CLINICAL CHARACTERISTICS OF HIV-INFECTED PATIENTS IN THE VOLGOGRAD REGION

Resumo

Objective. To give the clinical characteristics of patients with HIV infection in the Volgograd Region and to make recommendations for improving the diagnostic and treatment process for this disease. Materials and methods. The paper gives the clinical characteristics of HIV- infected cases in the Volgograd Region on the basis of the 2011-2015 data available in the Volgograd Regional Center for Prevention and Control of AIDS and Infectious Diseases (Annual Report Form No. 61 «Information on HIV-infected patients»). It analyzes the ratio of various stages of progression of the infection among those who are followed up and f irst detected, as well as the dynamics of their changes, the causes of death, and the frequency of deuteropathies. Results. The majority of patients were found to have a subclinical stage of HIV infection and to be identif ied at this stage. Infectious diseases, mainly those of bacterial etiology, prevail among the deuteropathies. The antiretroviral therapy coverage of those in need is consistently high. Tuberculosis is a leading cause of death. Conclusion. Based on the findings, the authors generate recommendations for enhancing the effectiveness of identifying HIV-infected patients, predicting the needs for different types of health and social care, and planning diagnostic and treatment measures. In particular, the authors recommend that amendments be made to a new statistical report form for HIV-infection and that automated data logging systems be introduced into routine practice.

Cancer incidence rates continue to grow in the Russian Federation, which requires that women should have the higher responsibility for their health. The previously published results of the project «Cancer Patrol» have shown that not all women are wise about their health: a behavioral response in almost half of the respondents ranges from neglectful to phobic, including their attitude towards the use of hormonal agents. This can be responsible for refusal to undergo follow-up examinations and to seek for medical care late. This article presents the results of the second stage of all-Russian educational project «Women of different ages between cancer negligence and cancer phobia: how to prevent cancer?», which was launched in 2015 and is currently ongoing. The study was initiated and is funded by the Russian Association of Genital Infections and Neoplasia (RAGIN) and the Interdisciplinary Association of Reproductive Medicine Specialists (MARS) in collaboration with the Far East Institute for Social Development, the Association of Medical Journalists and a number of medical and social organizations. To study the awareness of routine examinations and the impact of hormonal therapy on cancer morbidity rates among the female population and physicians, special questionnaires were filled out by 4846 women aged 18 to 65 years who resided in 32 cities of Russia. Based on questionnaire survey results, the authors developed an education project that involved interactive «Cancer patrol» lectures and a workshop on cervical pathology. The project covered 34 cities of Russia, as well as Azerbaijan and Kazakhstan. The active participants of seminars are healthcare administers, researchers, teachers of departments, physicians, cytologists, laboratory diagnosticians, and patients. Special workshops for teachers are planned.

CARDIOVASCULAR DISORDERS IN CHILDREN WITH INFLUENZA AND OTHER ACUTE RESPIRATORY VIRAL INFECTIONS

Cardiovascular disorders frequently accompany acute respiratory viral infections (ARVI) and influenza in children. Objective. To estimate the frequency and nature of cardiovascular events in children with ARVI and influenza. Subjects and methods. The results of a follow-up and examination of 1268 children and adolescents aged 1 to 18 years, who had been hospitalized for various ARVIs of specified etiology, were studied. Examination, and laboratory, electrocardiographic and echocardiographic studies were made. Results. 118 (9%) children were noted to have a combination of newly identified clinical changes with enhanced activity of cardiac specific enzymes, and disorders revealed by ECG and EchoCG. Conclusion. Cardiovascular events often develop in children with ARVI and influenza. Their prevention is to use timely adequate etiotropic therapy. Imidazolyl ethanamide pentandiol acid (ingavirin) is a drug with antiviral, antiinflammatory and immunomodulatory effects, which is recommended to treat ARVI and influenza in children aged 7 years or older.

