Abstract

The article presents the current data on irritable bowel syndrome (IBS), including its definition, epidemiology, etiology and pathogenesis, clinical features and diagnostic criteria. The role of visceral hypersensitivity as a leading pathophysiological mechanism of IBS is analyzed in detail. Topical non-pharmacological and pharmacological approaches to the treatment of IBS are discussed.

Abstract

This article discusses the mechanisms of action of some neurohumoral substances on the motility of the gastrointestinal tract, including intestinal contractions. The first, the role of serotonin in the regulation of motor activity of the digestive tube is discussed. The comparative analysis of currently used serotoninergic drugs (tegaserod, prucalopride) affecting the motor activity of the esophagus, stomach and intestines is presented. The clinical manifestations of deficiency and an excess of serotonin in the human body, and ways for their correction are discussed.

Abstract

Despite the continuous development of pharmacotherapy and a variety of approaches, chronic constipation remains difficult to treat disease. Today, under the existing approach in terms of evidence-based medicine, the consistent and graded use of dietary fibers, lactulose, polyethyleneglycol, and prucalopride is most justified. Lifestyle modification has no significant effect on the change in the frequency and nature of stool in patients with chronic constipation. Due to the lack of standardization, potential of biofeedback therapy and other rehabilitative treatment methods are not fully used. The surgical treatment of chronic constipation is relevant only in the case of clear indications, with failure of entire arsenal of therapeutic modalities (rehabilitative treatment) and pharmacotherapy.

Abstract

The frequency and severity of community-acquired cases of Clostridium difficile infections (CDI) are increasing around the world and account for 20 to 41 % in the structure of morbidity. The article highlights the list of main preparations for the treatment of CDI for the last 30 years; and currently available data on the effectiveness of each drug are considered. A new drug, the first representative of a 18-membered macrocyclic antibiotics, fidaxomicin, is described. Results of meta-analyzes confirming higher efficiency of fidaxomicin compared with vancomycin are presented.

Abstract

The article presents the results of evaluation of the efficacy of schemes of treatment of patients with H. Pylori-associated diseases. Quaternary schemes with inclusion of proton pump inhibitor rabeprazole and bismuthate tripotassium dicitrate allow for a higher eradication rate than the classical triple therapy, and thus improve the clinical efficacy and cost-effectiveness of eradication therapy.

Abstract

The efficiency of psychotherapeutic treatments in addition to standard medical therapy of patients with a combination of bronchial asthma (BA) and gastroesophageal reflux disease (GERD) was evaluated. Additionally, the impact of psychotherapeutic treatments on clinical course, quality of life, spirometry, esophagogastroduodenoscopy (EGDS), and 24-hour pH-metry was assessed. It is shown that the inclusion of psychotherapeutic methods in the complex therapy of patients with a combination of bronchial asthma and GERD has an additional therapeutic effect. Along with the positive dynamics of clinical symptoms and indicators of psychological response of patients, parameters of quality of life have improved.

Abstract

An open randomized clinical trial with randomization of patients with NASH and type 2 diabetes mellitus (n = 75) into 3 groups was performed. All patients received hypocaloric diet with restriction of carbohydrate. Patients in group 1 (n = 27) received metformin 500 mg 3 times a day, patients in group 2 (n = 23) received a comprehensive hepatoprotective drug containing essential phospholipids made by nanotechnology and glycyrrhizic acid 2 capsules 3 times per day, patients in group 3 (n = 25) received metformin and hepatoprotector in the indicated dosages. The assessment of results was carried out at 90 and 180 days. The data of study indicate that the combined treatment regimen leads to the relief of abdominal pain, dyspeptic and astenovegetative syndromes, normalization of biochemical parameters of cytolytic and cholestatic syndromes, reduction of dyslipidemia, decrease of degree of steatosis, hepatitis activity and stage of fibrosis in the liver on the background of improving of compensation of diabetes mellitus. Monotherapy with metformin or hepatoprotector is less effective in these patients.

This fact emphasizes the importance of pharmacoeconomic analysis of appointed therapies for their clinical and cost optimization. Abstract

The article examines the role of gastric acid production under normal and pathological conditions. The molecular mechanisms of gastric acid secretion by the parietal cells of the stomach are considered. The basis of pathogenetical treatment of acid-related diseases - the appointment of a proton pump inhibitors (PPIs) as a modern drugs for the correction of hyperacidity - are discussed. The advantages of the original PPI omeprazole (Losec MUPS) are analyzed; this dosage form comprises a plurality of acid-resistant microgranules releasing the active ingredient only in the alkaline environment of the duodenum that provides a predictable and reproducible antisecretory effect.

Abstract

The article presents a comparative description of the effect of different antispasmodic drugs according to the data of peripheral electrogastroenterography in patients with irritable bowel syndrome. Several features of action of antispasmodics in different modes of drug administration - protracted treatment, a single dose drug «on demand» - were evaluated. Antispasmodic Hyoscine butylbromide (Buscopan) has proved to be an effective drug in the treatment «on demand» for the relief of abdominal pain, that allows to recommend its use for this purpose in clinical practice.
