Abstract

In the article approach to clinical diagnostics of the mental disorders, applied in ICD-10 is analyzed from a position of phenomenological aspect. The conclusion that operating classification of mental and behavioral disorders leads to washing out of borders between mental and behavioral norm and pathology and to discrediting of scientific character of psychiatry.

Complex regional pain syndrome – old and new approaches in evaluation of development mechanisms

Abstract

Complex regional pain syndrome of the arm (Sudeck syndrome) is considered as typical pathological process, which main component is myofascial and ligamentous pain syndrome of the forearm rotators with irritation of the median nerve that contains a large amount of sympathetic fibers. The 1st stage of the pathologic process is blocking of the first rib with irritation of the stellate ganglion and simultaneous blocking of the elbow joint with formation of myofascial pain syndrome in the forearm rotators. The 2nd is regional myofascial syndrome with irritation of the median nerve in the upper third of the forearm in the fibro-muscular channel. The 3rd is completion of development of upper-quadrant vegetative-irritative typical complex regional pain syndrome of the arm.

Abstract

The results of the survey of patients with ischemic stroke with the development of seizures are presented in the article. There were revealed some clinical features of seizures, depending on the vascular basins, pathogenic subtypes of stroke, in relation to the period their manifestation in different types of ischemic stroke, as well as peculiarities of functional and neuro-imaging conditions of these patients.

Abstract

There was performed a study of clinic-neurological and immunological parameters in children with spastic displegia, formed in the result of periventricular leukomalacia. Level of motor disorders severity, evaluated by Ashworth, Motor Function Club and Rivermead scales, is more expressed in children with spastic displegia, formed in the outcome of periventricular leukomalacia. The most sensitive was the Rivermead scale. There were shown oppression of cellular immunity, increase in oxidation-reduction activity of neutrophils and increase of tumor necrosis factor-α in peripheral blood serum at children with periventricular leukomalacia. The interrelation of the level of the cytokine tumor necrosis factor-α with the level of severity of periventricular leukomalacia was established.

This review presents the current understanding of the pathogenesis of Parkinson’s disease with the participation of different molecules (mitochondrial dysfunction, metabolic enzymes, enzymes from lysosomes, endosomes and ER, factors of the immune response and neuroinflammation, factors of the oxidative stress and proteasome degradation of proteins, chaperones, regulators of physiological and pathological processes, structural and transport proteins, molecules of signal transduction, glial proteins and proteins of synaptic vesicles, neurotoxic proteins, molecules of neuron-glia communication), which may be the genomic and/or proteomic biomarkers of this type of neurodegeneration.

CHRONIC TICK-BORNE ENCEPHALITIS: THE CLINICAL AND DIAGNOSTICS FEATURES

Abstract

The article presented deals with some peculiarities of chronic tick-borne encephalitis (CTBE) course, its clinical manifestations and diagnostics as well as the review of some literary data. Several pathogenesis development versions have been described in every detail, different variants of pathogenesis such as long-term and silent virus persistence in the body as a result of neuroglia cells lesions being viewed. The problems of virus genome preservation within the human body and the possible ways of its “activation” are broached as well. The paper also gives the classification of the infection chronic forms together with the review of a few disease course variants. The peculiarities of clinical manifestations on the nervous system side and some special features of the infection laboratory diagnostics are under discussion, too. We also describe the CTBE neuroimaging characteristics supplied with the magnetic resonance imaging (MRI) data from our own examinations, the MRI picture showing the multiple lesions of the brain very much alike the ones in patients with multiple sclerosis.

Abstract

There were presented results of long-term study of the phenomena of destructive social epidemics. Based on extensive epidemiological, clinical, experimental psychological studies it was proved the existence of the general algorithm of the involvement in chemical and destructive psychological dependencies. Universal factors of psychological resistance to the involvement in destructive social epidemics of all types were identified. There were developed the effective social methods to counteract the spread of such epidemics as chemical and destructive psychological dependencies.

DISPENSARY SUPERVISION IN DRUG ADDICTION TREATMENT AND FUNCTIONING IN THE SYSTEM OF THE OBLIGATORY MEDICAL INSURANCE

Abstract

The importance of diagnostical accuracy of the patient’s condition sharply increases in the system of medical insurance. The changes made contrary to recommendations of WHO in adapted for use in the Russian Federation the version of ICD-10, radically change diagnostics of a syndrome of dependence and an assessment of a stage of remission at addictive diseases. Possibility of financing to system of medical insurance of rendering medical care at a rehabilitation stage is thus excluded. Strict observance of WHO standards treatment of ICD-10 and requirements of the Federal Law “About the Psychiatric Help and Guarantees of the Rights of Citizens at Its Rendering” allows establishing accurate criteria of diagnostics of a condition of the patient on each of the assistance stages, necessary for justification of the “insured event” guaranteeing financing. Possibility of unreasonable stigmatization and discrimination of the drug and alcohol addicts is thus excluded.

Abstract

90 Patients with psychorganic syndrome complicated by alcoholism were examined. It was observed that all of them have a number of basic features which moderately reflect adaptive abilities of the organism. Boundary data of decrease of adaptive abilities was determined and outcome of social adaptation was forecasted. Diagnostic coefficients were computed. These coefficients were base for working up of diagnostic table which allows deciding with some certain level trustworthiness about the level of adaptive abilities of people with psychoorganic syndrome due to exogenous-organic disorders complicated by alcoholism.
