VALIDATION OF THE RUSSIAN VERSION OF THE QUESTIONNAIRE TO ASSESS THE EFFECTIVENESS OF SURGICAL TREATMENT OF PATIENTS WITH ANTERIOR URETHRAL STRICTURE (PATIENT-REPORTED OUTCOME MEASURE FOR URETHRAL STRICTURE SURGERY (PROM-USS): A PILOT STUDY

Bazaev V.V., Shibaev A.N., Pavlova Y.V.

Abstract

Currently, the patient-reported outcome measures are widely used for the analysis of the effectiveness of treatment. In 2011, the questionnaire to assess the effectiveness of treatment of men with anterior urethral stricture (PROM-USS) was proposed. Study objective: during validation to perform pilot testing of the Russian version of the PROM-USS questionnaire for determining its psychometric properties (reliability, validity and sensitivity). The study involved 35 men with anterior urethral stricture, who completed PROM-USS questionnaires before and 6 months after surgery. The questionnaire consists of 14 questions and 2 visual analogue scales (VAS), combined in 2 domains: 1 - evaluation of lower urinary tract symptoms (LUTS), 2 - assessment of the quality of life; questionnaire also includes questions about treatment satisfaction. The reliability, validity and sensitivity of the PROM-USS questionnaire were assessed. High intraclass correlation (ICC) coefficients (0.7-0.96) and Cronbach's α = 0.76, indicated reliability of the questionnaire. The correlation coefficient between LUTS scoring and an index of maximum urine flow rate (Qmax) - R=-0,81 (p<0,05) confirmed the criterion validity. After the surgery, the total LUTS scoring decreased from 14.0±0.72 to 5.66±0.58 (P<0.05); VAS EQ-5D index increased from 51.43±5.3 to 71.42±7.64 (P=0.01) with an increase of Qmax from 3.4±0.3 to 17.9±0.9 ml/s (P<0.001), indicating the sensitivity of questionnaire to changes. Ninety-four percent of men were satisfied and very satisfied with the result of the surgery; 88% had residual LUTS not affecting the quality of life or affecting slightly. For the final conclusions about the feasibility of this tool, further studies involving a larger number of patients are required.

EXPERIENCE OF USE OF BLEMAREN® IN THE TREATMENT OF PATIENTS IN URIC ACID AND CALCIUM OXALATE UROLITHIASIS

Konstantinova O.V., Yanenko E.K.

Abstract

154 patients with urolithiasis were under outpatient observation for 2-8 years. Among them there were 76 women and 78 men aged 21-66 years, of which 46 patients with uric acid urolithiasis, and 88 - with calcium oxalate urolithiasis. Treatment of patients was carried out systematically, depending on their condition. Indications for the application of Blemaren® included the presence of uric acid stones, uric acid and/or oxalate crystalluria. The duration of treatment was 6.1 months. The dosage of the drug varied from 6 to 18 g per day and was selected individually, depending on the purpose of the appointment of Blemaren®. Reduction of the urine pH to 6.2- 6.8-7.2 was the criterion for properly selected dose. To dissolve uric acid stones in the presence of hyperuricemia and/or hyperuricuria, Blemaren ® was administered in combination with allopurinolata dose of 0.1 g 3-4 times a day. Besides pharmacotherapy, treatment included diet therapy. It was found that the morning urine pH in urate urolithiasis is sustainable and has a range of 5.0-6.0, in 80.4% of cases - range of 5.0-5.5. In calcium oxalate urolithiasis this parameter is also stable and has a range of 5.0-6.7, in 82.9% of cases - range of 5.5-6.0. Optimal urine pH to eliminate uric acid and oxalate crystalluria in patients with uric acid and calcium oxalate urolithiasis is the interval of 6.2-6.4. It was shown that Blemaren® is a highly effective agent for treatment and prevention of uric acid and calcium oxalate crystalluria in calcium oxalate and uric acid urolithiasis. Further, its effectiveness in dissolving of uric acid stones in the absence of an infectious inflammatory process is 82.3%.

SILODOSIN IN THE TREATMENT OF PATIENTS WITH BENIGN PROSTATIC HYPERPLASIA - RESULTS OF RUSSIAN MULTICENTER OBSERVATIONAL STUDY

Pushkar D.Y., Bernikov A.N., Sadchenko A.V.

