Mastoid obliteration using platelet-rich plasma

The aim of the research.  Increasing the effectiveness of treatment of children with chronic purulent otitis media with cholesteatoma by improving the method of obliteration of the mastoid cavity.

Materials and methods.  At the Krasnoyarsk Interdistrict Children's Hospital No. 4, Children's Department of Purulent Surgery, 54 children with chronic purulent otitis media with cholesteatoma underwent reoperation in the period from 2019 to 2024. In group 1 we used bone chips for mastoid obliteration, in group 2 bone chips, platelet-rich plasma and plasma gel were used. All children underwent a comprehensive examination upon admission and 1 year after surgery.

Results.  In otomicroscopy and MSCT/MRI in non-EPI DWI mode of the temporal bones after 1 year in the group where the classical method of obliteration with bone chips was used, recurrence of cholesteatoma was detected in 36% of patients. When using PRP and plasma gel in addition to bone chips, cholesteatoma was detected in 1 patient (4%).

Conclusion. The technique is characterized by simplicity of implementation, safety and has demonstrated effectiveness in patients with chronic purulent otitis media with cholesteatoma.
