A rare case of a combination of cholesteatoma of the external auditory canal and obturation keratosis on different ears

Abstract
Previously, it was believed that cholesteatoma of the external auditory canal (EECC) and obstructive keratosis (OK) represent the same pathological process. It has now been proven that these are two different diseases. Usually, EECC has a unilateral lesion, and OK is more often bilateral, which is one of the points of differential diagnosis. For the first time, we describe a rare case of a combination of both nosological forms, OK in one ear and EECC in the other, in the same patient, dynamic follow-up and treatment for 25 years. We have once again confirmed the opinion of a number of authors that long-term conservative treatment of OK is possible and effective, but requires high-quality dynamic monitoring and an ear toilet with micro exenteration every 4-6 months. With EECC, surgical treatment of various volumes is required, depending on the stage of the disease. The occurrence of EECC in the other ear against the background of OK in the observed ear is possible, as is the transition of OK to EECC with ear injury, careless toilet on the part of the patient or doctor, especially under unfavorable external conditions and provoking factors. An analytical review of the literature is provided.