Abstract

The study was aimed to the evaluation of the therapeutic effect of the drug Silodosin at a dose of 8 mg once a day in patients with symptomatic benign prostatic hyperplasia (BPH). This study included 1,000 patients. The average age of the patients was 64.99±8.07 years (median 64.5 years), mean duration of BPH - 3.64±5.058 years (median 1.1 years). Such a large scatter is due to the enormous heterogeneity of the data; in such cases, it is advisable to focus on the median value. 12 (1.2%) patients discontinued study early, 4 of them due to adverse drug reactions, 1 patient withdrew informed consent, 7 patients were lost to follow up. Among comorbidities, cardiovascular diseases and cerebrovascular disease had the highest frequency - both in 39 (3.9%) patients. The frequency of other preexisting disease did not reach 3%. According to the history of cases, urinary retention occurred in 115 (11.5%) patients, urinary tract infections -in 131 (13.1%), prostatitis - in 171 (17.1%), abnormal ejaculation - 55 (5.5%), erectile dysfunction - in 103 (10.3%). The primary efficacy variable was the change of total IPSS score. At visit 1, the average total score was 19.29±5.93, at visit 2 (4 weeks) -13.30±5.31 (P<0.001), at visit 3 (8 weeks) - 9.80±4.95 (P<0.001), at last available to the observation visit - 9.84±4.90, and the change was statistically significant compared with baseline score (P<0.001). The study revealed the positive dynamics of lower urinary tract symptoms, as well as positive changes in the assessment of quality of life. Thus, at the time of the last available observation, the QoL score was 1.93±1.03 points against 4.25 points at visit 1 and was statistically significant (P<0.001). During the study, 12 (1.2%) patients had 13 adverse events, and doctors associated 11 of them with taking the drug (silodosin); information on relation with any drug was absent in 2 of 13 cases.

DIAGNOSIS AND TREATMENT OF OVERACTIVE BLADDER SYNDROME IN PELVIC INJURIES

Neymark A.I., Voytenko A.N., Bondarenko A.V., Muzalevskaya N.I., Kruglykhin I.V.

Abstract

Case records of 1994 patients with injuries of the pelvic ring were analyzed. The article presents the results of examination and treatment of patients with urinary tract dysfunction. A frequent complications of pelvic fractures include overactive bladder syndrome (OAB) (18.9%). Many factors play a role in the development of this syndrome, in particular damage to the pelvic diaphragm and the development of pelvic hematoma. OAB is more common in unstable pelvic fractures (24.8%). Surgical stabilization of the pelvic ring injuries and the use of M-cholinoblockers and a1-blockers lead to a reduction of OAB.

ASSESSMENT OF THE RISK OF INCIDENTAL PROSTATE CANCER PROGRESSION

Marisov L.V., Vinarov A.Z., Alyaev Y.G., Gerasimov A.N., Bezrukov E.A.

Abstract

At present, methodology based on the stage T1a or T1b is used to determine the risk of incidental prostate cancer (IPC) progression. This approach was developed before the introduction of prostate-specific antigen (PSA) into the practice of the urologist, and does not account of the degree of IPC. For the prediction of IPC progression, multifactorial prognostic model was created. This model is based on the analysis of two parameters - the total PSA level before the surgery and the Gleason score according to the results of morphological examination after transurethral resection of the prostate hyperplasia. This model has a high predictive value (AUC 86.6%) and can be easily used in practice. For the interpretation of the calculated results special table is formed that allows to assess the risk of progression over the next 3.5 years.

PCA3 AND TMPRSS2:ERG GENES EXPRESSION IN BIOPSIES OF BENIGN PROSTATE HYPERPLASIA, INTRAEPITHELIAL NEOPLASIA, AND PROSTATE CANCER

Mikhaylenko D.S., Perepechin D.V., Grigoryeva M.V., Zhinzhilo T.A., Safronova N.Y., Efremov G.D., Sivkov A.V.

