ANALYSIS OF THE RESULTS OF EPIDEMIOLOGICAL STUDY ON PREVALENCE OF ERECTILE DYSFUNCTION IN THE RUSSIAN FEDERATION

Abstract

With the aim of evaluation of prevalence of erectile dysfunction (ED) and other violations of the sexual function of men in the Russian Federation, anonymous questionnaire survey of 1400 men was performed. The study involved seven centers located in the most densely populated regions of the country. 1225 questionnaires were analyzed. It was found that ED symptoms were present in 1101 (89.9%) respondents. Age-related symptoms (by AMS questionnaire) were detected in 554 (45.2%) patients, most of whom were aged 45-59 years. The importance of active detection of ED in men, as it can be one of manifestation of a more serious disease, was demonstrated.

LONG-TERM RESULTS OF APPENDICOURETHEROPLASTY

Abstract

The article presents the long-term results of 15 operations for replacement of urethral strictures of various locations by appendix. One patient undergone plastics of top third of the right ureter, other undergone replacement of it pelvic part, and replacement of left ureter was performed in 3 cases of replacement of pelvic part. Surgical technique was modified in 12 cases by using a cut area from head of blind colon for better appendicocystanastomosis. The follow-up period ranged from 3 months to 14 years, on an average 6.1 ± 3.2 years. Postoperative complications requiring surgical correction occurred in 2 (13.3%) patients. Urinary tract patency was restored in all cases. Thus, good short-term results are obtained for the 2 (86.7%) patients, and good long-term results - for all patients. Fatal cases were not registered. Results of our clinical observations suggest the possible success of replacement of the any part of right ureter and left pelvic ureter by appendix.

ROLE OF SUPRAVESICULAR URINE DIVERSION IN THE TREATMENT OF PATIENTS WITH RENAL AND URETHRAL TUBERCULOSIS

Abstract

The article presents the results of the survey, etiopathogenetic and surgery treatment of 73 patients with nephrotuberculosis complicated by tuberculous ureteritis. Patients were divided into 4 groups. 1 - 3 Groups of patients underwent urine diversion by percutaneous puncture nephrostomy, open nephrostomy and internal stent placement. Patients in Group 4 not underwent surgery with urine diversion. It was found that the long-term etiopathogenetic therapy against the background of retention changes resulted in progression of changes and loss of renal function in 63% of cases. Early urine diversion, depending on its method allows preserving the functional ability of the kidneys in 70.8 to 94.4% of cases. The combination of early renal drainage with antituberculous polychemotherapy is significantly superior to conservative treatment, leads to a rapid reduction of chronic renal failure (CRF) and allows to subsequently perform the greatest number of reconstructive operations: in 62.6% of cases after the external drainage and in 73.3% - after internal drainage (p<0.05). At the same time, good results of plastics were achieved only in patients undergoing a two -stage surgical treatment, and poor results (relapse of stricture, progression of hydroureteronephrosis or CRF) were significantly more often observed (60%) in patients without urine diversion (p<0.05).

HERBAL PREPARATION CANEPHRON ®N IN THE TREATMENT OF PATIENTS WITH UROLITHIASIS

Abstract

Under current conditions, high-technology herbal preparations take an important place in the complex treatment of patients with urolithiasis. Canephron®N (Bionorica, Germany) is one of these herbal drugs widely used in urology. The article presents the results of comprehensive evaluation of the effectiveness of the canephron®N in patients with urolithiasis, who undergone extracorporeal shockwave lithotripsy, in the postoperative period and in the period of early (up to 3 months) metaphylaxis. It is shown that the use of the drug in complex treatment of patients with urolithiasis contributes to more rapid discharge of fragments of stones and effective improvement of renal function after removal of stones. The use of canephron ®N has great potential in the complex metaphylaxis of urolithiasis.

THE CLINICAL COURSE AND TREATMENT OF FUNGAL INFECTIONS IN THE EARLY PERIOD AFTER KIDNEY TRANSPLANTATION

Abstract

The article discusses the critical issues of clinical manifestations and treatment of fungal infections in patients after kidney transplantation. In fungal infection, which is usually detected in the composition of microbial associations, lungs are more often affected. In this case, mortality reaches 60%. Affecting the renal transplant by Candida spp. or Aspergillus spp. can lead to the loss of function of transplanted kidneys. At the current stage, lipid formulations of amphotericin B are drugs of choice for antimycotic therapy in posttransplant period. Nephrotoxic effect of amphotericin B is reversible and does not represent a serious threat to the function of the transplantate. Administration of lipid formulations of amphotericin B is the most justified, since it does not affect the concentration of immunosuppressive drugs in the blood serum of patients after transplantation.

