Abstract

Вопросы эффективности использования в терапии остеоартрита хондропротекторов - симптоматических препаратов природного происхождения на пресс-конференции «Остеоартрит и боль в спине: вместе увидим целое. Новый подход к известной проблеме» рассмотрели ведущие эксперты в области медицины. В конференции приняли участие: академик Насонов Е.Л., академик Мартынов А.И., а также проф., д.м.н. Алексеева Л.И., проф., д.м.н. Данилов А.Б., проф., д.м.н. Ковалев Г.И., научный сотрудник, к.м.н. Таскина Е.А. В ходе мероприятия были представлены результаты двойного слепого рандомизированного плацебо-контролируемого исследования симптомо- и структурно-модифицирующих действий оригинального препарата Алфлутоп компании «Биотехнос», которое было проведено на базе ФГБНУ НИИ ревматологии им. В.А. Насоновой, МГМУ им. И.М. Сеченова и РНИМУ им. Н.И. Пирогова. Согласно выводам специалистов, данный хондопротектор можно рассматривать как эффективный и безопасный препарат при заболеваниях суставов с доказанным структурно-модифицирующим действием.

Osteoporosis - a pediatric disease with gerontological consequences. When, whom and how to prevent it?

Abstract

The review is devoted to modern approaches to the prevention of osteoporosis (OP) and its consequences - fractures. Prevention of OP, as well as other chronic non-communicable diseases, is the main way to reduce the incidence rate. Despite the fact that the OP is more common in old age, disease prevention should begin in childhood and its purpose should be to build a strong skeleton, preventing or slowing the loss of bone mass. Scientific concept of prevention of OP after reaching the peak bone mass includes the concept of risk factors. The main attention in the development of primary prevention measures is paid to modifiable risk factors that can be corrected or eliminated. It focuses on the correction of behavioral risk factors such as good nutrition and exercise, especially the use of synthetic nutrients for the prevention of OP and fractures.

Abstract

Effective therapy of osteoarthrosis (OA) is a difficult task due to the chronic and steadily progressing course, sometimes without any apparent predisposing factors. However, we should strive to do everything possible to prevent the serious structural changes in joint tissues. Glucosamine sulfate has reliable symptom-modifying properties, helping to reduce the intensity of pain and stiffness, and improve the functional activity of the target joints. This drug has the potential pathogenetic structure-modifying (basic) action aimed at reducing the rate of cartilage degeneration. Pharmaceutical activity of glucosamine sulphate allows its successful use in the treatment of OA, primarily the knee OA.

Osteoporosis in real clinical practice: focus on alfacalcidol

Abstract

The article presents the results of survey of physicians of 6 specialties involved in the treatment of osteoporosis, and patients suffering from this disease. A survey of doctors has showed that they appoint alfacalcidol to 27% of patients, including as a monotherapy in 8% of cases. Among patients, 25% of the respondents received alfacalcidol in combination with other drugs for the treatment of osteoporosis (22%), or as monotherapy (3%). In addition, article presents the analysis of the literature, which is justified the advantages of alfacalcidol from the perspective of evidence-based medicine.

Application of combined antihypoxant in the process of etiopathogenetic treatment of degenerative spine diseases was high effective and fully justified.

Experience of use of Cellex® for the treatment of patients with acute ischemic stroke

The data indicate the effectiveness of Cellex, because administration of this drug in stroke patients has led to significant improvement in the patients’ condition by the end of the observation period (20 days), and it was confirmed by the dynamics of NIHSS score (total score; movement) and «Speech Questionnaire» score.

Clinical efficacy of local therapy in the complex treatment of patients with osteoarthrosis

Abstract

Clinical efficacy of local therapy with glucocorticosteroids (GCS) in the treatment of patients with osteoarthrosis was evaluated. The study included 25 patients, 15 of which received an additional 1-2 injections of corticosteroids. As a result, local therapy on the background of the primary treatment led to achievement of best successes of positive dynamics of clinical and functional parameters, as well as improvement of quality of life of patients.

Vascular disorders in patients with dermatoses

Abstract

Microcirculatory disorders are one of the most important pathogenetic factors for chronic inflammatory dermatoses. Pharmacotherapy and physical therapy for the correction of vascular changes are traditionally included in the comprehensive treatment of dermatoses of various etiologies. The optimal choice of angiotropic therapy and minimization of the risk of polypharmacy, however, require adequate assessment of the degree of vascular disorders in the skin of the patient with dermatosis, especially at the early stage of development of pathology. In this regard, technique for determining the titers of natural antibodies to tissue autoantigens, allowing early diagnosis of vascular disorders, is of particular interest. For the correction of microcirculatory disorders, complex treatment of dermatoses should be supplemented with angioprotective and antiplatelet drugs.

