Medical examination of military personnel as a system of preventive and medical-diagnostic measures in the Armed Forces: experience

Abstract

The article describes the historical aspects of the development of the system of medical examination in the Armed Forces, its main problem issues at the present stage. The main documents regulating the organization and content of clinical examination, the characteristics of their content are given. It is shown that in the prophylactic examination of military personnel under the contract there are many more doctors involved, and the volume of diagnostic studies allows to fully achieve the goals set for the medical examination. Methodical approaches to the evaluation of the quality and effectiveness of clinical examination are outlined. The use of modern survey methods, quantitative approach, quality control made it possible to achieve the efficiency of military personnel under contract in IHD at 91,6% at 9 LDCs, 91,2% in hypertensive disease, 96,4% in diseases of the endocrine system. The questions of economic analysis and information support of medical examination are reflected.

General approaches to the formulation of the diagnosis of malignant neoplasm in the conduct of military medical examination

Abstract

Formation of an expert diagnosis of malignant neoplasm during military medical examination of military personnel is an important element of the activity of medical specialists of military medical organizations. The rules for the formulation of the diagnosis remain unchanged in the conduct of military medical examination in order to determine the validity for military service, the granting of sick leave or exemption from the performance of military service duties. Examples of the most common clinical diagnoses of malignant neoplasms are given. The information contained in the clinical and expert diagnosis is always crucial for subsequent patient requests for medical care, characterizes the prognosis of the disease, the nature and duration of medical rehabilitation, the frequency and volume of examinations during dispensary observation, allows you to determine the category of fitness for military service.

Abstract

The results of the questionnaire survey of students of the residency of the S.M.Kirov military medical academy are described in order to determine their professionally important qualities that affect the practical activities, professional competencies and the educational process in the discipline «Emergency medicine». Through feedback from students, their educational needs in the field of disaster medicine have been established, and the following groups of factors have been identified: those who determine the professionally important quality of the listeners are «communicative», «professional» and «personal»; influencing the training, practical activities and professional competence - «methodology of managerial activities of specialists», «organizational and staff structure of the medical service» and «methodology for the intended purpose with the organizational and staff structure of the medical service»; affecting the educational competence - «regulatory legal framework, regulation of the activities of specialists», «methodology for the intended use», «methodology of managerial activities of specialists». Their ranking is done, the significance is established. The directions of the development of professionally important qualities, professional knowledge and activity of military medical specialists in terms of their intended use in eliminating the medical and sanitary consequences of emergencies have been determined.

Ways to solve the problem of the «golden hour» in the pharmacotherapy of acute poisoning among servicemen (Literature review)

Abstract

The article is devoted to the substantiation of approaches to the improvement of the first and medical (pre-medical and first medical) assistance to servicemen in acute poisoning. In the provision of first aid, the effectiveness of medicines is determined by their speed and the possibility of using self-help and mutual assistance. Compliance of these requirements with the medicinal forms used in the military unit of the medical service is analyzed. In acute poisoning, the use of drugs in the injection dosage form at the prehospital stage involves a number of risks.The prerequisites for the use of nasal spray under these conditions are shown. The conditions for the suitability of pharmaceutical substances for use in the nasal spray dosage form for the treatment of acute poisoning in the prehospital stage are formulated.

Peculiarities of elbow joint replacement in case of wounds and injuries: long-term results

Abstract

The aim was to study the results of total elbow arthroplasty in the treatment of gunshot wounds and open injuries. The analysis of treatment and long-term results of 69 patients with gunshot wounds and open injuries of the elbow joint, which performed total endoprosthetics, was carried out. The duration of the preoperative period was 3-16 months after receiving injury or trauma, postoperative follow-up - no less than 12 months after arthroplasty of the joint. The volume of motion in the joint (ROM), functional indices (DASH) and quality of life (SF-36) at 1, 3, 6 and 12 months after the operation were studied. It has been established that both in severe trauma and in gunshot wounds, endoprosthetics of the elbow joint equally reliably reduces the pain syndrome, gives a high level of physical functioning, which improves the quality of life of patients.

Experience in using lumbar sympathectomy in a central military hospital

Abstract

The article presents data from the experience of using lumbar sympathectomy surgery in the treatment of pathology of the main arteries in the department of vascular surgery of the central military hospital for more than 45 years. The efficacy of the operation with different degrees of ischemia was analyzed. Based on an evaluation of the results of 1,764 independent and combined lumbar sympathectomies with vascular reconstructions, it was established that this operation, with minimal trauma and a low level of complications, improves the quality of life and patients for many years. Long-term results show that the compensation of limb blood circulation, achieved due to lumbar sympathectomy, allows the majority of patients to maintain a limb for a long time: in case of II А-B grade ischemia, 91% of patients, 2 years - 90%, 5 years - 62%, 10 years - in 57% of patients; with ischemia of III-IV degree - respectively in 80, 72, 45 and 40%.

Abstract

Clinical examination is one of the methods of early detection of chronic diseases in children and adolescents. The article presents data on the results of medical examination of students of the Boarding School of the Ministry of Defense of Russia for 2011-2016. Optimization of the methods of medical examination based on new scientific data made it possible to identify, correct and prevent various deviations in the health status of pupils in a timely manner, which led to a decrease in 2014-2016. compared with 2011-2013. prevalence of chronic pathology of the nervous system (by 6%), gastrointestinal tract (8,4%), ENT organs (17,3%), cardiovascular system (4%), and the proportion of pupils with 1st group of health increased by 5,7%.

Development of B12-deficiency anemia in a patient with type 2 diabetes mellitus in the treatment of metformin

Abstract

The article describes the case of development of B12-deficiency anemia in a 68-year-old patient with type 2 diabetes mellitus after 4 years of metformin therapy. A brief review of literature data on metformin-induced B12-deficiency anemia and its correction methods is presented. Patients who take metformin for a long time are recommended to study the concentration of vitamin B12 and, at its reduction, to carry out preventive maintenance therapy with vitamin B12 to avoid the development of anemia and/or polyneuropathy. Particular attention in patients with type 2 diabetes should be given early diagnosis of polyneuropathy, which may be associated with the use of metformin, so that, at the first signs of it, an adequate treatment should be prescribed.

Clinical results and morphological state of the buccal graft after replacement urethroplasty

Abstract

Results of treatment of 68 patients who underwent urethroplasty with the use of the mucous membrane of the cheek were presented. The overall effectiveness of performed urethroplasty operations was 89,7%. Histological examination of buccal autografts in patients with recurrent constrictions did not reveal structural changes. The multilayer squamous, non-coring epithelium, which was under prolonged exposure to an aggressive medium, remained unchanged.

Application of hyperbaric oxygenation therapy in the complex treatment of periodontal tissue diseases

Abstract

The experience of using hyperbaric oxygenation in the treatment of 38 patients with periodontal disease in the age of 48-60 years in outpatient settings is presented. The closest result after the course of hyperbaric oxygenation is regarded as positive in almost 100% of patients. In addition to normalizing the periodontological status, patients who received hyperbaric oxygenation improved their general condition, sleep and appetite, and increased their efficiency. The stage of remission in these patients ranged from 1 to 3 years.

Comparative evaluation of protective effectiveness of materials of medical special clothes

Abstract

An experimental comparative evaluation using test-aerosols of the protective effectiveness of materials for medical special clothes was carried out. Tests under static and dynamic conditions have shown that increasing the density of nonwovens, laminating them and increasing ply lead to a sharp decrease in bacterial permeability. The results of the research make it possible to select objectively a material with the necessary characteristics and to issue medical special clothes based on it with the highest possible protective effectiveness.
