Dynamics of traumatism in adult population of the Russian Federation

Abstract

Purpose: to study the dynamics of traumatism, to analyze the injury structure as well as the structure of mortality from external causes in adult population of the Russian Federation.

Material and methods. Injury mortality and injury morbidity in adults were analyzed on the basis of official statistical data for the period 2013—2017.

Results. The decline of indices with preservation of gender differences as well as the differences in both the age groups and between the urban and rural population was noticed.

Conclusion. Development of complex preventive regional program is necessary to the decrease of traumatism and its negative sequels.

First experience of arthroscopic treatment of patients with false joint of the middle third scaphoid bone of the hand

Abstract

Introduction. Fractures of the scaphoid bone are the most frequent — among wrist bone fractures and in 10— 15% of cases they end with the development of a false joint. The pain is a restriction of movements in the wrist joint, a decrease in the strength of the hand, significantly limiting the working capacity of patients, as a rule, of young and middle age. In recent years, arthroscopic techniques have been widely used to treat non-union scaphoid in the middle third of the rook. Experience in the treatment of false joints of the scaphoid with the use of arthroscopic techniques in the N.N. Priorov National Medical Research Center of Traumatology and Orthopaedics is presented in this review.

Aim of Study: studying the effectiveness of arthroscopy during the healing of the non-union of the scaphoid bone.

Material and methods. In the department of microsurgery and hand injuries. NN. Priorova Ministry of Health of the Russian Federation from 2015 to 2018. 28 patients were treated with non-union scaphoid bone localized in the middle third of the bone’s body. The clinical examination included: measurement of movements in the wrist joint, the strength of the fist grasp; assessment of pain syndrome on a visual analogue scale (VAS), data from the DASH questionnaire; X-ray of the carpal joint in three projections (straight, lateral, ¾), computed tomography.

Results. Consolidation of the non-union scaphoid was achieved in 24(86%) patients within 8 weeks after surgery. Consolidation did not occur in 4 (14%) patients.

Long-term results 6 months after surgery were followed up in 15 patients. The range of motion in the wrist joint was an extension of 68° (from 45° to 90°), the volume of active flexion of the hand, respectively,66° (from 55° to 90º. Pain syndrome on a visual analogue scale (VAS) 1.5points (from 1 to 3 points). Fist grasp strength 6 months after surgery was 41 kg (from 25 to 57 kg). DASH scale after surgery 10.5 points (from 5.5 to 16.3 points).

Conclusion. Arthroscopic resection of the pseudarthrosis of the middle third of the bone with the plastic of the crushed spongy bone autograft from the ileal wing allows for consolidation of the scaphoid bone within 8 weeks. The first experience testifies to the effectiveness of this method and the need for further research in this direction, the analysis of long-term results.

Assessment of the functional state of the spine in athletes and ballet dancers with lumbosacral pain syndrome

Abstract

The paper presents the data of evaluation of dysfunction in lumbosacral pain in 898 athletes, ballet and circus artists aged 15 to 45 years. The median age was 25.8 year. 537 men and 361 women. In 409 people, pain syndrome is caused by osteochondrosis of the lumbar spine. 238 patients were diagnosed with spondylolysis of the lower lumbar vertebrae,172 with facet syndrome, spondylarthrosis and 79 with pathology of the ligaments of the lumbosacral spine.

Asymmetry in strength, tone of muscles-stabilizers of the spine and their bioelectric activity, which are eliminated in the course of treatment, was noted.

Personalized preoperative planning in children with recurrent patellar dislocation severe

Abstract

The aim of the study: was to improve the results of treatment in children with recurrent severe patellar dislocation, through the development of a differentiated approach to preoperative planning.

Material and Methods. In SLLP Clinics Samara state medical University from 2014 to 2018 was observed 54 patients with this disease. In 37 patients recurrent severe dislocation. The average age of patients is 14.2 years. The patients underwent postural radiography of the lower extremities, radiography of the knee joint in the lateral projection, computed tomography of the knee joints, computed tomography of the hip joints. Research patients measured parameters, confirming mnohopocetny dysplastic changes of the joint angle Q, the index Insall—Salvati, the congruence angle on Merchant and anticarsia of the femoral head. Because of the severe degree of the disease, bone plastic surgery is indicated. Patients underwent corrective supracondylar derotationdialysisa osteotomy of the femur in combination with medialization and distalization of the tibial tuberosity. During this operation, it is possible to perform a correction in all three planes.

Results. In the postoperative period,32 patients have no complications,1 patient has relapse,4 patients have contracture. In the long-term postoperative period 86.5% of positive results and 13.5% of unsatisfactory results, which is much lower than 36.1% — the percentage of complications according to the medical literature.

Conclusion. The proposed personalized approach in preoperative planning for recurrent dislocation of the patella of severe degree, allows you to choose the best option for surgery

Abstract

De Quervain’s disease, despite its low prevalence, is accompanied by pain syndrome, dysfunction and a decrease in the strength of the hand. This leads to a significant decrease in the quality of life of the patient, disruption of his work and daily activities. Patients with de Quervain’s disease, make up a significant part of those who turn to orthopedists, surgeons, rehabilitation doctors. However, the individual features of the structure of the first back canal of each person, formed in the process of phylogenesis, the lack of data on the exact cause of the disease cause the complexity of diagnosis and treatment of the disease. In the first part of the lecture, devoted to de Quervain’s disease, modern literature data concerning the features of the anatomical structure of the first back canal, the etiology and pathogenesis of the disease are summarized. It also presents a description of the clinical picture of de Quervain’s disease, tests and instrumental methods of diagnosis, the algorithm of examination of the patient with this pathology.

Abstract

Until recently, there were two global platforms where meetings of traumatologists and orthopedists from all over the world were held to discuss leading orthopedic and traumatological problems, define the concept of their correction and develop new protocols, and train colleagues based on the latest achievements of science and technology. These sites were held under the auspices of the European Federation of National Associations of Orthopedics and Traumatology (EFFORT) and the American Orthopedic Association (AOA). Recently, the third global “center of power” has appeared - on June 28-29, 2019, the Eurasian Orthopedic Forum (EOF) was held in the capital of the Russian Federation under the roof of the Expocentre.
