Changes in blood pressure in different vascular beds in healthy volunteers during a passive orthostatic test 60

Abstract

The aim of this study was to evaluate the orthostatic changes in different vascular beds BP indicators and PWV in 40 healthy young volunteers in passive tilting 60°. In horizontal position, at rest, the foot artery BP is significantly higher than brachial one. Tilt up at 60° does not cause significant changes in the brachial BP, while in the distal artery there is a significant increasing in SBP and DBP and a decreasing in PBP with a simultaneous increasing in PWV. The influence of the gender factor on orthostatic changes in blood pressure and PWV was revealed. The height has a significant effect on the increasing in blood pressure in distal parts of the vasculature during tilt up, but practically does not influence on the PWV in the horizontal and tilt up positions of the subjects.

Medical rehabilitation of patients with coronary heart disease after coronary shunting taking into account their adaptation potential

Abstract

The analysis of results of rehabilitation of the sick JHD which have undergone treatment at late hospital and sanatorium stages has revealed that practically at all patients disadaptation manifestations take place. For studying of nonspecific adaptation reactions of an organism at patients with coronary heart disease after operation of coronary shunting and assessment of efficiency of the program of their medical rehabilitation on the basis of increase in adaptation potential 155 sick JHD have been surveyed. From them 129 sick JHD have undergone operation KSh and underwent landmark medical rehabilitation, being on recovery treatment at a late hospital stage. All examined patients were men aged from 40 up to 74 years. Average age of patients was 49,7±3,7. The main contingent of the examined patients consisted of people highly - emotional and brainwork. Inclusion in the program of rehabilitation of hyperbaric oxygenation promoted transition of the majority of sick JHD after KSh in more favorable extents of adaptation. Application of the program of rehabilitation with addition of KVCh-therapy has made favorable impact on a condition of adaptation of the sick JHD which have transferred KSh. As a result of complex rehabilitation at sick JHD lipidic exchange has improved. Under the influence of the carried-out program of rehabilitation the number of sick JHD after KSh has increased in favorable extents of adaptation. The most effective KVCh - therapy has appeared at patients with adaptation violation 2 and 3 degrees. Complex assessment of efficiency of the carried-out treatment in points has allowed to note «improvement» of 70,9% of patients.

Сellular therapy in the treatment of peripheral arterial disease

Abstract

Peripheral arterial disease contributes to decreased exercise tolerance, impaired proprioception, muscle atrophy and weakness, with advanced cases resulting in critical limb ischemia, ulceration and gangrene. Current treatment options by re-establishing in line blood supply to the limb are limited, with poor outcomes predicted by multilevel arterial disease. Therapeutic angiogenesis has been considered for the treatment of patients with critical limb ischemia who do not qualify for surgical revascularisation or percutaneous intervention. Angiogenesis can be induced with growth factors and/or with stem cell preparations containing bone marrow derived mesenchymal stromal cells and endothelial progenitor cells, also capable of forming vascular endothelium. Numerous trials published in 2000-2014 showed that bone marrow-derived stem cells containing endothelial progenitor cells and mesenchymal stem cell have been successfully and safely harvested and implanted in patients with critical limb ischemia as intravenous or intramuscular infusions and and there was a significant clinical improvement in patients as measured by pain scores, amputation rates, claudication distance and appearance on angiography. This trials did show a significant lengthening in the «time to treatment failure», determined as doubling of ulceration, major amputation or new gangrene. This trials did not demonstrate any adverse effects due to the intravenous or intramuscular injection of autologous bone marrow-derived tissue repair cells. In patients who have received transplants, observed improvement of microcirculation and complete healing of ulcers. After stem cell application the foot amputation was possible to avoid the major amputation.

Structural mechanisms of kidneys functional systems integration in the regulation of protein homeostasis

Abstract

To decryption the mechanisms of kidneyshomeostatic functions, integration of functional systems that provide its work a model of protein loading in different age periodswasmade. However, structural mechanisms of interaction between different functional systems of kidney in different physiological conditions remains poorly understood. This led to the aim of the study - to identify the structural basis of kidneysfunctional systems integrationin the regulation of protein homeostasis. In the experiment carried out on white rats, we studied the morphology of the kidney tissue by morphometric and electron microscopic methods in protein load (15 rats), protein starvation (15 rats) and in control (15 rats). The kidneys tissue corresponding to the superficial and juxtamedullary nephrons, were studied on the first, third and seventh day of the experiment, after fixation in a 2.5% solution of buffered glutaraldehyde acid. It is shown that on 1 day of protein load activation of juxtaglomerular apparatus(JGA) cells wasobservedwith a gradual decrease in activity by 3 day, whereas during protein starvation, the activation of JGAwas not observed. Herewith, both experimental groups showed an increase in the proportion of glomeruliwith a greater degree of blood capillariesopening. However, at protein load, these changes persisted up to 7 days of observation, as at protein starvation after 7 days the degree of blood capillaries opening remained high only in juxtamedullary nephrons. In control ratscondition of juxtaglomerular apparatus and degree of blood capillaries openingindicate moderate functional activity of kidneys. The authors come to the conclusion that at different physiological conditions logical changes of juxtaglomerular appa-ratuscells and capillaries of superficial and juxtamedullary nephronsglomerulioccur, which aims to increase the functional reserve of kidneys.
