Prospective planning of activity of the Medical service of the Armed Forces of the Russian Federation for 2016-2020

Abstract

The article reflects the characteristics of the Plan of activities of the Ministry of Defence of the Russian Federation for 2016-2020 - an important long-term planning document of the Armed Forces. It stressed the need for synchronization of activities for chain of command and deadline. Presented structure of the Medical Service Plan Military District operations (fleet) military medical organization. The attention is focused on the content of its sections. For example, a military hospital layouts presented an action plan and a schedule of events. Reflecting the requirements of the Minister of Defense for the development and adjustment plans, indicators of their performance.

Modern technology in assessing predisposition to deviant behaviour among servicemen.

Abstract

One of the major problems in the acquisition of units of the Armed Forces is timely detection of individuals with deviant behaviour, which are prone to delinquent and addictive behaviour. The article presents the results of an analysis of existing methodologies aiming to identify servicemen abusing and occasionally using narcotic drugs and psychotropic substances. The characteristics of the proposed new automated complex psychological diagnosis socially determined behavioural disorders and methods is given. The high efficiency of this characteristics in the allocation among «at risk» servicemen is given.

Comparative evaluation of drainage methods used for penetrating gunshot cerebral injuries.

Abstract

The paper presents a comparative evaluation of the methods of gunshot craniocerebral wounds draining, applied when the primary surgical treatment at the stages of medical evacuation of the limited contingent of Soviet troops in Afghanistan. An analysis of occurring infectious complications and outcomes of surgical treatment of the wounded is given. Based on clinical observations revealed most effective method of the active drainage gunshot traumatic brain injuries. The advantages of tidal drainage, appropriate for mechanical cleaning of wounds, allows controlling hemostasis, using antibacterial solutions are given.

Preparation for assisted reproductive technology in the course of infertility treatment in the female soldiers

Abstract

The level of obstetric morbidity in servicewomen remains high. Infertility occurs more often among the families of servicemen, than among the other families. The leading causative factor among the families suffering from infertility is tuboperitoneal or tubal (up to 85%). Assisted reproductive technologies are often the only possible mean to solve the problem of infertility in case of these forms of infertility. The families of servicemen suffering from infertility were suggested the echelon principle of health care in military-medical institutions of the Ministry of Defence of the Russian Federation. Defined selection rules and directions, requiring the separation of the assisted reproductive technologies Clinic of Obstetrics and Gynaecology of the Kirov Military Medical Academy to carry out in vitro fertilization procedures.

Sanatorium-and-surgical treatment of patients with complicated spinal trauma in a specialized department of a sanatorium.

Abstract

We studied the results of sanatorium treatment of 256 patients with complicated spinal injury and decubital ulcers. 74% of patients were male patients aged 38,5±11,8 years. 89,4% of patients suffered from severe abnormalities, mild abnormalities were registered in 10,6% of patients. In 82,6% of patients decubital ulcers were localized to the skin only on one site, in 17,4% was registered multiple localization. Duration of the patients’ follow-up varied from one to three years or more. Patients were divided into 2 clinical groups: the first group consisted of 132 patients who were applied during the preoperative preparation balneological factors and nanosilver, 124 patients included in the second group, which preoperative preparation was carried out in conventional manner. Using complex medical factors in preparation for operations contributed to reducing the duration of training from 26±3 days to 14±2 days. The application of complex restorative treatment system allowed in the first group compared with the second group increased the number of patients with good results.

Multiple sclerosis vaccine: possibilities of the use of dendritic cells immunomodulatory potential

Abstract

For the first time we carried out a clinical assessment of the safety, tolerability and clinical efficacy course of repeated administration of experimental modified autologous vaccine interlеykin (IL-10) dendritic cells in two patients with secondary-progressive multiple sclerosis patient and one with relapsing-remitting multiple sclerosis. In the course of treatment, we carried out clinical and immunological monitoring. It was found out that intradermal dose of 3 ґ 106 cells applied to spinal area 6-12 times did not cause any serious side effects. After the treatment with dendritic cells the following results were observed: 1) a significant positive clinical effect in patients with secondary-progressive multiple sclerosis exacerbations; 2) moderate positive clinical effect in patients with relapsing-remitting multiple sclerosis, in a state of remission; 3) a complete absence of any clinical results in patients with secondary-progressive multiple sclerosis without exacerbations. The immune response was characterized by a significant absolute and relative increase of serum T-regulatory cells. Discovered distinct anti-inflammatory properties of dendritic cell therapy allow us to consider it as a promising area of personalized treatment based on an individual vaccination against multiple sclerosis.

