Methodical approaches to evaluating the effectiveness of implementing the results of project activities in the practice of military health care

Abstract

Methodical approaches to evaluating the effectiveness of implementing the results of project activities in the practice of military healthcare. The article outlines methodological approaches to assessing the effectiveness of implementing the results of project activities in the practice of medical support for the Armed Forces. The assessment methodology has been developed. Its objects, criteria for the effectiveness of implementation are determined: medical, economic, social, etc. The characteristic of two approaches to the integral assessment of the effectiveness of the implementation of the results of project activities - multi-criteria and single criteria is presented. Along with a quantitative assessment showing the degree of achievement of a standard result in fractions of a unit or a percentage, a qualitative assessment of the effectiveness of introducing the results of project activities into the system of medical support for troops by the level of reliability was proposed. It is shown that the work of medical personnel of the Armed Forces can be attributed to productive work, and its costs should be included into the costing of the reproduction of labor-power. Therefore, this work should be allocated to a special, medical, branch of social production.

Features of the organization and conduct of clinical and military trials involving military personnel. The directions of research of military drugs and technical equipment of the medical service, the tests of which involve the involvement of volunteers, are outlined. Reflects aspects of attracting military personnel, considering the characteristics of factors of military labor, and identified the needs for the formation of a legislative framework for the development of research on the system of pharmacological support for military labor. The possibilities of the scientific and clinical center of the Institute for the organization of testing samples of weapons and medical equipment with the participation of volunteers are indicated.

Abstract

The main results of research work on the creation at the institute of means of indicating and identifying highly toxic substances, antidotes, means of pathogenetic and symptomatic therapy of poisoning, means and methods of partial sanitization of skin and clothing are presented. Particular attention is paid to the prevention and emergency treatment of intoxication with organophosphate compounds, pulmonary toxicants, relief of toxic convulsive syndrome. A number of promising ways of increasing the effectiveness of the indication of highly toxic substances, emergency medical care for those affected, as well as preventing the development of organophosphate neuropathies leading to disability of the injured were noted. Methodical approaches to increasing the effectiveness of military professional activities of military personnel in high mountains have been substantiated, and preparations have been selected for the prevention of altitude sickness.

Prospects for the use of cochlear implantation in the Armed Forces of the Russian Federation

Abstract

According to the results of their own research, performed by employees of the Department of Otolaryngology at the S.M.Kirov Military Medical Academy in peacetime and wartime, as well as literature data, in 70-75% of cases, victims and wounded after mine-explosive wounds in the acute and distant period have impaired function of the auditory system of varying severity. Mine-explosive injury leads to an increase of irreversible pathological changes in the auditory system and the development of sensorineural hearing loss, including deafness and disability of the victims. Prolonged or pronounced short-term exposure to military labor factors also leads to the development of III-IV-degree sensorineural hearing loss in military personnel, deafness. A modern method for the restoration of auditory function is cochlear implantation, which can be carried out with mine-explosive damage to the auditory system one year after suffering trauma in the absence of post-traumatic deformity of the cochlea.

Features of blood transfusions in the period of traumatic shock in patients with severe concomitant injury with acute massive blood loss of severe and extremely severe stage

Abstract

Acute massive blood loss remains the leading cause of death in the first hours of traumatic disease in more than 50% of deceased patients. In this regard, in the case of acute massive and extremely severe blood loss and delayed evacuation of the wounded, it is advisable to study the efficacy and safety of transfusions of red blood-containing blood products of the universal donor (0 (I) Rh-), starting with the first level of care (first aid, first medical). It has been established that involuntary use of red blood cells containing blood products of a universal donor in the first hours after injury in victims with acute massive severe blood loss is as effective as the use of phenotyped red blood cells containing drugs authentic in group and rhesus affiliation and is not accompanied by the development of blood transfusion reactions. In addition, early (in the first 6 hours) and complete replenishment of acute massive blood loss of an extremely severe degree compared with double replenishment of such blood loss (within 24 hours) is accompanied by a more stable course of traumatic disease, reduces the required volume of blood transfusions in the first 3 days (on average 0.96± 0.11 l), and does not entail an increase in the frequency and severity of the massive hemotransfusion syndrome.