CLINICAL EFFICACY OF UMIFENOVIR IN ACUTE RESPIRATORY VIRAL INFECTIONS

Objective. To evaluate the clinical efficacy of umifenovir in acute respiratory viral infections (ARVI) in adult patients. Subjects and methods. The investigation covered 47 patients aged 18 to 60 years with ARVI, of them there were 26 (55.3%) men and 21 (44.7%) women. 53.2% of the patients were aged 18 to 30 years. According to the timing of their search for medical care and their treatment, the patients were divided into 2 groups: 1) 23 patients who started receiving treatment during the first 48 hours after onset of symptoms; 2) 24 patients sought medical advice at 2-3 days of the disease. All the patients received 2 umifenovir capsules 4 times daily for 5 days. To verify the diagnosis, the investigators applied a rapid influenza diagnostic technique by an immunochromatographic test using the Influenza A + B test system («Vegal Farmaceutica S.L.», Spain), a PCR assay for detecting the viruses of influenza and ARVI (nasopharyngeal swabs), as well as comprehensive examination including clinical blood analysis, general urinalysis, chest radiography, and electrocardiography (if necessary). Phagocytic activity of monocytes and granulocytes, IFN-like activity of serum and supernatant fluid of cultured blood cells incubated in the medium with addition of phytohemagglutinin (PHA) or Newcastle disease virus, or without them (interferon status) were studied over time. Results. A clinical effect is noted in the treatment of catarrhal respiratory syndrome, in the reduction of the febrile period, and more rapid recovery in Group 1. Conclusion. Early treatment can considerably shorten the duration of the disease and reduce its major symptoms and the risk of complications.

MOLECULAR GENETIC ANALYSIS OF HIV-1 AMONG PRISONERS ON ANTIRETROVIRAL THERAPY IN THE PENITENTIARY FACILITIES OF THE SVERDLOVSK REGION

Objective. To carry out a molecular genetic analysis of HIV-1 among prisoners receiving antiretroviral therapy (ART) in the penitentiary facilities. Materials and methods. Viral load was determined in 89 prisoners; out of them 32 individuals with a viral load of >1000 copies/ml were examined for HIV-1 resistance to reverse transcriptase inhibitors and protease inhibitors by a genotypic method. A survey method was used to estimate the degree of compliance with antiretroviral therapy in 138 prisoners. Results. 20 (62.5%) of the 32 examined HIV-1 nucleotide sequences were interpreted as susceptible to 19 antiretroviral drugs. None of the 32 sequences contained mutations of high-level resistance to the prescribed drugs. Genotyping revealed that subtype AFSU was predominant; five genetic variants were CRF03 AB and two variants were CRF63 02A1. Conclusion. The high or multiple resistance of HIV-1 to reverse transcriptase inhibitors and protease inhibitors is not responsible for the virologic failure of ART in prisoners.

THE NATURAL COURSE OF CHRONIC HEPATITIS C IN PATIENTS WITH UNDIFFERENTIATED CONNECTIVE TISSUE DYSPLASIA

Objective. To evaluate the natural course of chronic hepatitis C (CHC) in patients with undifferentiated connective tissue dysplasia (UCTD). Subjects and methods. The investigation covered 95 CHC patients who were divided into 2 groups: 1) 63 patients with signs of UCTD; 2) 32 patients without UCTD. Complaints and clinical laboratory parameters were comparatively analyzed in 2011 and 3 years later, in 2014. Results. After 3 years of the natural course of CHC, Group 1 patients were observed to have pronounced liver functional changes: hepatic tissue disorders and blood biochemical abnormalities, which were accompanied by hemorheologic alterations. In Group 2, the found disorders were unidirectional, but their severity was much less. Conclusion. The presence of UCTD in CHC patients is important and crucial to the high activity of an inflammatory process and to the rapid development of pronounced hepatic fibrotic processes.

