Prospects of IT usage in the field of military psychiatry

Abstract

Development of effective information support for monitoring of mental health, based on the principles of complex diagnostics, provides the necessary tools objektive evaluation and prediction of mental health in servicemen. Effectiveness of psychatric prophylaxys technologies depends on compliance with a number of fundamental principles, the main of which should be considered: multidimensional assessment of mental health in servicemen and real cooperation of different specialists (commanders, specialists of medical and psychological service) for its implementation. In this case, an introduction of information and communication technologies on the basis of the district (fleet) and the Central Military Hospital, as well as other bodies of military control will be almost justified only when creating a full-fledged «information field of military psychiatry» that implements the basic principles of the monitoring system of mental health and includes necessary infrastructure. The practical implementation of this system will allow to personifyfy medical and psychological assistance to military personnel, to improve early diagnosis of mental disorders, addictive and suicidal behaviour, and thus increase the effectiveness throughout psychiatric prophylaxis in the army.

Abstract

The article covers organizational aspects of development of innovative technologies in the field of regenerative medicine. It is shown that for the effective design and implementation into medical practice of regenerative medicine requires a united complex of military health care, military medical research and education. The main goal is to formate a biological insurance of personnel to treat different consequences of radiological incidents, burn disease, identification of the remains of the victims, the maximum returning to action after disturbed as a result of health services. Proposes the creation of «Interdepartmental Clinical Research and Education Center for Regenerative Medicine», combining research, clinical, industrial and educational potential of the leading institutions of various departments that will enhance the national security of the Russian Federation.

An improvement of experimental model for a study of local hemostatic agents’ effectiveness

Abstract

With the help of tests on 10 big biological objects (sheep) was studied a modern model of severe lateral damage (6 mm diameter) of femoral artery. Within 45 seconds blood loss was 432,5±258,2 ml. The usage of domestic local hemostatic agent - «Gemostop» based on zeolite and «Gemofleks» based on chitosan. The total three-hour mortality was 50%. Primary hemostasis was recorded only in 20% of biological objects, the final hemostasis in 60%. Thus, the model of standard 6-mm lateral damage of femoral artery with a free primary hemmorage during 45 seconds should be considered as optimal for evaluating the effectiveness of modern hemostatic agents.

An importance of early diagnosis in the course of knee joint instability treatment in military personnel

Abstract

The aim of the study was to determine the cause of the high frequency of meniscal tear coexisting rupture of the anterior cruciate ligament of the knee joint and to establish an optimal timing of surgical stabilization of the knee joint. Medical records of 476 patients who had surgery at the clinic of the Kirov Military Traumatology and Orthopedics of Military Medical Academy were studied. In 82 patients operation on the reconstruction of the anterior cruciate ligament was performed in the early period after injury (up to 6 weeks), 347 patients were operated within a period exceeding 6 weeks. It was revealed that the soldiers who had the operation in the period after 6 weeks from the date of injury, secondary damage of meniscus and articular cartilage occured twice as much as in surgery performed during the acute phase. Thus, the reconstruction of the anterior cruciate ligament should be carried out in a specialized department in the early stages after injury, when not yet any secondary damage to the meniscus and articular cartilage.

Clinical guidelines for management of patients with acute chronic obstructive pulmonary disease

Abstract

The treatment of acute chronic obstructive pulmonary disease is aimed at minimizing the manifestations of this aggravation and prevention of its progression. This article covers the information about the etiology of acute illness and its diagnosis criteria, recommendations about methods of assessing of exacerbations, place of treatment of this pathology and definition of treatment policy. The data about preferrable means of pharmacotherapy - bronchial spasmolytics, corticosteroids and antibacterial drugs, is given. Recommendations concerning oxygen therapy, respiratory support, mucoactive therapy and prevention of thromboembolic complications are given. An algorithm for the management of patients on an outpatient basis in a hospital is presented.

Myocardial infarction in young population. Description of clinical observation and literature review.

Abstract

Myocardial infarction in patients younger than 45 years is rare, but it is an important clinical, organizational and psychological problem. A case of myocardial infarction in 19-years old patient, who suffered since 6 years from kidney disease, is described. Transmural left-ventricular myocardial infarction has developed on the background of chronic glomerulonephritis, excessive exercise, and traditional risk factors for cardiovascular disease. Coronary venous bypass with the benefit-pleasing outcome is performed. When analysing the literature, the authors emphasize that in comparison with elderly patients, young people have different profiles of risk factors, clinical manifestations and prognosis of myocardial infarction. It is emphasized that kidney chronic disease, regardless the stage, worsen short-term and long-term outcomes of cardiovascular disease. Early stabilization is possible under the condition of risk stratification and early revascularization, which leads to better clinical outcomes. Particular attention should be given to a comprehensive assessment, it prognostic criteria, risk factor modification, secondary prevention of major and associated diseases, clinical-and-dynamic observation, including patients with asymptomatic course of the disease.