Abstract

Morphological analysis of the biopsies for prostate cancer (PCa) often is a difficult task due to heterogeneity and multifocality of tumors. At the same time, a lot of data exist about the potential molecular genetic markers of PCa. The aim of our study is to determine of PCA3 and TMPRSS2:ERG genes expression in benign hyperplasia (BPH), low and high grade intraepithelial neoplasia (PIN), PCa for revealing of diagnostic value of those genes expression in benign and precancerous changes in prostate. Total RNA was isolated from 53 biopsies, reverse transcription was performed, gene expression was determined by real time PCR (RT-PCR) then deltaCt index was determined as Ct(PCA3) - Ct(KLK3). Average deltaCt and its SD in BPH were 8,28±3,13, low PIN - 8,56±2,64, high PIN -8,98±1,69, PCa - 1,08±2,36. We have demonstarted that deltaCt did not differ in patients with BPH, low and high grade PIN, whereas significantly increased in PCa relative to any of the three groups listed above (p<0.0001). Expression of TMPRSS2:ERG was absent in BPH, PIN, but it was detected in 40% (4/10) of PCa cases. ROC-analysis showed that the AUC (area under ROC-curve with 95% CI, p<0.0001) was 0,98 ± 0,02 in the analysis of a combination of overexpression of PCA3 and TMPRSS2:ERG. Thus, the expression analysis of the PCA3 and chimeric oncogene TMPRSS2:ERG in biopsy cannot be used for differential diagnosis of BPH, low and high grade PIN. However, overexpression of PCA3 and expression of TMPRSS2:ERG are characteristic in PCa. Expression analysis of these genes by the proposed RT-PCR modification at the threshold level deltaCt 3,22 has diagnostic accuracy 90% to detect PCa in biopsy specimens.

THE CONTROL OF BLEEDING FROM THE LUMEN OF THE ISOLATED INFERIOR VENA CAVA SEGMENT WHEN REMOVING THE TUMOR THROMBUS

Shchukin D.V., Lesovoy V.N., Garagaty I.A., Hareba G.G., Polyakov N.N., Ilyukhin Y.A., Altukhov A.A.

Abstract

The paper analyzes surgical methods of bleeding control from the lumen of the isolated inferior vena cava (IVC) segment during thrombectomy. The study included 13 patients with IVC tumor thrombus. The sources of bleeding were divided into variant lumbar veins and lumbar veins of the infrarenal IVC segment. Variant lumbar veins were classified as vessels entering IVC at its posterior surface at suprarenal level. The volume of blood loss from the lumen of the isolated IVC segment during thrombectomy ranged from 100 to 3500 ml with an average of 507.3 ml. The main sources of bleeding were lumbar veins of the infrarenal IVC segment, located at a distance of less than 10 mm from the mouth of the right renal vein (85.7%). Variant veins were found much rarer - in 35.7% of cases. Their orifice diameter and, consequently, the blood loss volume was significantly smaller than those in the group of bleeding from infrarenal lumbar vessels. In 2 (15.4%) cases, the mouths of the venous tributaries were stitched inside the vena cava. In 6 (46.2%) patients the bleeding was stopped by applying a clamp to lumbar veins following the blood clot removal. This maneuver proved successful in 4 (66.7%) of those cases. In the remaining 5 (38.5%) cases, the bleeding was stopped by compression of probable variant or lumbar vein mouth localization with finger or cotton swab. If unable to stop bleeding in the described way, the blood from IVC lumen was evacuated by suction, and cavatomy was closed as quickly as possible. Given the low effectiveness, high complexity and risk of the available methods of IVC lumen bleeding control, visual information about the location and the number of possible sources of bleeding should be obtained preoperatively.

NARROWED SPECTRUM DIAGNOSTICS AND COMPUTER CHROMOENDOSCOPY IN THE DIAGNOSIS OF EPITHELIAL NEOPLASMS OF THE LOWER AND UPPER URINARY TRACT

Martov A.G., Solomatnikov I.A., Baykov N.A., Andronov A.S., Khalmurzaev O.A.

Abstract

Tumors of epithelial origin occupy the first place in the structure of oncological diseases. This study was aimed to the evaluation of the possibilities of narrowed spectrum endoscopy and computer chromoendoscopy in the diagnosis of epithelial structures of the bladder and upper urinary tract. Materials and methods. Additional diagnostic endoscopic examination of the lower urinary tract was performed in 76 patients (132 urethrocystoscopies). For the evaluation of the efficacy of complementary technologies in the endoscopic examination of the upper urinary tract, 47 patients were examined (57 endoscopic examinations of UUT). Results. During endoscopic examination of the bladder only in the «white light», 80 suspicious lesions were identified and removed. The additional use of NBI - diagnosis allowed to reveale and remove 9 (9.5%), abnormal structures, 6 of them (66.7%) were representatives of transitional cell carcinoma. The additional use of a computer chromoendoscopy revealed 6 (6.3%) neoplasms missed during endoscopy in white light: 4 samples (66.7%) were transitional cell carcinoma, 1 (16.7%) - squamous papilloma. Endoscopic examination of the upper urinary tract only in the «white light» identified and removed 47 suspicious lesions. Additional application ofnarrowed spectrum regimen has allowed to identify and remove 8 (12.9%) pathological lesions, 3 (37.5%) of which were morphologically transitional cell carcinoma. The additional use of a computer chromoendoscopy indentified 7 (11.3%) tumors missed during endoscopy of UUT in «white light», 4 (57.1%) of which were transitional cell carcinoma. All other biopsies obtained were representatives of chronic inflammation, or material was not suitable for morphological examination. Conclusions. Our experience of the use of narrowed spectrum diagnostics and computer chromoendoscopy allowed to improve the diagnosis of bladder cancer by 14.3%, and to improve the detection of transitional cell carcinoma of the upper urinary tract by 14.9% at the stage of development of the method.

