The role of polymorphisms genes of detoxification of xenobiotics in the development of endometriosis

Abstract

The review presents the results of investigations of gene polymorphism of enzymes involved in estrogen metabolism: CYP1A1, CYP1A2, CYP19 and SULT1A1 in the buccal epithelium samples from 703 women by RFLP (Restriction Fragment Length Polymorphism). The significant association of the presence of allele C and genotype T / C and C / C gene CYP1A1, allele A and genotype C / A and A / A CYP1A2 gene and the T allele and genotype C / T and C / C gene SYP19 with the risk of adenomyosis.

Optimization of a cytologic and bacteriological assessment of a pathological secret at chronic duktoforit at women with a mastopatiya

Abstract

The given paper describes high efficiency of detecting chronic inflammation of the mammary ducts (96.2 %) in women with mastopathy by enlargement of cellular composition by means of optimization for the technique of intraductal secretion sampling with the help of mammary duct lavage in comparison with exfoliative method (touch smear) (24.8 %, p < 0.001). The technique of sampling with the help of mammary duct lavage increases detectability of pathogenic flora (82.7 %, p < 0.001) by 2.6 times in comparison with exfoliative method (31.6 %, p < 0.001). It has been established that preliminary sanitation of secerning ducts of a mammary gland prior to ductography increases by 10 times information value for detecting distorting inflammation of the mammary ducts (24.7 %) in comparison with contrast mammography without preliminary ductal lavage (2.5 %, p < 0.001).

THE PECULIARITIES OF CHANGES IN THE STRUCTURE OF PSYCHOEMOTIONAL SPHERE IN WOMEN DURING THE GESTATION FLAVORS OF PREGNANCY

Abstract

We examined the structure of psychoemotional sphere of 153 pregnant patients in different terms of pregnancy and 48 non-pregnant women. Our data demonstrated that first-term pregnancy is accompanied by a significant emotional stress. In second-term pregnancy those changes are more positive, at third-term pregnancy the level of the patients feels is significantly increased and the psychoemotional sphere is stabilized. Thereby the specific features of nervous system are influence the status of the pregnant women. Evidently pregnant patients with the high level of anxiety, introversion, psychotic and nevrotic characteristics are in need of the increased attention of the obstetric-gynecologic specialists, anaesthesiologists and psychologists to make correction of the psychoemotional sphere when preventive preparation of birth and the choice of anesthesia delivery.

The Immunomorphological condition placentae of the women connected with tobacco smoking

Abstract

Immunomorphological, histologic research with application of semi-thin cuts bioptats placentae of the women connected with tobacco smoking to pregnancy, during all pregnancy and in the I trimester of pregnancy is carried out. Immunomorphological researches of placentae of studied groups showed that the greatest percent of the revealed fixed pathogenic immune complexes (PEAK) including existence fibrinogen, C3 fraction of a compliment, antibodies and pro-inflammatory citokins, were observed in group of women which smoked all pregnancy. The smallest percent of identification PEAK was in a placenta of the women, stopped to smoke to pregnancy. By parallel kliniko-laboratory comparison it is established that the greatest frequency of complications of pregnancy is observed at women who smoked all pregnancy. At histologic research of placentae of women of studied groups in the field of adjournment the PEAK is observed tsirkulyatorno-dystrophic and necrotic processes. The most widespread changes of a placenta are found at the smokers which pregnancy became complicated the gestozy. In the field of adjournment the PEAK leads damages of membrane structures of a placentary barrier to destructive changes of a placenta, to development of immunopathological process with violation of an immune homeostasis and to development of placentary insufficiency that is adversely reflected in an outcome of pregnancy and childbirth, and also on a fruit and newborn condition.

Characteristics of clinical progress of vaginal delivery after cesarean section

Abstract

The 982 cases of women with uterine scar after cesarean section were analyzed. Clinical criterion of uterine scar consistency is the “maturity” uterine cervix and greater response of uterine cervix to the delivery preparation. Characteristics of vaginal delivery after cesarean section clinical progress are the following: spontaneous labor onset (95,5 %), abnormal labor (11,9 %), premature rupture of membranes (46,4 %). Induction of labor caused increased the frequency of dystocia by a factor of 2, whereas risk of repeated cesarean section increased 2.3. Frequency of the uterine rupture in these labor cases was 0,73 %.

EVALUATION OF COMBINED NEFOPAM HYDROCHLORIDE AND KETOPROFEN IN THE TREATMENT OF POSTOPERATIVE PAIN IN PATIENTS WITH ENDOMETRIOSIS

Abstract

Conducted a study to assess the effectiveness of a new centrally acting analgesic nefopam hydrochloride (Acupan). Included 45 patients who underwent laparoscopic surgical treatment of endometriosis of varying severity. Found that pre-emptive analgesia with nefopam and ketoprofen is more effective in patients with endometriosis, I–II severity. In patients with endometriosis stage III application ketoprofen and nefopam significantly improved the quality of postoperative analgesia and reduced the need for narcotic analgesics. In patients with severe endometriosis (IV stage), this scheme was inadequate.

The results of surgical treatment of 315 patients with genitals prolapse are presented in the paper. Reasonability of utilization of synthetic materials from polypropylene for genitals prolapse correction depending on the operative intervention access and implantable material amount is displayed. Efficiency of urethropexies of trans-vaginal tapings is confirmed by concomitant urinary incontinence of stress genesis.

The paper presents the evaluation of ovarian reserve of women with impaired reproductive function. Prognostic significance of the coefficients are given of factors that contributed to reduce the ovarian reserve in women of childbearing age, as an additional factor considered thrombophilia. Knowledge of the factors responsible for ovarian reserve, can form a risk group and carry out measures for its conservation and implementation of reproductive function.