BUILDING OF DEFECTS OF THE BLADDER WALL BY MEMBRANE, CREATED ON THE BASIS OF TYPE I COLLAGEN (AN EXPERIMENTAL STUDY)

Abstract

Experimental study was performed on 24 Chinchilla rabbits, which underwent resection of the bladder with building of defect by membrane “Collost”, created on the basis of type I collagen. The functional state of the bladder in situ was assessed by infusion cystomanometry during repeated surgery at 7, 14 day and at 1, 3, 6 months, and at 1 and 1.5 years. It is found that developing detrusor pressure during bladder contractions was decreased by 10 times in the first week of the study; it was in line with the subcompensation after 3 months, and was restored at 6 month. At 1 and 1.5 years, increase of cumulative function of bladder while saving detrusor pressure was observed. Dilating cystoplastics using biopolymer “Collost” provides good long-term functional results.

FEATURES OF HYDRODINAMICS IN DRAINAGE OF THE UPPER URINARY TRACT (EXPERIMENTAL STUDY)

Abstract

The present article is devoted to the search for the physical factors affecting the intrapelvic pressure in patients with kidney catheters. Based on mathematical prediction and experiment, value of air bubbles, hampering urine outflow and contributing to the development of purulent postoperative complications in urology, was demonstrated. A certain number of air bubbles in catheters leads to the formation of air-urinary impaction, causing a sharp increase of intrapelvic pressure. Correlation analysis showed a direct relationship between the amount of air bubbles in the drainages and resistance force to urinary flow. The study showed that the presence of air bubbles in the catheter and drainage tubes is unacceptable. Compliance with certain rules and standards of work allows to prevent their formation, which in turn reduces the risk of urinary infection and the development of septic complications. This approach will help to predict the possible postoperative complications, accelerate the introduction of modern technologies, and improve the quality of urological care.

OVERACTIVE BLADDER AS A MASK OF CHRONIC PROSTATITIS

Abstract

Order to investigate the frequency of symptoms of overactive bladder (OAB) in patients with chronic prostatitis and determine the effectiveness of trospium chloride in their treatment, the analysis of case histories of 154 patients referred to a urologist for the exacerbation of chronic prostatitis was performed. 27 (17.5%) patients had a diagnosis of overactive bladder; in 19 patients it was accompanied by chronic prostatitis, and in 8 patients OAB was diagnosed as separate disease. All of them have received trospium chloride 30 mg once daily for a month; patients with prostatitis simultaneously have received standard etiopathogenic therapy for this disease. A month later, a comprehensive treatment led to significant positive effect. Urinary frequency decreased by 56.1%, the functional bladder capacity increased by 82.8%, the number of urgent vesical tenesmus was reduced more than by half.

RELATIONSHIP OF SYSTEMIC FACTORS IN THE PATHOGENESIS OF CHRONIC PELVIC PAIN SYNDROME IN MEN

Abstract

Survey of 250 men with category III chronic pelvic pain syndrome/ chronic prostatitis (CPPS/CP-III) showed that the urological disease often proceeds against the background of obesity (29.6%), androgen deficiency (38.4%), increased tone of sympathetic nervous system (62.9%) and the activation of the plasma lipid peroxidation system (oxidative stress) (100%), with significant correlations between these factors. Working hypothesis was formulated; according this hypothesis, the components of metabolic syndrome in men and associated systemic reactions of sympathetic hyperactivity, oxidative stress, and androgen deficiency play an important role in the pathogenesis of CPPS/CP-III. This allows to consider CPPS/CP-III as an interdisciplinary complex issue of men's health and to review the existing traditional approaches to its diagnosis, treatment and prevention.

AFALA IN THE TREATMENT OF PATIENTS WITH BPH: EFFICACY AND SAFETY

Abstract

The double-blind, placebo-controlled, randomized clinical trial involving 94 patients has evaluated the efficacy and safety of domestic preparation afala in patients with symptoms of I-II stages benign prostatic hyperplasia (BPH). It was shown that a 6-month course of treatment with afala at a dose of 2 tablets 4 times a day resulted in a significant reduction in the severity of urinary disorders, estimated by total IPSS score, relative to baseline values and compared to placebo therapy. The most pronounced therapeutic effects of the drug were registered in respect of irritative symptoms of BPH. According uroflowmetry, peak flow rate after 6 months of treatment was increased by more than 50%. Significant clinical benefit persisted not only within the 6-month course of therapy, but 3 months after. Long-term therapy had no effect on the concentration of total, free and complex PSA, testosterone (total and free), dihydrotestosterone, and prolactin in the blood. Absence of adverse effects, biochemical abnormalities, changes in clinical blood and urine confirmed the safety of 6-month therapy.

DETECTION OF PROSTATE CANCER BASED ON MONITORING OF PROSTATE-SPECIFIC ANTIGEN IN OUTPATIENT CLINIC

Abstract

The study included 320 men with suspected prostate cancer (PC). The mean age was 59 years. The detection rate of prostate cancer depending on PSA level was evaluated. According to the results of histological examination, prostate cancer was detected in 32.8% (105 patients) of cases. The prostate cancer most frequently was diagnosed in patients with PSA levels above 20 ng/ml. Thus, the probability of detection of prostate cancer is directly dependent on the level of PSA. All groups showed signs of prostatic intraepithelial neoplasia (PIN), both low and high degrees, but the direct relationship of identification of PIN depending on PSA level was not established.