System of comprehensive immunization in the Armed Forces: state and prospects for improvement

Abstract

In the Russian army, immunization is carried out according to the vaccination schedule and the vaccination schedule according to epidemic indications. The system of mass immunoprophylaxis of personnel, based on the use of associations and complexes of immunobiological drugs, implemented with the help of high-performance technical means, helps to ensure the protection of military personnel from actual epidemiological infections and potential biological damaging agents. When developing emergency situations of a biological nature or when sending personnel to regions with a poor epidemic situation, it may be necessary at the same time in a short time with maximum coverage to vaccinate against several infections. In this situation, vaccination with associated or complex drugs, carried out by group (mass) methods of immunization (needleless, aerosol, oral) are of great importance. To effectively and qualitatively carry them out in modern conditions, the activities of the troops (fleet forces) require the modernization of the existing vaccination system for personnel based on the principles of associated and comprehensive immunization. Possible solutions to this problem are discussed.

Abstract

The main stages and results of scientific research in the field of anti-epidemic protection, conducted at the State Research Testing Institute of Military Medicine of the Ministry of Defense of the Russian Federation are presented. For the first time in the country, the institute employees justified the concept of medical protection of troops (fleet forces) and the population from biological weapons, developed guidance and methodological documents on the indication of pathogenic biological agents and decontamination in biological contamination sites, several drugs and technical means of protection against biological weapons. Effective methods and schemes for rapid analysis, diagnostic preparations for the detection of pathogens in natural foci of infections were proposed. Methods of mass vaccination using associations and complexes of vaccines, schemes for emergency prophylaxis and etiotropic treatment of dangerous infectious diseases, original disinfectants, insecticides and repellents have been developed. For the future, scientific work is planned to improve the means and methods of laboratory diagnostics, prevention and etiotropic treatment of dangerous infectious diseases, ensuring biological safety and disinfection in the foci of biological infection, methods of controlling the epidemic process.

Abstract

The article describes the history of research and development work in the field of creating personal protective equipment for military personnel from electromagnetic radiation, presents their results. The need to prepare a special concept of ensuring electromagnetic safety of personnel and the formation of a separate (departmental) regulatory framework governing the development and testing of personal protective equipment was noted. It is necessary to substantiate the feasibility of having their various options for use in various types of Armed Forces and the service of troops, as well as the need to conduct separate departmental tests of these facilities according to certified methods in the specialized center of the Russian Defense Ministry. It is necessary to revise the existing and publish new regulatory documents that establish requirements for the design of personal protective equipment, methods and bench bases to determine their protective and operational performance, as well as to the standards of supply.

Abstract

The experience of an integrated approach to the treatment of alimentary-constitutional obesity and metabolic syndrome in military personnel undergoing military service under the contract is described. Psychotherapeutic correction and non-drug methods of «traditional» medicine were used (ozone therapy, reflex therapy, manual therapy, osteopathy, hirudotherapy). The combined use of these methods allows you to adjust eating behavior, reducing the severity of obesity and metabolic syndrome. Normalization of clinical and laboratory parameters and behavioral stereotypes formed improve health, increase tolerance and resistance to stress, provide better tolerability of negative factors of military labor and increase professional longevity.

Abstract

Some facts are presented concerning the creation in March 1969 of the Research Institute of Military Medicine of the USSR Ministry of Defense (military unit 41598) and its Moscow branch. The main milestones of the activities of the specialized unit in the development of military medical equipment in the framework of reorganization changes of the military medical research complex in the period from 1964 to the present are stated. It is demonstrated on a historical example that an integral part of the work of a research organization is the preservation of personnel potential that ensures the multi-vector activity in solving the tasks being formed. The main efforts should focus on optimizing the scientific potential, creating a unique material and technical base, and organizing the coordinated work of various research teams. An important component of the work of a research team is the maintenance of a scientific school that ensures historical continuity.