TOPICAL ASPECTS OF THE LABORATORY DIAGNOSIS OF PARASITIC DISEASES

Objective. To estimate the diagnostic value of laboratory diagnostic techniques for parasitoses that are relevant to southern Russia. Materials and methods. For verification of parasite infestation in 1500 patients attending the Clinic of Infectious and Parasitic Diseases, Rostov Research Institute of Microbiology and Parasitology, Russian Federal Service for Supervision of Consumer Rights Protection and Human Well-Being, the investigators used laboratory diagnostic techniques regulated by the Methodological Guidelines (MG) 4.2.3145-13 «Laboratory diagnosis of helminthiasis and protozoan disease» and MG 3.2.1173-02 «Serological tests for laboratory diagnosis of parasitic diseases». Results. Parasitic diseases were found in 58.5% of the patients who had visited the clinic with a referral diagnosis of helminthic invasion. The most frequently recorded diseases were enterobiasis (35.5%), echinococcosis (18.7%), toxocariasis (15.5%), and taeniarhynchosis (5.8%). The most effective method for diagnosing enterobiasis was a triple examination of the material collected from the perianal folds, which could identify pinworm (Enterobius vermicularis) eggs in 91.7% of the patients. The permanent laboratory signs of toxocariasis were peripheral blood eosinophilia and a positive result of ELISA with Toxocara antigen. A one-month follow-up after etiotropic therapy showed that 65.0% of patients had a two-fold or greater increase in antibody titers with a reduction of eosinophilia and normalization of peripheral blood parameters. There were negative results of ELISA with Echinococcus antigen in 9 (42.9%) patients prior to surgery and only in 4 (19.1%) patients a month after surgery. Five patients had a high risk of recurrence due to larval dissemination during surgery. The diagnosis of taeniarhynchosis was based on the results of parasitological examination of helminth segments before and after antihelminthic therapy. Examination of the scrapped perianal material revealed taeniid eggs in 62.5% of the patients. Conclusion. One of the important factors in optimizing the diagnosis of parasitic diseases is the choice of a method and the frequency of examinations, as well as the parallel application of all direct and immunological methods for the laboratory and instrumental verification of a diagnosis.

ANTIVIRAL THERAPY FOR FACIAL NEUROPATHY ASSOCIATED WITH HERPES SIMPLEX VIRUS TYPE 1

Previous investigations have shown the value of herpes simplex virus 1 (HSV-1) in the development of nervous system lesions. Lesions of cranial nerves occur in patients of any age and substantially disrupt a patient’s ability to work and quality of life. Facial neuropathy (Bell’s palsy) is most common. This disease is characterized by the high rate of detection of HSV-1 markers and by the efficiency of antiviral therapy. However, the high prevalence of HSV-1 infection in the population, including also among healthy individuals cannot be viewed as convincing evidence for the role of the virus in the development of Bell’s palsy. The paper gives a clinical example of Bell’s palsy developing in a 27-year-old female patient with an exacerbation of HSV-1-induced infection. The treatment with anti-inflammatory and neurotropic drugs in combination with microcirculation-improving agents and non-drug therapy were ineffective. Lower-grade fever, facial asymmetry complaints, and headache persisted after a cycle of treatment. Blood tests showed lymphocytosis and the higher titers of IgM and IgG antibodies to HSV-1. The use of acyclovir resulted in the relief of symptoms.

THE VALUE OF CHEMOPROPHYLAXIS OF TUBERCULOSIS AMONG HIV-INFECTED PATIENTS

The growing epidemic of HIV infection plays a considerable negative role the development of the tuberculosis (TB) epidemic situation. The diagnosis and treatment of latent tuberculosis infection are of special significance for HIV-infected patients because they are at high risk for active TB forms. In this connection, the role of chemoprophylaxis is great for this category of patients, which can reduce the risk of TB by 90%. The paper gives data on the role of TB chemoprophylaxis in HIV- infected patients now when drug-resistant TB is spreading. Under the existing conditions, low TB chemoprophylaxis coverage for HIV-infected patients increases the number of newly diagnosed TB cases, by worsening the overall epidemic situation in the country. The importance of affordable and convenient regimens for chemoprophylaxis of TB and for relief of adverse reactions is underscored.
