Острая окклюзия артериального русла почек у детей с наследственной тромбофилией

Аннотация

Цель. Определить критерии диагностики инфаркта почек у детей и выделить группу риска по развитию тромбозов сосудов почек у детей с наследственной тромбофилией.
Материал и методы. Было обследовано 66 детей в возрасте от 1 года до 18 лет с наследственной тромбофилией: 1-ю подгруппу (44 ребенка) составили дети соматически здоровые, являющиеся носителями генотипических сочетаний, ассоциированных с риском тромбозов, которые были обследованы в связи с отягощенным семейным анамнезом; 2-ю подгруппу (22 ребенка) - дети с наследственной тромбофилией и перенесшие острый тромбоз различной локализации. Группу контроля составили 20 детей. Наряду с общепринятым клинико-лабораторным обследованием, детям проводили генетическое исследование, определение плазменных уровней гомоцистеина и липопротеина (а), оценивали коагулологические показатели и выполняли ультразвуковую допплерографию почек.
Результаты. При исследовании полиморфизмов генов гемостаза были выявлены различные генотипические сочетания. У 8 детей с наследственной тромбофилией определялись аваскулярные гиперэхогенные зоны в паренхиме почек по данным ультразвукового исследования с допплерографией - инфаркт почки.
Заключение. Наиболее уязвимы в плане формирования инфаркта почки дети с наследственной тромбофилией. В группу риска по развитию инфаркта почек могут быть отнесены пациенты с высоким уровнем липопротеина (а), превышающем более, чем в 2 раза контрольные значения.
