Acute intracranial hemorrhages: pathogenesis, diagnosis, correction of impaired cerebral hemodynamics

Abstract

Cerebral blood flow dynamics and its dependence on a state of systemic hemodynamics and intracranial pressure were investigated. Informative potentialities of different methods, used for studying a state of cere­bral circulation, were estimated from the point of view of prognosticating a postoperative course and asses­sing efficacy of treatment of neurosurgical patients. Disorders of cerebral circulation in intracranial hemo­rrhages of different etiology were compared. Structural and functional approaches, combined in studying disturbances of cerebral hemodynamics, allowed to reveal general peculiarities of organ blood circulation, phenomena of autoreguation and functional hyperemia.

The study of immune mechanisms of development of atherosclerosis and new methods of its diagnostics ant treatment

Abstract

The autoimmune lipoprotein-antibody complex was isolated from the blood and the vessel wall of man and high atherogenecity was shown to be mainly due to its citotoxicity and the ability to induce formation of foam cells. It was found that high density lipoproteins (HDL) retard peroxidative modification of low density lipoproteins and formation of autoimmune complexes. Proposed ratio of non-HDL cholesterol to HDL choles­terol in blood, referred to as cholesterol coefficient atherogenecity, is widely used nowadays, account for dan­ger of atherosclerosis development. Reliable data were obtained in favor of atherosclerosis being chronic immune inflammation. New methods of diagnostics based on determination of lymphocyte sensibilisation to lipoproteins and quantitative determination of autoimmune lipoprotein-antibody complexes in patient’s blood were developed. Besides, the methods for treatment of atherosclerosis were worked out and introduced in practice aimed at prevention of formation and elimination from blood of modified lipoproteins and lipopro­tein-antibody complexes.

Abstract

The system and new methods of research of functional condition of the heart of animals and man based on multichannel synchronous measurements, the computer analysis and mathematical modelling of cardio­electric field parameters inside cardiac musle and on the heart and body surface are developed. Unique experimental data on the parameters of cardioelectric field of man and various classes of vertebrates which are the physiological base for verification of theoretical models of cardioelectric field formation and creation of cardio-diagnostics methods of new generation are obtained.

The problem of womens reproductive system care in ecological crisis

Abstract

The woman reproductive system is very sensitive to harmful environmental factors of even weak intensi­ty. Findings demonstrate disadvantageous (adverse) influence of constant and variable biotic and non-biotic environmental factors (including manufacturing conditions) on pregnancy and labor, fetus and infant state. Prophylaxis and corrections of ecology-dependent impairment of reproductive function and health disorders of fetus and infant are of key significance for prophylactic medicine in its entirety: we can speak about social and individual prophylaxis of very broad disease range which determine national health now and in the future. Researches of general and partial items of ecology reproductology acquire the essential significance at pre­sent when demographics became baffling and will remain similar for at least 15 years according expert’s conclusion.

Abstract

In review article, the current conceptual and methodological directions in surgical infection area are dis­cussed. The interdisciplinary approach to complex clinical challenge is reviewed. The surgical aspects of soft tissue infections are done from author’s point of view. A new possible approach to treatment necrotizing fasci­itis and cellulites has been developed. The clinical patterns for evidenced based data using in treatment severe sepsis are presented.

Interferon induction: New approaches in generating functional inductors

Abstract

Interferons type (IFN-a/p) I are the most important factors of a body defense against viral and bacterial infections. Indeed, these proteins induction create first line of protection against infections and due to these functions are used in medical practice for treatment of acute and chronic viral diseases. However, long-term application of a recombinant IFN-a/p leads to understanding of practical demand of interferon inducers. Physiological induction of endogenous IFN-a/p have evident preference in comparison with injection of a recombinant IFN-a/p preparations in high dosages, as it’s apply usually in treatment of hepatitis C or cancer. Physiological induction of IFN-a/p is based on utilization of a natural pathways of activation of IFN-a/p gene’s expression, specific set of cellular receptors and cellular signal systems, involved in genetic control of innate immune response. Therefore, for development of a new approaches in creation of an interferon inducers fun­damental research had been needed in a field of physiological pathways of interferon regulation and mecha­nisms of down regulation and inhibition of a cellular antiviral defense. IFN type I and II have the main contri­bution in this defense as a key factors of innate immune response. As a result of these studies structure and function of IFN-a/p, their genes, IFN-a/p - receptor interaction, and intracellular signaling cascade involved in gene regulation are deciphered.

Among leading IFN-inductors are dibasol, imiquimod, cycloferon and ds-RNA preparations synthetic and natural origin. Benzimidazole derivatives had been studied in the Institute of Experimental medicine from 50th years till 70th. As a result of these studies dibasol and imiquimod analogues were approved for medical application. The same could be noted for acridone derivatives and origin of cycloferon. This preparation is rather unique and belongs to acridine’s set of compounds, well known in international pharmacology. First studies of acridone derivatives were initiated in the Institute of Experimental medicine at 70th years in a frame of investigation anti-tumor antibiotic actinomycine D acricine - ring analogues/ All of these compounds were synthesized in Department of Dye chemistry Leningrad’s Institute of Technology. Most of those compounds elicited wide spectrum of biological activities. Key activity of those compounds was identified as a unique property: high capability to induce endogenous interferon synthesis. These compounds lacked capability to intercalate to the DNA molecules. Later several compounds were approved as strong an interferon inducers, for activation of a first line protection against viral infections

Modern strategy of surgical rehabilitation in oncoproctology

Abstract

The article is dedicated to the comprehensive approach aimed at the management of undesirable seque­lae of surgery for rectal cancer. The effectiveness and expediency of sphincter-sparing surgery for rectal car­cinomas located closer to the anal sphincter is proved. The operation with partial resection of anal sphincter and its substitution with colonic tissues autograft is offered. Results of continent perineal stoma as primary surgical rehabilitation after abdomino-perineal excision are reported. Also surgical strategy aimed at reestab­lishing of intestinal and urinary continuity in patients, who are undergone pelvic exenteration, is described.
