Аннотация

В статье анализируется тема существования специфических психопатологических особенностей в рамках аддиктивной патологии, в частности, при героиновой зависимости. На основе совокупности нейробиологических данных, эпидемиологии и клинической практики авторы отстаивают существование комплексной модели, объясняющей феноменологию зависимостей. Отмечается, что, несмотря на предполагаемую тесную связь между зависимостью и другими психическими состояниями, в психиатрические классификации до настоящего времени не внесены изменения, нацеленные на уточнение психиатрической коморбидности аддиктивного поведения. Указывается, что современная нозография психиатрии ограничивается преимущественно описаниями поведенческих аспектов зависимости и не учитывает другие стороны психической экспрессии, такие как аффективные и когнитивные установки. Констатируется, что в связи с отсутствием основных поведенческих и психопатологических критериев многие психопатологические симптомы попадают в другие категории, оцениваемые как дополнительные или сопутствующие расстройства (в рамках коморбидности). Сделан вывод о том, что высокую частоту появления психопатологических симптомов у зависимых, употребляющих героин, не следует недооценивать.
