Predementia cognitive disorders: modern approaches to terminology, diagnosis and treatment

Abstract

Predementia cognitive disorders (PDCDs) are a heterogeneous group of states with variable course, prognosis, and key clinical symptoms. This article presents a modern understanding of the terminology and criteria for the diagnosis of PDCDs, their clinical features; the concepts of subjective cognitive impairment and partial type of mild cognitive impairment are introduced. The difficulties of their identification are associated with insufficient data on the degree of permissible functional decline and the features of daily activity in various variants of cognitive disorders. Given the frequent absence of the responsible informant, especially in the conditions of hospitalization, a scale for assessing daily activity in the hospital is proposed. Differentiated approach to the diagnosis of PDCDs will optimize the treatment of patients and improve their quality of life.

Abstract

Alzheimer’s disease (AD) is the most common form of dementia in clinical practice. The early-onset AD is difficult to diagnose due to its rare occurrence. This article presents an up-to-date review of data on the incidence, pathomorphology of the brain, clinical picture, diagnosis and pharmacotherapy of early-onset AD. Particular attention is paid to the genetic aspects of AD with autosomal dominant mode of inheritance.

This review summarizes information indicating the generality of the pathogenetic links of rheumatoid arthritis and atherosclerosis, as well as participation in their development of such components of the immune system as Toll-like receptors, with description of structure and functions. The results of recent studies on the role of Toll-like receptors 5 in the development and progression of rheumatoid arthritis and atherosclerosis are discussed.

Potentials for functional recovery of the upper limb in stroke patients using the brain-computer-controlled interface for hand exoskeleton with multichannel biofeedback

Abstract

Background. The consequences of cerebrovascular disorders have significant impact on the quality of life of patients. An important aspect of the possibility of self-maintenance and self-efficacy of stroke patients includes the preservation of cognitive functions, as well as hand functions, first of all, fine hand motor skills. Objective. Evaluation of the effect of the BCCIHE simulator (brain-computer-controlled interface for hand exoskeleton) with kinesthetic and visual feedback on improving/restoring the functional activity of the upper limb in daily activities, as well as possible causes of non-use of the upper limb with intact motor characteristics. Methods. The current analysis included 29 patients with a similar severity of neurological deficits. BCCIHE simulator with kinesthetic and visual feedback was used in the study. Results. The use of BCCIHE simulator with kinesthetic and visual feedback as part of the complex rehabilitation of patients of the main group who interacted with the neurointerface in the active paradigm has led to the significant improvement of upper limb’s motor activity, mainly due to the restoration of its proximal parts, and improvement of motor control during active actions. Conclusion. The use of the BCCIHE simulator complex with kinesthetic and visual feedback as a part of rehabilitation measures has a positive effect not only on the isolated restoration of physical activity, but also on the expansion of daily activity indicators by improving the quality of movements performed bimanually.

Principles of dopaminergic therapy for Parkinson’s disease

Abstract

In the treatment of Parkinson’s disease (PD), the correction of dopamine deficiency with dopaminergic drugs, which can have both short-term and long-term effects, is of key importance. Longstanding maintenance of a long-term effect is one of the most important areas in modern approaches to optimizing the treatment of PD, one of which is the use of long-acting dopaminergic receptor agonists. The early transition to a combination therapy of PD provides a more optimal anti-Parkinsonian effect in patients with early and advanced stages, allowing maximum use of mobility resources in PD patients. High adherence to therapy and flexible dose titration make long-acting ropinirole one of the most effective treatments for PD. The article presents data on the possibility of using ropinirole in the practice of treating PD.

Non-cognitive psychopathological symptoms as early manifestations of dementia: diagnostics and therapy

Abstract

Understanding of prodromal conditions or early clinical manifestations of dementia helps early detection of the disease and can lead to the development of effective prevention measures for dementia. Mild cognitive impairment (MCI) is considered as the earliest manifestation of dementia. Non-cognitive psychopathological symptoms (NCPS) are common in MCI and occur in 35-75% of patients. The most common symptoms of NCPS include apathy, anxiety, depression, and irritability. It has been shown that the presence of NCPS in MCI increases the risk of dementia. Studies have shown the effectiveness of Cerebrolysin in preventing cognitive deficits in patients with MCI and reducing the severity of NCPS at the stage of mild and moderate dementia in Alzheimer’s disease, as well as in complex therapy of depression (in combination with an antidepressant) in elderly people and patients with organic disorders of vascular genesis.

Features of diagnosis and treatment of vertebrogenic pain syndromes in elderly patients

Abstract

Back pain is one of the most frequent complaints of elderly patients, but until recently this problem did not attract proper attention. In part, this is true: at least one episode of back pain occurs within a year in 30-50% of patients older than 70 years. However, the clinical relevance of back pain in the elderly is more significant. In the context of more frequent comorbid conditions and lower socioeconomic status, it has a more negative impact on the quality of life, restricts mobility and independence in daily activities to a greater extent, and is an immediate factor that increases the risk of falls, and causes a persistent fear of movement due to ability to fall and be injured, which closes the vicious circle of inactivity. In conditions of low mobility and limited pharmacotherapy, the tendency to chronization of pain syndrome is increasing.

Onset of rheumatoid arthritis in the elderly age - difficulties of diagnostic and treatment: a clinical case

Abstract

Background. Rheumatoid arthritis (RA) is a chronic autoimmune disease, manifested by progressive destruction of the joints, systemic inflammation of the internal organs and a wide range of comorbid diseases associated with chronic inflammation, and often with side drug effects. Description of the clinical case. The article presents a clinical case of successful diagnosis and treatment of RA in an elderly patient debuted with recurrent synovitis of the knee joints. The presence of serious comorbid pathology, the occurrence of side effects against the first-line background therapy created certain difficulties in the management of the patient. Conclusion. Early diagnosis and treatment of RA in elderly patients is significantly difficult even with the availability of modern research methods for a rheumatologist. The presence of comorbid pathology may limit the use of certain pharmaceuticals and cause side effects. Not only the presence of comorbidities, but also the age, features of the course of RA should be taken into account in selection of background therapy.
