Continuous monitoring of lung sounds by electronic auscultation – reliable method for assessing asthma control

Abstract
Introduction. The assessment of asthma control typically relies on clinical evaluation, patient questionnaires, and pulmonary function tests (PFT). Lung sound monitoring has the potential to optimize the diagnosis of nocturnal asthma and assess the degree of asthma control.
Objective. To evaluate the reliability of continuous monitoring of respiratory sounds in patients with varying degrees of asthma control and to assess the dynamics of wheezing.
Materials and Methods. Eighty-six patients with asthma were enrolled in the study. Patients were categorized into three groups: Group 1, well-controlled asthma; Group 2, partially controlled asthma; and Group 3, uncontrolled asthma. Spirometry was performed using a Vitalograph ALPHA spirometer (UK). Lung sounds were monitored using an electronic auscultation method developed in-house. The resulting audio files underwent computer-based analysis.
Results. Episodes of wheezing demonstrated a direct correlation with the degree of asthma control and the severity of asthma. In Group 3, the mean frequency of nocturnal episodes reached 3.3 ± 0.4 (p < 0.05). Monitoring revealed a strong positive correlation between the number of wheezing episodes and the degree of asthma control (r = 0.74, p < 0.05). A moderate positive correlation was observed between wheezing episode frequency and obstructive PFT abnormalities, as well as subjective symptom scores (r = 0.39, p < 0.05). A weak positive correlation was noted between the duration of wheezing episodes and the severity of PFT abnormalities (r = 0.23, p < 0.05).
Conclusion. Continuous lung sound monitoring is a reliable method for monitoring the course of asthma, enabling the registration and assessment of respiratory sound dynamics.