PARAMETERS OF SPERMATOGENESIS IN MEN EXPOSED TO DIFFICULT ENVIRONMENTS

Nikolaev A.A., Loginov P.V.

Abstract

This study was aimed to the comparison of the effects of various unfavorable factors on spermatogenesis and the level of free radical oxidation in the ejaculate. 40 men of reproductive age who visited doctor for lack of children within 2-3 years of marriage were examined. Patients were divided into 4 groups depending on the action of the unfavorable factors: the oil and gas industry workers, workers of radio-location and television and radio broadcasting facilities, creative workers, the unemployed and low-income people. The control group consisted of physically healthy men of similar age who had children within marriage. The malondialdehyde levels in the ejaculate and morphokinetic parameters of sperm cells were assessed. Natural gas and oil-dissolved gas were the most toxic against fertilizing properties of the ejaculate, they led to sharp decline in sperm concentration and motility of sperm cells by reducing the active mobile forms in oil and gas industry workers. In workers of radio-location and television and radio broadcasting facilities, essential modifications of sperm cell morphology were caused by mechanical damage of the microwave radiation. In creative workers suppression of spermatogenesis had complex nature due to the enhanced dynamics of free-radical oxidation, as well as the participation of the central regulatory mechanisms. The unemployed and low-income people with hyponutrition and malnutrition had suppression of spermatogenesis caused by a lack of biological resources necessary to maintain spermatogenesis and provide reliable antioxidant protection. There was an increase in the dynamics of aging of sperm cells. Thus, the intensity of lipid peroxidation, as well as the nature and direction of specific unfavorable factor determine the cumulative effect of the toxicity of this factor and the fertilizing capacity of the ejaculate.

FEATURES OF CLINICAL COURSE AND MORPHOMETRIC PARAMETERS OF BENIGN PROSTATIC HYPERPLASIA IN MEN WITH METABOLIC SYNDROME AND ANDROGEN DEFICIENCY

Tyuzikov I.A., Grekov E.A., Kalinchenko S.Y.

Abstract

The study was aimed to the evaluation of the effect of the components of the metabolic syndrome (obesity, insulin resistance (IR)) and androgen deficiency on the clinical course of lower urinary tract symptoms against the background of benign prostatic hyperplasia (LUTS/BPH) and nocturia, as well as on some of the parameters of BPH (prostate volume, residual urine volume, total prostate-specific antigen (PSA) blood level). The comprehensive survey of 160 men with LUTS/BPH (mean age 56.7±3.3 years) was performed; based on the results of survey, three comparison groups were formed: Group 1 (n=70) - patients with isolated obesity; Group 2 (n=36) - patients with obesity and insulin resistance; and Group 3 (n=54) - patients with obesity, insulin resistance and androgen deficiency. The control group consisted of 30 patients with LUTS/ BPH without these metabolic and hormonal disorders. In patients with LUTS/BPH and obesity, higher frequency of nocturia compared with the control group was revealed (63.7% vs 23.3%; P<0.05), as well as the relationship between the waist circumference and free testosterone level (n=160; r=-0.322; P=0.005) and waist circumference and total prostate volume (n=160; r=0.121; P=0.005). In patients of Group 2, significant positive correlation between waist circumference and blood insulin level was identified (n=36; r=0.461; P=0.005). Patients with obesity, insulin resistance and androgen deficiency had the highest average prostate volume and residual urine volume compared with those of other groups (P<0.05). Thus, obesity, insulin resistance, androgen deficiency are associated pathological conditions, greatly aggravating the clinical course of LUTS/BPH due to adverse impact on the BPH parameters, acting both together and separately. The most severe LUTS/BPH were associated with the presence of all three of the above systemic disorders.

