complications of different operations for hypospadias and ways of their prevention.

VARICOCELE: STATE-OF-THE-ART
Abstract
According to the data available in the literature, the rate of varicocele in the general population is 10-30%. The prevalence of the disease increases with age and may be as great as 75-77,3%. With the introduction of current studies, almost 80% of cases of varicocele are found to have it bilaterally. Bilateral varicocele leads to hypotrophy of the testes and their considerably reduced volume regardless of the degree of the disease. Varicocele is a cause of primary and secondary male infertility in 40 and 80% of cases, respectively. The bilateral process can cause severe oligoasthenoteratozoospermia, including azoospermia. Sperm DNA fragmentation is more marked in men with varicocele. If untreated, varicocele may give rise to infertility even in those who have normal values of a spermogram. Microsurgical subvaginal varicocelectomy described by Marmara is the gold standard for treating patients with varicocele. Many works have provided support for the positive impact of varicocelectomy on the likelihood of spontaneous conception in the partners of patients, by improving their spermogram, reducing sperm DNA damage, normalizing blood hormone levels, elaborating fewer reactive oxygen species, and recovering Leydig cell function irrespective of patient age.

POTASSIUM AND MAGNESIUM DEFICIENCY AS A CONSEQUENCE OF DIARRHEA AND INTESTINAL DYSBIOSIS: MECHANISMS OF DEVELOPMENT AND WAYS OF CORRECTION
Abstract
The paper gives data on the mechanisms for the development of potassium and magnesium deficiency as a consequence of diarrhea and colonic dysbiotic changes. It provides evidence that electrolyte deficiency should be corrected to stop loose stools and to recover the intestinal microflora. Combination drugs containing both potassium and magnesium (such as Panangin) are given to achieve a significant positive effect.

IRRITABLE BOWEL SYNDROME
Abstract
The paper gives a concise review of the present views of the pathogenesis of irritable bowel syndrome (IBS), its clinical symptoms and algorithms for its diagnosis and differential diagnosis. It emphasizes the importance of excluding intestinal diseases that have a certain morphological substrate and that run with IBS-like symptoms. The main groups of non-drug methods and current agents used to treat IBS with diarrhea or constipation are presented; medications that are undergoing clinical trials are also indicated. There is a concept of bowel training in which regular defecation may be formed during a rather long-term course of therapy with a laxative such as polyethylene glycol 4000 (Forlax). Neuroplasticity is likely to underlie this action.

DYNAMIC MULTIDETECTOR SPIRAL COMPUTED TOMOGRAPHY IN THE PRIMARY DIAGNOSIS OF GASTRIC CANCER
Abstract
In 2009 to 2010, multidetector computed tomography (CT) (MSCT) constructing multiplanar reformations and dynamic video files was performed in 49 patients with gastric cancer (GC). Dynamic MSCT was carried out using a 320-slice dynamic volume Toshiba CT scanner with a 16-cm detector width. Sixteen patients underwent virtual endoscopy. Dynamic MSCT could detect gastric tumor lesion in 47 of the 49 patients. The sensitivity of MSCT for the detection of GC was 96% and its specificity was l00%. The possibility of detecting GC depends on the depth of tumor invasion: it was 73 and 100% in T1 and more disseminated forms, respectively. Virtual endoscopic study is characterized by a high image quality; the latter in intraluminal tumor growth is highly competitive with that of traditional video-assisted endoscopy. The capacities of using dynamic MSCT, including that in the virtual endoscopic view as a screening method, call for further investigation.

SPASMOLYTICS IN THE TREATMENT OF BILIARY PAIN
Abstract
Agents that selectively affect gastrointestinal tract smooth muscles, such as mebeverine, pinaverium bromide, are preferred to relieve abdominal pain in biliary pathology. The use of mebeverine (Duspataline®) in patients with biliary tract disease promotes a stable reduction in the magnitude of abdominal pain, disappearance of dyspeptic signs, improvement or normalization of an ultrasonographic pattern and laboratory indices.

USE OF BUTYRIC ACID IN THE TREATMENT OF NONSPECIFIC ULCERATIVE COLITIS IN CHILDREN

The key mechanism of the pathogenesis of intestinal inflammatory diseases is the failed immu-nological tolerance to intestinal antigens, above all microbial and food antigens, causing the im-munological control over a progressive intestinal inflammatory process to be lost. In this connec-tion, it is relevant to use combination drugs containing a prebiotic component along with short-chain fatty acids, particularly butyric acid that exerts a cytoprotective effect. Zacofalk®NMX is shown to be effective in using as an adjunct to therapy in children with nonspecific ulcerative colitis.

IRON (III) HYDROXIDE POLYMALTOSE COMPLEX, A NEW-GENERATION DRUG, FOR THE TREATMENT OF IRON-DEFICIENCY ANEMIA

The paper gives the results of preclinical and clinical studies evaluating the efficacy and safety of iron (III) polymaltose complex in the treatment of iron deficiency with and without anemia being developed. The adverse reactions of the complex were generally the same or less pronounced than those of iron salts. The complex may be used for the treatment and prevention of iron-deficiency anemia in all patient groups.

BIOFEEDBACK PELVIC MUSCLE TRAINING IN PATIENTS WITH URINARY INCONTINENCE FOLLOWING RADICAL PROSTATECTOMY
Abstract
Seventy-five patients aged 54 to 72 years were followed up. All the patients had biofeedback pelvic muscle training using an electromyogram. It was actual for 37 patients to restore intimacy, 14 planned to do this as soon as possible and they were given a phosphodiesterase type 5 inhibitor (tadalafil 5 mg daily) and 23 intended to resume their sexual life as urinary continence recovered. The median time to recover urinary continence while training the pelvic muscles and using cialis 5 mg was 3.1 months and that without using tadalafil was 5.4 months. In patients who did not plan to restore sexual function, this index was 7,6 months.

ANDROGEN SCREENING IN MEN WITH PROSTATE DISEASES
Abstract
The adverse impact of androgen deficiency on males determines the need for mandatory androgen screening after 50 years of age. Testosterone levels were measured in 454 men aged 20 to 87 years (mean age 60,5 years). One hundred and ninety-six (43,2%) men with age-related hypogonadism were identified. A diagnostic algorithm for age-related androgen deficiency (ARAD) in men was proposed. The high prevalence of prostate diseases and a number of somatic abnormalities was determined in the ARAD group.

PRIMARY ENDOPROSTHETIC REPLACEMENT OF THE HIP JOINT USING TRABECULAR TANTALUM TROCHANTERIC COMPONENTS

The paper presents the results of total endoprosthesic replacement of the hip joint using trabecular tantalum trochanteric components. The study is based on 358 operations performed in 2008-2012. The authors consider indications for surgery; the surgical results are traced over 5 years; there is no case of instability of the cup of a tantalum-coated endoprosthesis.

MINI-INVASIVE TREATMENT FOR ABSCESSING MASTITIS IN GIRLS
Abstract
The paper gives the results of treatment in 57 girls aged 11-15 years, admitted to hospital for acute mastitis. Treatment policy has been determined depending on the form of mastitis. Analysis of the results has shown the efficiency of an echographically guided puncture drainage method recommended by the authors for the treatment of destructive forms of breast inflammation.