Presented the main guidelines concerning the application of methods and forms of physical therapy in the complex sanatorium treatment and rehabilitation in sanatoria and health resorts of the Ministry of Defence of the Russian Federation. It is concluded that the basis for the application and further development of forms and methods of physical therapy should be based on the methodological principle of a differentiated approach to the assessment of the severity of dysfunction cardiorespiratory and nervous system, musculoskeletal system, the mode of motor activity and exercise tolerance.

The first experience of hybrid correction of injuries of main artery of the head in military hospital

Abstract

Tandem internal carotid artery injury in combination with common carotid artery and the brachiocephalic trunk is quite rare. Development of adequate tactics of surgical correction of each of the concrete patient is always complex and strictly individual. In recent years, surgical correction of pathology of the aortic arch and its branches means the dominance of hybrid approaches. The paper presents the first successful experience of hybrid surgical treatment in a patient with immediate injury of right internal carotid artery and the brachiocephalic trunk. Analyzed evidence advantages and necessary conditions for performing of hybrid surgical interventions for aortic arch branch.

Abstract

The authors have shown that complex X-ray and ultrasonic examination reveals the greatest number of symptoms of neck damage in children with perinatal injury of the cervical spine. It was found that the structure of long-term complications of perinatal spinal injury prevails unfixed scoliosis I degree. The young men 13-17 years the most common complication of birth trauma of the neck is the instability of the cervical segments.

Prevention and treatment of cardiovascular diseases using CEM-technology in an outpatient setting and in the practice of ship’s doctor

Abstract

The article presents data on the prevention and treatment of hypertensive heart disease using CEM-technology. Devices of this series generate electromagnetic waves of extremely high simplicity in the millimetre range. In outpatient a positive effect of lowering blood pressure, and reduce the number and dose of drugs was observed in 68% of cases. These devices can be used in a ship due to the very limited possibilities for the use of other natural therapies.

Abstract

The authors describe clinical observation of penile implant surgery in a patient with previously implanted three-piece inflatable penile implant after radical radical prostatectomy for erectile dysfunction. The service life of inflatable penile implant of IY generation (Alfa-1, firm Mentor USA) until its technical malfunction was 16 years. It is concluded that in case of technical malfunction of the inflatable three-piece penile implants complete replacement is required.

Organization of medical support for troops, defending Leningrad and the people of the blockaded city.

Abstract

The data on the composition of forces of medical services and organization of medical-evacuation support for troops defending the blockaded Leningrad are presented. The information about the health losses among the population of Leningrad as a result of bombing, shelling and disease is given. Extremely high rates of morbidity and mortality in residents were associated with hunger, hypothermia and emotional stress. The clinical picture of some diseases has different peculiarities because of alimentary dystrophy background. The city health service suffered huge losses: 482 medical institutions were destroyed, only about 300 people from 1,5 thousand of medical personnel in 1942 saved working capability. The health care service of the local air defense played an essential role in delivery of medical aid. The contribution of civil and military health workers in saving residents lives in the blockaded Leningrad was appreciated.

Scientific and research experimentation center of aviation and space medicine and human engineering celebrates 80th anniversary.

Abstract

The article is devoted to the history of the Research Test Center Aviation and Space Medicine and military ergonomics, now included in the Central Research Institute of the Air Force Defense Ministry. The center throughout 80 years history is a leading research organization in the country for the integrated study of the human factor in aviation and problems connected with it. The world-famous scientific schools in aviation physiology, hygiene and radiolorgy, emergency medicine, aviation psychology and ergonomics have been grounded on the basis of this center. With a high qualified scientific staff and laboratory-and-bench-scale base including unique seminatural airplanes and helicopters complexes, posters and installation simulating the impact of flight factors (centrifuge, hyperbaric chambers, shaker, vestibulyar-WIDE stands, etc.) the center has successfully solved tasks concerning an improvement of flight crews protection from occupational hazards, ergonomic demands to capabilities of aircraft, professional and psycho-physiological training. Automatic systems of medical decision-making on assessment of the health status in the medical-flight expertise and dynamic medical supervision, planning, treatment and preventive and remedial actions aircrew training are currently being developed.