HYPERBARIC OXYGEN THERAPY IN THE TREATMENT OF MALE INFERTILITY ASSOCIATED WITH INCREASED SPERM DNA FRAGMENTATION AND REACTIVE OXYGEN SPECIES IN SEMEN

Metelev A.Y., Bogdanov A.B., Ivkin E.V., Mitrokhin A.A., Vodneva M.M., Veliev E.I.

Abstract

The aim of this study was to explore the potential of hyperbaric oxygenation (HBO) for reduction of sperm DNA fragmentation level and reactive oxygen species (ROS) in semen. The study included 90 men with idiopathic infertility. Patients of the treatment group (n=60) underwent HBO before the vitro fertilization (IVF) procedure. In the control group (n=30) IVF was carried out without prior course of HBO. Sperm DNA fragmentation analysis was carried out using the TUNEL assay, the level of ROS in the ejaculate was measured by chemiluminescence. HBO treatment resulted in a significant decrease in the mean level of sperm DNA fragmentation from 33.2±7.5 to 11.9±5.9%, and the median ROS in sperm from 0.89 to 0.39 mV/s (p<0,05). In the control group these changes were not statistically significant. Pregnancy after IVF occurred in 63.3% (38/60) of sexual partners of the treatment group men and in 36.7% (11/30) of the control group (p<0.05). The high efficiency of HBO in overcoming the adverse effects of oxidative stress on sperm parameters allows us to consider it as a promising method for the treatment of men with idiopathic infertility.

Kustov A.V., Strelnikov A.I., Airapetyan A.O., Moryganov M.A., Zhuravleva N.I.

Abstract

The paper presents the results of the 1-year screening of biochemical parameters of urine in female patient with recurrent calcium urate urolithiasis. Based on the data of quantitative X-ray phase analysis of the composition of stone and a complex of clinical and laboratory examination, reasons for recurrent stone formation were determined. The main reasons included hypocitraturia, hyperosmolarity of urine and uric acid diathesis. Therapy with citrate mixtures and adequate water schedule (daily urine - 2-2.5 liters) for 10 weeks allowed to increase the urinary pH, led to 2.5-fold increase in daily urinary citrate excretion and getting rid of residual stones in both kidneys. The effect of citrate therapy lasted for a further six months after drug withdrawal. Subsequently, the daily urinary citrate excretion and pH decreased, but even after 8 months remained still above baseline values. Obviously, for high risk patients it is necessary to define the daily excretion of citrate, and diagnosis of hypocitraturia requires long course therapy with citrate mixtures, with a break of not more than six months and obligate maintenance of adequate diuresis.

REPRODUCTIVE DYSFUNCTION AS A RESULT OF INFECTIOUS AND INFLAMMATORY DISEASES

Osadchy A.V., Kulchavenya E.V.

Abstract

Tuberculosis remains an important medical and socio-demographic problem. Chronic inflammation of the prostate gland - of both nonspecific and tuberculous etiology - reduces the quality of the ejaculate; this in the initial low level of fertility in a population can have dramatic effects. Infertility can be the first symptom prompting the patient to visit a doctor; and patient is diagnosed with tuberculosis after survey. However, the influence of tuberculosis of the prostate on the functional parameters of ejaculate has not been studied. The positive influence of selenium and zinc preparations on the quality of the ejaculate in patients with chronic nonspecific prostatitis, idiopathic infertility was demonstrated, but the effectiveness of their complex use in tuberculosis of the prostate gland has not been studied.

Breusov A.A., Kulchavenya E.V., Brizhatyuk E.V., Filimonov P.N.

Abstract

The review analyzed 59 sources of domestic and foreign literature on the use of immunomodulator izoprinozin in treating patients infected with human papilloma virus, and the results of their own experience. The high prevalence of HPV and its role in the development of cervical cancer are shown, the mechanisms of HPV development and the host protection from this infection are described. The authors present approaches to the treatment of HPV-infected patients with particular attention to izoprinozin. Isoprinosine belongs to immunomodulators with antiviral activity. It inhibits the replication of viral DNA and RNA by binding to cell ribosomes and changing their stereochemical structure. HPV infection, especially in the early stages, may be successfully cured till the complete elimination of the virus. Inosine Pranobex (izoprinozin) having dual action and the most abundant evidence base, may be recognized as the optimal treatment option.
