Аннотация
Представлены эпидемиология, клиническая биохимическая и молекулярно-генетическая характеристика гликосфинголипидозов, сопровождающихся нарушением метаболизма и избыточным накоплением в различных паренхиматозных органах, костном и головном мозге не только сфинголипидов, но и свободного холестерина. Прежде всего, это сфингомиелиновый липидоз, или болезнь Ниманна – Пика, — клинически полиморфная и генетически гетерогенная группа редких моногенных заболеваний. Различающиеся по дебюту и тяжести течения типы А и В являются аллельными заболеваниями и обусловлены присутствием рецессивных мутаций в гене лизосомной кислой сфингомиелиназы (SMPD1). Тип А — это классическая острая нейронопатия, при которой в 85 % случаев болезнь дебютирует до 6 мес., смерть наступает в возрасте до 3 лет. Причиной заболевания становятся мутации с преждевременной терминацией трансляции или тяжелыми нарушениями каталитической активности фермента. При типе В чаще обнаруживаются миссенс-мутации. Это хроническая висцеральная форма, при которой неврологическая симптоматика, как правило, отсутствует, и больные доживают до подросткового возраста. Юношеские и взрослые формы хронической нейронопатии типа C генетически гетерогенны. В 95 % случаев они обусловлены мутациями в гене NPC1 (тип С1) и в 5 % — в гене NPC2 (тип С2). Продукты этих генов — трансмембранные белки, отвечающие за транспорт холестерина и других липидов. Болезнь накопления эфиров холестерина, или болезнь Вольмана, обусловлена наследственной недостаточностью лизосомной кислой липазы А. Обсуждается возможность ранней диагностики этих заболеваний на базе неонатального скрининга с целью повышения эффективности их профилактики и лечения. Подчеркнуто значение экспериментальных моделей для изучения молекулярных основ патогенеза этих тяжелых наследственных заболеваний и разработки различных терапевтических подходов, таких как трансплантация костного мозга, ферментная заместительная терапия и субстратредуцирующая терапия. Представлен клинический пример болезни Ниманна – Пика типа С.