PROSTATE BIOPSY IN THE OUTPATIENT SETTING

Abstract

The article describes the methodology of transrectal diagnostic prostate biopsy under ultrasound guidance with regard to settings of municipal polyclinic; existing complications are listed. The analysis of the results of 876 biopsies performed within 3 years is presented. The distribution of the results of histological examination depending on the level of total PSA, ratio of free and total PSA, and PSA density was followed-up. Relationship between PC detection rate using standard biopsy and prostate volume is shown. The analysis of the degree of PC differentiation (Gleason score) depending on the level of total PSA, the ratio of free and total PSA, and PSA density was performed. Practical recommendations to improvement of PC detection rate are presented. Based on the analysis, it was concluded that the screening for prostate cancer for the purpose of its earlier diagnosis is reasonable.

ESTIMATION OF RENAL FUNCTION RESERVE IN PERCUTANEOUS NEPHROLITHOTOMY OF CORAL NEPHROLITHIASIS

Abstract

From November 2010 to December 2011, 56 patients with coral nephrolithiasis underwent percutaneous nephrolithotomy. To assess the renal function before surgery and before discharge (6 to 7 days after surgery), glomerular filtration rate (GFR) by Cockcroft-Gault equation was defined. Preoperative renal functional reserve was evaluated by method using a single oral loading of protein drugs. 46.4% of patients have decreased GFR before surgery, and before discharge the proportion of such patients was 39.3%. In 80% of patients with reduced GFR, postoperative filtering function of the kidneys in response to a protein load was increased, but did not reach the pre-operative no-load values, and in 7.5% of cases corresponded to a load indicators. Filtering function of the kidneys before discharge below baseline and load levels was observed only in 8.9% of cases. The data obtained in most cases allowed to predict GFR decline in the postoperative period in patients with coral nephrolithiasis. 87.5% of patients before surgery were characterized by the absence of renal functional reserve, but in 28.6% of patients in the postoperative period there was an increase in GFR, indicating the positive impact of endoscopic percutaneous nephrolithotomy on the functional state of the kidneys in the immediate postoperative period.

PHOTOSELECTIVE VAPORIZATION IN TREATMENT OF BENIGN PROSTATIC HYPERPLASIA IN ELDERLY AND SENILE MEN

Abstract

The article presents the results of photoselective laser vaporization of the prostate (PVP) performed in 50 men aged 71.84±6.3 years with obstructive form of BPH. The operation was performed with the use of 80 W potassium-titanyl-phosphate laser. All patients had complicated cardiac history (CHD, arterial hypertension, circulatory failure, artificial pacemakers). Intraoperative bleeding occurred in 1 (2%) patient and required electrocoagulation. There was no need for transfusion of blood components in any cases. The patient’s condition, blood pressure and heart rate remained stable during all surgeries. During the follow-up period, the positive dynamics in IPSS and QoL scores, as well as prostate volume, peak urine flow rate, and residual urine volume were observed. Thus, the PVP method is a good alternative to transurethral prostatic resection, especially for patients with burdened anamnesis

EARLY DIAGNOSIS OF NEPHROSCLEROSIS IN CHILDREN WITH VESICOURETERAL REFLUX

Abstract

Comparison of results of standard complex of survey with changes of urine levels of biological inflammatory marker (MCP-1), marker of fibrogenesis (TGF-β1), marker of angiogenesis (VEGF), markers of nephron damage (collagen IV, α-GST, π-GST) in 140 children with vesicoureteral reflux before and 6 months after treatment was performed. It was found that nephrosclerotic process occurs even at low degrees of vesicoureteral reflux. Even in the absence of structural and functional disorders of the kidneys according to traditional methods of evaluation, there were significant changes in the levels of biological markers of inflammation, fibrosis, and angiogenesis, as well as damage of main elements of the nephron. The method for calculating the «index of early renal damage» is proposed; method allows to monitor the state of the renal parenchyma of children with this disease.

EXPERIENCE OF PERFORMING THE EXTRAURETHRAL TRANSVESICAL ADENOMECTOMY IN PATIENTS WITH BPH

Abstract

Despite the development of endourology, issues of open surgical treatment of prostate adenoma remain relevant. It is not always possible to perform transurethral electro-resection of prostate or other minimally invasive surgery. Urologists performing the transvesical adenomectomy still often face serious complications in both early and late in the postoperative period. This article presents a 15-year experience of performing the extraurethral transvesical adenomectomy according to N.F. Sergienko procedure in 308 patients with BPH. Assessment of functional results was performed using uroflowmetry and IPSS score. The effectiveness of extraurethral transvesical adenomectomy (EUTA) was statistically confirmed. The use of EUTA technique allows minimizing intraoperative injury, maintaining the integrity of the front semicircumference and lateral sides of the vesico-urethral segment, which provides early rehabilitation of damaged tissues and contributes to a significant reduction in the number of early and late postoperative complications.