New mechanisms of action of nsaids: focus on cardiovascular safety

Abstract

In recent years, cardiovascular safety of non-steroidal anti-inflammatory drugs (NSAIDs) is the object of attention of researchers and doctors. In patients receiving NSAIDs, increased risk of cardiovascular disease has several reasons, including prohypertensive effects. The greatest prohypertensive effect was described for naproxen and indomethacin. The therapeutic effect of NSAIDs is accomplished by blocking the enzyme cyclooxygenase-2 (COX-2) induced in the focus of inflammation, which leads to reduced formation of prostaglandin H2 and subsequent products of its metabolism. Recent studies of COX in combination with oxicams presented the first detailed description of the method of binding of oxycams through the network with two water molecules in the main channel of the active COX site. Also, interest to this class of NSAIDs has increased due to the discovery of the fact that certain derivatives of oxicams are not only potent inhibitors of microsomal prostaglandin E1 synthase, but microsomal prostaglandin E2 synthase, which is involved in the final synthesis of the primary mediator of inflammation - prostaglandin E2 (PGE2). These data open a prospect for the development of a new class of selective PGE2 inhibitors.

Dangerous combination of angiotensin converting enzyme inhibitors, diuretics, and nonsteroidal antiinflammatory drugs in clinical practice

Abstract

The article presents a review of the scientific data on the issue of polypharmacy, particularly unfavorable combination of diuretics and blockers of the renin-angiotensin-aldosterone system (RAAS) with non-steroidal anti-inflammatory drugs (NSAIDs). Nephrotoxic side effects of NSAIDs (analgesic nephropathy) are well known. The use of NSAIDs significantly increases the risk of acute kidney damage in patients with cardiovascular disease who are taking diuretics and RAAS blockers. The combination of diuretics and NSAIDs may be accompanied by adverse renal impairment as a result of the interaction of the pharmacodynamic effects on renal hemodynamics, vascular tone and volaemia. Combination of NSAIDs with angiotensin converting enzyme inhibitors (ACE inhibitors), which have nephroprotective effect, leads to the counteraction of NSAIDs influence on the kidney. However, the triple combination of an ACE inhibitor+NSAIDs+ diuretics leads to acute renal dysfunction - iatrogenic kidney disease, which can be fatal. This combination is called «triple whammy», and recognized by the World Health Organization as an unfavorable combination, requiring special monitoring of renal function and the need to avoid the use in clinical practice.

Modern view on the analgesic drugs for osteoarthritis: evidence from meta-analysis on the effectiveness

Abstract

The commentary discusses the results of a meta-analysis of the clinical efficacy of various nonsteroidal anti-inflammatory drugs (NSAIDs) and paracetamol (acetaminophen) for the treatment of pain in osteoarthritis (OA). Data from 74 randomized clinical trials with a total of 58 556 patients were included in the authors’ research. The low efficiency of the use of paracetamol, including compared with placebo, and comparisons of different NSAIDs in the current recommendations, as well as individual choice of treatment of patients with OA (gonarthrosis, coxarthrosis) are discussed. Given the confirmed high efficacy of etoricoxib, not inferior to high doses of diclofenac, and recently obtained data on its favorable safety profile (including the benefits in terms of gastrointestinal complications, and results of evaluation of cardiovascular complications comparable with nonselective NSAIDs),this selective drug should be considered as a first-line analgesic therapy for OA.

Modern possibilities of treatment of tendinopathy using complex homeopathic preparation Traumeel S

Degenerative changes in periarticular tissues with inflammatory process, called tendinopathies or tendinosis, are frequent pathology of the musculoskeletal system. Traumeel S fully meets these requirements, and its efficacy and safety in patients with tendinopathies have been demonstrated in numerous multi-center, randomized clinical trials.

Abstract

The review analyzes results of studies on the efficacy of complex homeopathic preparation Mucosa compositum in patients with gastroesophageal reflux disease. It is concluded that the addition of Mucosa compositum to the complex therapy not only improves the clinical efficacy of the standard treatment, but also increases the remission of the disease. Taking into account the high safety profile of Mucosa compositum, it seems reasonable to add this drug to the complex therapy of patients with GERD.
