Abstract

Advances in hormonal contraception have led to the creation of new estrogen-progestin-releasing systems as NovaRing vaginal ring and transdermal patch EVRA. We consider their contraceptive efficacy, effect on the menstrual cycle, body weight, and metabolic parameters. It is emphasized that these systems have a number of advantages (the release of small doses of hormones, the absence of daily fluctuations in the hormone excretion and the primary effect of passing through the liver, usability, minimal systemic effects, etc.), making them the most prospective methods of contraception.

Abstract

Problems of prevention and therapy of consequences of irreversible bone tissue loss in menopausal women are discussed. The authors believe that the pathogenetic-substantiated prevention of osteoporosis and pathological fractures in peri- and postmenopausal women, aimed at slowing the mechanisms of bone resorption at the stage of relatively intact capacity for adequate bone formation, provided by recovery of still moderate estrogen deficiency by using hormone replacement therapy is most effective. At the same time the diet correction in the direction of full consumption of products containing calcium, phosphorus and magnesium, active lifestyle with regular physical activity, renunciation of social habits are also required.

The goal of treatment of bacterial vaginosis is the restoration of normal vaginal ecosystem, wherefore microorganisms (causative agents of disease) elimination, activity of lacto- and bifidobacteria increase, prevention the superinfection are required. Abstract

We consider the prevalence and characteristics of development and course of stroke and myocardial infarction in pregnancy. In recent years the prevalence of stroke among pregnant women is 34.2 cases per 100 thousand deliveries, myocardial infarction - 6.2 per 100 thousand. Diagnosis and treatment of stroke and myocardial infarction in pregnancy have their own characteristics (use limitation of radiological techniques and thrombolysis, etc.). It is emphasized that the development of stroke or myocardial infarction in pregnant is quite a rare occurrence now. However, we have seen a significant increase in the frequency of risk factors for cardiovascular disease in pregnant women, age increase of women planning pregnancy in recent years. Consequently, we can expect growth in the incidence of severe vascular accidents in pregnancy in the near future.

Current antibiotic therapy in out-patient pediatric practice

Abstract

We consider the general principles of antibiotic use in children in outpatient treatment. It is emphasized that acute antibacterial drugs administration in treatment of acute respiratory diseases (ARD) and enteric infections should be limited to cases with signs of bacterial infection, which occurs not more than in 5-10% patients. Actually, 25-70% of patients with ARD receive antibiotics, which increases the risk of flora resistance and side effects, and often embarrass the diagnosis. We provide recommendations for antibiotic therapy in children with acute respiratory disease, skin and subcutaneous infections, intestinal and urinary infections in outpatient practice.

Abstract

The article provides an overview of the frequency and diagnostic features of infections caused by mycoplasma and chlamydia in children. The importance of clinical diagnosis of such pneumonia and bronchitis bring into focus, because the high rate of carrier state and delayed rising antibody titer aggravate diagnosis in early disease. Clinical manifestations of mycoplasmal pneumonia and bronchitis and pneumonia caused by Chlamydia trachomatis in infants are sufficient for the correct empirical choice of antibiotic. Lack of treatment efficacy of pneumonia by beta-lactam antibiotics is important for further diagnosis. Drug of choice for treatment of "atypical" infections is azithromycin.

Experience of selective non-steroid anti-inflammatory drug Nise use in diagnostics and treatment of dysmenorrhea in girls

Abstract

We present current data on classifications, mechanisms of development and diagnosis of dysmenorrhea. The results for assessment and treatment analysis of 197 women with dysmenorrhea are presented. This make it possible to characterize the structure of dysmenorrhea in adolescents, pain syndrome particular characteristics, and the dynamics of pain syndrome relief at a provocation test by selective non-steroidal anti-inflammatory drug Nise (nimesulide). These results demonstrate the importance of differentiated approach to diagnosis and treatment of dysmenorrhea, including the various hormonal and nonhormonal drugs administration for the management of various disease manifestations in girls.

Causative agent for genital warts is human papillomavirus. This virus is wide-spread and causes a number of skin and mucosa diseases. Thus far, there is no drug that can destroy or remove the papillomavirus from the body. Therefore, treatment is limited to local warts removal and the course of immunotherapy. Solkoderm deserves attention as one of the destructive chemicals. The drug apparently has no contraindications. Solkoderm is well tolerated by patients. Scars remain very rare situation after its application.

Phytoestrogens in the complex therapy of premenstrual syndrome

Abstract

We consider the problem of premenstrual syndrome (PMS) and discuses ways to its management. In connection with the improvement of pharmaceutical technologies in recent years plant preparations containing phytohormones or plant regulators are an increasingly popular means for hormonal imbalance correction. In the reproductive age coformulated drugs containing phytoestrogens (natural estrogen) in combination with fitoprogestins and tocopherols are able to relieve the PMS symptoms by action at the sex steroids receptors level. These drugs are treatment of choice in patients young and juvenile ages with all PMS forms as well as with contraindications to synthetic estrogens of combined oral contraceptives.

Acute tonsilliti in pediatric age: diagnostics and treatment

Abstract

In childhood beta-hemolytic streptococcus group A is causative agent of acute tonsillitis only in one-third of children, same frequency is observed in diseases caused by Epstein-Barr virus and respiratory viruses (most adenoviruses). Clinical differences between these forms of tonsillitis are almost exclusively absence catarrhal symptoms in streptococcal tonsillitis and the presence of nasopharyngitis in patients with viral forms, as well as conjunctivitis in adenovirus. Laboratory markers of bacterial inflammation show minor aid in the differential diagnosis of bacterial and viral forms, dramatize the importance of streptococci identification by microbiological and serological methods. Antibacterial therapy is justified only in streptococcal forms, in which it gives immediate effect. The lack of effect suggests a viral tonsillitis, and stop treatment with antibiotics. The deta-lactams use in streptococcal tonsillitis requires a 10-day course of therapy. Azithromycin or better - jozamycin (less often develops resistance) use allows the 5-day course of therapy.

Peculiarities of magnesium use in the period of pregnancy and in pediatric practice

Abstract

The normal level of magnesium in vivo recognized as a fundamental constant, which controls human health. Since 1995, WHO classifies pathological condition "magnesium deficiency" as a disease. In obstetric-gynecological practice, magnesium preparations are traditionally used for reduces the uterus hypertonia and premature delivery prevention because magnesium lowers the muscle tissue tone due to antagonism with calcium, providing muscle contraction. A pronounced positive effect was obtained by orotate magnesium administration in pregnant women with arterial hypertension. As a component of the drug orotic acid in this particular case is an intermediate step in the pyrimidine biosynthesis, glycogen synthesis, etc. Magnesium and orotic acid have a synergistic effect. For that reason, drug Magnerot, one tablet which contains 500 mg of magnesium orotate dehydrate, come under notice. This salt is poorly water-soluble, therefore nonreactive with gastric juice and has no laxative effect. As a source of magnesium, drug was originally designed to correct magnesium deficiency in various diseases and pregnancy.

Efficacy of Isoprinosine in the treatment of chronic cervicitis associated with papillomaviral infection

Abstract

We present the results of the study, including 45 women with chronic cervicitis associated with papillomavirus infection (PVI) in conjunction with ectopia uterine cervix (EUC), whose goal was the justification for inclusion immunocorrective drug Izoprinozin (inosine pranobeks) in the comprehensive treatment of such patients. Izoprinozin administration in comprehensive treatment of chronic cervicitis associated with AAH, increases the therapy efficacy, provides significant anti-inflammatory effect and leads to the complete elimination of the human papilloma virus in 82% of cases.
