NEUROPSYCHIATRIST AND PSYCHOSOMATICS. CURRENT TRENDS
Abstract
We have presented two psychosomatic models that are not associated with «psychogenesis» or «comorbidity». Depression is often a mental syndrome of physical illness. Obesity is consistent with the concept of «psychosomatic illness». Practical recommendations on the use of two models of psychosomatic are available.

OCCUPATIONAL CHRONIC LUMBOSACRAL RADICULOPATHY: CURRENT VIEWS AND DIFFERENTIAL DIAGNOSIS
Abstract
Chronic lumbosacral radiculopathy is an occupational disease associated with long (at least 10-year) systemic static muscle tension, forced position of the trunk or limbs, considerable physical strain, forced posture, frequent deep torso bends at work, prolonged sitting or standing in a fixed working posture, and an inconvenient fixed working posture. The clinical and diagnostic features of chronic lumbosacral radiculopathy and the characteristics of pharmacotherapy are described.

Abstract
The paper considers the clinical situations in which the entire small bowel (SB) has to be examined most frequently; it presents the evolution of enteroscopy techniques that can make a lifetime analysis of the internal structure of the longest part of the human body, as well as its associated problems. Endoscopic techniques for diagnosing SB diseases, such as video capsule and balloon-assisted enteroscopy, indications for and contraindications to their use, their importance for clinical practice and their effectiveness are depicted in detail.

APOPTOSIS AND MALE FERTILITY
Abstract
The authors give the results of their and other authors’ investigations, which demonstrate the negative impact of apoptosis on sperm quality. In subfebrile males, the amount of ejaculated apoptotic spermatozoa correlates with their concentration, mobility, and morphological defects. The presently accumulated facts allow one to regard the laboratory signs of apoptosis (phosphatidylserine externalization, Fas expression on the cells, DNA fragmentation) as factors that decrease sperm fertility.

TWO DISEASES: ONE DRUG (ACE INHIBITORS IN THE TREATMENT OF HYPERTENSION IN PATIENTS WITH CORONARY HEART DISEASE)
Abstract
Numerous controlled trials have demonstrated that ACE inhibitors are highly effective in reducing the cardiovascular risk in the treatment of patients with hypertension and coronary heart disease (CHD). The benefits of Prestarium A are the stabilization of its antihypertensive effect during 24 hours, including morning hours when a rapid elevation of blood pressure and high circadian frequency of ischemic episodes double the risk of complications in these categories of patients, as well as high tissue selectivity and good tolerability, which enables its recommendation for the treatment of patients with hypertension and CHD.

The investigation conducted in 111 patients with osteoarthrosis (OA) has shown that the rate of oxidative stress is more significantly decreased by drugs that affect the direct and selective inhibition of interleukin-1ß elaboration and are involved in the mechanism responsible for blocking the receptors of this cytokine, one of the key factors of the pathogenesis of OA.

CHANGE IN VASCULAR WALL RIGIDITY IN HYPERTENSIVE PATIENTS WITH PERSISTENT ATRIAL FIBRILLATION
Abstract
Examination of vascular wall rigidity revealed a significant increase in pulse wave propagation velocity (PWPV) and total peripheral vascular resistance in hypertensive patients with persistent atrial fibrillation (AF) versus in those without cardiac arrhythmia. There was a strong direct relationship between PWPV and left ventricular anteroposterior size in hypertensive patients with AF (r = 0.74; p < 0.05).

SUPER-RESPONSE TO CARDIAC RESYNCHRONIZATION THERAPY AND MYOCARDIAL SCINTIGRAPHY IN PATIENTS WITH HEART FAILURE
Abstract
Cardiac resynchronization therapy (CRT) is a unique treatment for heart failure (HF) with augmented QRS; in this regard, it seems urgent to find new criteria for the prediction of a CRT super-response. While searching for these criteria, a variety of functional parameters may be put in the forefront depending on selected patient groups and used diagnostic techniques. However, the functional criteria are reversible in the vast majority of cases if HF is successfully treated. But cardiosclerosis is irreversible. Scintigraphic methods are shown to play a role in evaluating both myocardial functional parameters and cardiosclerosis. The results of the authors’ trials and clinical examples are given.

EXTRAESOPHAGEAL MANIFESTATIONS AND THEIR EARLY SYMPTOMATIC THERAPY IN ELDERLY PATIENTS WITH GASTROESOPHAGEAL REFLUX DISEASE
Abstract
Extraesophageal syndromes are more characteristic for elderly patients with gastroesophageal reflux disease than for maturity-age patients (58.5 and 24.7% of cases, respectively; p<0.001) and present mainly in the elderly (67.9%) with the concurrence of 2 more or forms, more commonly with a combination of bronchopulmonary and otorhinolaryngopharyngeal syndromes. The abilities of sodium alginate to reduce the rate of the latter and its relief time with the better tolerability are higher than in antacids.
