PREVALIN V LEChENII SEZONNOGO ALLERGIChESKOGO RINITA

Abstract

This study was aimed to the evaluation of the anti-inflammatory efficacy of combination of standard therapy of seasonal allergic rhinitis with the barrier preparation (Prevalin). The open perspective randomized comparative parallel-group study included 49 patients with moderate to severe seasonal allergic rhinitis caused by wormwood pollen sensitization. All patients were treated with intranasal glucocorticosteroids in combination with a non-sedating antihistamines, 30 patients additionally received Prevalin 4 times a day. The frequency and severity of symptoms during treatment, the need for drug therapy, standard laboratory parameters, the level of eosinophil cationic protein in serum and nasal contents were evaluated. The results showed that the use of Prevalin in addition to the standard pharmacotherapeutic complex promoted more rapid and effective suppression of disease symptoms, as well as support of pharmacological remission of the disease using a smaller volume of basic therapy. More rapid achievement of clinical effect was recorded against the background of a more effective suppression of allergic inflammation.

Abstract

The article is devoted to the modern principles of treatment of seasonal allergic rhinitis (AR) in children. The current classification of AR is presented; the possible and necessary elimination measures for AR patients are described; pharmacotherapy is considered, including the use of systemic and local antihistamines, decongestants, mast cell stabilizers, topical corticosteroids, and antileukotriene drugs. Characteristics of main pharmacological agents used in the treatment of AR in children are discussed. Modern methods of allergen-specific immunotherapy of children with AR are described.

Abstract

The article presents the modern approaches to the topical treatment of atopic dermatitis (AD). Selection of optimal topical treatment of AD is based on leading role of immune inflammation and the presence of other pathological changes (dry skin, hyperkeratosis, lichenification, secondary infection). The place of conventional drugs, topical corticosteroids, calcineurin inhibitors (pimecrolimus, tacrolimus) in therapy of AD is considered. The need for use of medical cosmetics is emphasized.

Abstract

The article describes the modern approaches to allergen-specific immunotherapy (ASIT) in children with allergic diseases. Allergens used for ASIT are presented; ways for its administration and the effect of this treatment on the immune response are shown; high therapeutic efficacy and safety of parenteral, sublingual and intranasal ASIT in bronchial asthma, pollen disease and insect allergy are discussed. Careful monitoring for children’s health during of immunotherapy, individual approach to the increase of therapeutic doses of allergens taking into account the patient’s condition, maximizing the dose of allergens and subsequent continuous maintenance therapy are an essential condition for optimal effect of ASIT.

The upward trend in the prevalence of atopic dermatitis (AD), clinical manifestation in early age, as well as increased incidence of severe forms of the disease determine the great social health importance of this problem in pediatrics. Basis of complex, pathogenetically justified treatment of atopic dermatitis include topical anti-inflammatory therapy. Proper selection of external medicines and their rational use in accordance to the general principles of treatment contribute to achieving maximum therapeutic effect.

Abstract

The article presents the modern data on the mechanisms of action and potentials for the use of antihistamines, primarily cetirizine, in the treatment of allergic diseases. The results of application a cetirizine in the therapy of 27 children (10 boys, 17 girls) aged 2-9 years with mild-to-moderate atopic dermatitis with disease duration of 3-5 years are discussed. In addition to cetirizine, patients have received topical glucocorticosteroid (GCS) mometasone furoate. Maximum duration of use of topical GCS therapy was 7 days, and minimal symptoms of the disease in 11 patients were controlled by receiving cetirizine only. At Day 10, the remission of AD was achieved in 23 children; exacerbation of disease was arrested in all patients at the end of the 2nd week. Follow-up for 2-4 months showed that 21 (77.8 %) patients achieved long-term remission of atopic dermatitis, and 6 patients had one episode of mild exacerbation.
