THE EFFECTIVENESS OF SPHEROGEL IN THE TREATMENT OF KNEE OSTEOARTHRITIS

Abstract

The resuts of randomized, double-blind, placebo-controlled, prospective trial in parallel groups of evaluation the efficiency safety and tolerability threefold (once a week) intraarticular injection of Spherogel are discussed in the study. It is estimated of positive effect of this medication to simptoms of gonarthritis: reducing of pain and improvement of functional activity of joint with a good tolerability. This effect continuos for least 3 month.

CHARACTRISTICS OF CYTOKINES, PHENOTIPIC FEATURES AND NEUTROPHILSPHAGOCYTOSIS ACTIVITY IN ASTHMATIC PATIENTS DURING EXACERBATION

Abstract

The aim of our study was to measure the level of IL -8, IL-6, IL-4, IL-17 depending on spontaneous or induced release by peripheral blood leukocytes in patients with bronchial asthma exacerbation with comparative analysis of asthmatics' blood neutrophils phenotypic features. The obtained data indicate that the level either pro-inflammatory or anti-inflammatory cytokines was increased in patients with asthma exacerbations and significant increase of concentration of CD45+CD66b+CD11b+ cells in comparing with healthy donors. We determined that during asthma exacerbation the concentration of CD45+, CD66b+, CD11b+ blood granulocytes almost doubled in comparing with healthy donors. Also there is a significant increasing of blood granulocytes' phagocytosis activity. We observed the elevated level of spontaneous production of inflammatory mediators. Moreover, the stimulation of cytokines synthesis and release in patients with asthma does not always lead to increase of induced level of cytokines in contrast to healthy donors.

TRANSPLANTATION OF GENETIC MODIFIED STEM CELLS OVEREXPRESSING THERAPEUTIC GROWTH FACTORSFOR THE TREATMENT OF EXPERIMENTAL RAT MYOCARDIAL INFARCTION

Abstract

We tested the hypothesis that simultaneous transgenic overexpression of a select quartet of growth factors activates diverse signaling pathways for mobilization and participation of various stem/progenitor cells for cardiogenesis in the infarcted heart. IGF-1, VEGF, SDF-1α, and HGF plasmids were synthesized and transfected into rat skeletal myoblasts (SM). Overexpression of growth factors in transfected SM (TransSM) was confirmed by reverse transcription PCR, western blotting, and fluorescence immunostaining. The conditioned medium (CM) from TransSM was cytoprotective for cardiomyocytes following H 2O 2 treatment, promoted a higher transwell migration of HUVEC cells and in vitro tube formation. Intramyocardial transplantation of 1.5×10 6 TransSM in a rat model of acute myocardial infarction induced extensive mobilization of stem/progenitor cells into the infarcted heart on day 7 and improved integration of TransSM in the heart compared to NatSM. Extensive neomyogenesis and angiogenesis with resultant attenuation of infarct size and improvement in global heart function was observed at 8 weeks. In conclusion, simultaneous activation of diverse signaling pathways by overexpression of multiple growth factors caused massive mobilization and homing of stem/progenitor cells from peripheral circulation, the bone marrow, and the heart for accelerated repair of the infarcted myocardium.

Abstract

It is described the clinical case of a rare disease – cholangiocarcinoma of the common bile duct, which developed in patient with chronic pseudotumor-like pancreatitis. It is reflected the complexity of diagnosis at an early stage of the disease. The method of confocal laser endomicroscopy was for the first time applied for the given pathology for diagnostic purposes that allowed to clarify and verify the diagnosis. The authors suggest that in diseases of the pancreato-biliary zone the method of confocal laser endomicroscopy can be crucial in cases of inefficiency or uninformativeness of other methods.

Dynamic observation of the patient during 12 months confirmed the success of combined treatment – absence of arrhythmia recurrences and decreasing of NYHA heart failure functional class.

PRE-OPERATIVE THERAPEUTIC PREPARATION FOR PATIENTS IN ONCOSURGERY

The paper deals with current possibilities of pre-operative therapeutic assessment and the management of patients with concurrent cancer and cardiac diseases.Adequate diagnostics, modern surgery and therapeutic methods in pre- and postoperative period allow to survive 12 months without features of both diseases in 68.4% of patients. 3-5 year survival rate is respectively 25.8-12.9%.