Modern means of statokinetic improvement and vestibular rehabilitation

Abstract

The article presents the results of studying the effectiveness of the complex vestibular workouts based on non-invasive neuromodulation technology in 37 healthy patients aged 18-20 years who have symptoms of aerial sickness in 5 or less minutes. Non-invasive neuromodulation wire-familiarize with the apparatus for vestibular rehabilitation «Brain-Port» (USA), performing electric tactile stimulation of the tongue with biofeedback. An indicator of statokinetic stability was considered from time to tolerance to the vestibular load up to development of vegetative manifestations of aerial sickness. Improvement of statokinetic tolerability vestibular training confirmed by computed posturography and gait analysis. Increase of statokinetic stability accompanied not only by an increase in exposure, necessary for motion sickness, but also a decrease in autonomic manifestations, which are the main obstacle to the operator’s military activities. Improving balance and gait performance after the swipe-of vestibular training course demonstrates the possible realization of two mechanisms of compensatory and restorative processes: sensory substitution equilibrium function and induction of neuroplasticity in short time.

Сell engineering in the treatment of diabetes mellitus: current state and prospectives

Abstract

The article is dedicated to the current state and prospectives of cell engineering in the treatment of diabetes mellitus. The material is based on the analysis of clinical trials in which cell engineering has been applied for the treatment of diabetes mellitus and its complications. It has been shown that despite the decade of intensive research the clinically significant results have not been achieved yet. At the same time the fast growth of the number of publications in the field testified to intensity of basic and applied research and the large quantity of ongoing clinical trials on the topic make it possible to look at the prospectives of the employment of cell engineering in the treatment of diabetes mellitus with a certain optimism.

Use of «Lokus» as a part of a mobile vacuum system of wounds treatment

Abstract

The results of treatment of wounds of various aetiologies with a method of vacuum therapy with urethane-foam as a drainage material are given. Medical blanket «Lokus» is notable for the method of application, as opposed to conventional polyurethane materials: «Lokus» applied to the wound in a semiliquid state reduplicating its relief and contour. Polymerization occurs within a few minutes after applying; the result of polymerization is a urethane-foam sponge, which is similar to the sponge specially applied for the vacuum treatment of wounds. A further method of treatment did not differ from that in the conventional treatment of wounds using negative pressure. The method was used as a stage for the preparation of a plastic closure of wounds. In all cases, the treatment of wounds marked positive, is to accelerate the change of wound healing phases.

Study of the level of histamine in the peritoneal fluid in case of peritonitis

Abstract

The analysis of the histamine levels in the peritoneal fluid of patients with peritonitis caused by diseases and injuries of the upper urinary tracts. Patients of the main group, in addition to conventional methods of treatment, were carried lymphotropic administration of amikatsin and processing the abdomen exogenous nitrogen monoxide. On the fifth day the restoration of normal levels of histamine was registered in patients. In patients from control group receiving conventional treatment, histamine levels by this time was 3 times higher.

Clinical biochemical and toxicological evaluation of Silybum marianum

Abstract

A dosage form with Silybum marianum oil meal as a mean for correcting liver condition is developed. To evaluate the effectiveness and tolerability profile was performed a study involving 40 volunteers with destructive changes of the liver. On completing the study, it was revealed that the given medical product has the following effects: hepatoprotective activity of the developed formula, reduction of the risk of bile duct obstruction, prolonged beneficial effect on the functional status of hepatocytes. The medical product did not cause toxic side effects and showed a good tolerability profile.

Sanitation measures of the final period of World War II included the sanitary surveillance of water and power forces, observation of their placement and content of the territory occupied by them, monitor the implementation of military rules of personal hygiene and health education. The content of anti-epidemic measures was control of vectors of infectious and parasitic diseases, the protection of troops against the penetration of these diseases from the outside, the sanitary-epidemiological investigation and vaccination of staff, early detection and isolation of infectious patients, their timely hospitalization, disinfection of the source of infection, identify the sources of infection and epidemiological surveillance behind the hearth. Epidemiological welfare of the Red Army has been achieved by the virtue of the hard and persistent work of many thousands of military doctors, good organization of anti-epidemic protection of troops and use of military medical service of science.

Abstract

Koshelev N.F. made a significant contribution to the development of general and military hygiene. Under his leadership at the S.M.Kirov Military Medical Academy were actively conducted researchers in the field of general hygiene, hygiene, military labour, water, food and sanitation support for the troops formed a new way of hygienic structure understanding, its goals, methods, object and subject of study. Koshelev N.F. developed provisions of methodological basics, structure and algorithm of hygienic diagnostics. He also defined basic principles of the universal theory of hygienic regulation and developed its structure, made a number of other original theoretical positions. Koshelev N.F. created a scientific school, continues the best traditions of domestic hygiene.

Deployment and organization of off-design airfield evacuation care center

Abstract

The aim of the article is to discuss the deployment experience off-design airfield evacuation care center and the formulation of conclusions and practical recommendations on the organization of its work. The article is based on experience carried out by the medical service of the Central Military District during 24-28 August 2015 of special tactical exercise to ensure the comprehensive group of troops. The challenge for the deployment of off-design airfield care center was delivered suddenly, due to a sharp change in weather conditions at a number of airports and the ensuing changes in the direction of the medical evacuation of casualties from the area of an emergency.
