Onset of rheumatoid arthritis in the elderly age - difficulties of diagnostic and treatment: a clinical case

Abstract

Background. Rheumatoid arthritis (RA) is a chronic autoimmune disease, manifested by progressive destruction of the joints, systemic inflammation of the internal organs and a wide range of comorbid diseases associated with chronic inflammation, and often with side drug effects. Description of the clinical case. The article presents a clinical case of successful diagnosis and treatment of RA in an elderly patient debuted with recurrent synovitis of the knee joints. The presence of serious comorbid pathology, the occurrence of side effects against the first-line background therapy created certain difficulties in the management of the patient. Conclusion. Early diagnosis and treatment of RA in elderly patients is significantly difficult even with the availability of modern research methods for a rheumatologist. The presence of comorbid pathology may limit the use of certain pharmaceuticals and cause side effects. Not only the presence of comorbidities, but also the age, features of the course of RA should be taken into account in selection of background therapy.
