Prophylactic medical examination of military personnel with ENT-diseases

Abstract

One of the most important goals in military otolaryngology is a prevention ENT-diseases of armed forces personnel. Prophylactic medical examination of their staff is the major instrument in it. The article presents detailed project of its realization in accordance with present-day classification of ENT-diseases (International Classification of Diseases by World Health Organization), new structure of medical service and its resources and conditions. The diseases of the nose, ear and throat were considered. Algorithm of patients’ observation and volume of diagnostic examinations were given.

Results of the experiment of skin disease reduction in military personnel of Airborne troops

Abstract

The article reflects the results of the experiment of skin disease reduction in the Airborne troops. Some aspects of skin disease in the individual armed services are revealed, the reasons of its increase in comparison with the analogous data in the Armed Forces of the Russian Federation in whole, concrete measures to reduce the number of diseases are offered. The most effective interventions are clothing exchange and bath operations 2 times a week, as well as the use of personal hygiene kits.

«Low infusion resusction» in the treatment of wounded and injured with acute massive blood loss

Abstract

Acute massive blood loss (AMBL) of severe and extremely severe degree is still one of the leading causes of unfavourable traumatic disease. 95% of potentially preventable lethal outcomes in severe gunshot trauma is reported to depend largely on the adequacy of AMBL correction (Howard P., 2003). An alternate approach to the issue studied was the development of preparations of hyperosmotic saline solutions (7,5% sodium chloride) combined with hyperoncotic colloid solutions (dextrans, hetastarch). As a result, solutions were developed (so-called, hyperosmotic hyperoncotic volume expanders) allowing to achieve rapid and stable volemic and hemodynamic effect in case of low volume infusion (usually, 4 ml/kg of body weight). The present study allowed to conclude that «low infusion resuscitation» technique in patients with multiple trauma accompanied by acute massive blood loss of extremely severe degree enables to reduce lethality, to achieve early subcompesatory hemodynamic state in acute traumatic disease.

Complex treatment of patients with lymphphleboid failure of lower limbs

Abstract

The article is concerned with the study of use of physical factors in the treatment of chronic lymphphleboid failure of lower limbs. As a result, it was able to show a significant positive dynamic under the complex approach of treating this disease. In patients with chronic lymphphleboid failure of lower limbs justified individual tactics, based on the use of a wide range of modern medicines, physical therapy techniques, therapeutic exercises, lymphatic massage of the lower limbs. This leads to early rehabilitation and improvement of quality of patients’ life.

Resistant hypercholesteremia in clinical practice: new insight into the causes and methods of treatment

Abstract

Leading role in the development of atherosclerosis belongs to violations of cholesterol metabolism. Reducing mortality from ischemic heart disease primarily associated with adequate correction of the lipid metabolism. The research of the possible causes and correction of resistant hypercholesterolemia is done. Determined that some patients with resistant to statins hypercholesteremia and normal or elevated levels of hormones T 3 and T 4 in the blood have isolated symptoms of hypothyroidism, which may be a manifestation of reducing tissue sensitivity to thyroid hormones. Adding to the statins levothyroxine in daily doses of 25-50 mkg have additional cholesterol reducing action, allowing to achieve target levels of lipoprotein in blood in patients. The use of levothyroxine in low doses in combination with statins is highly effective and economically justified.

Epidemiological and clinical significance of detection HbsAg and anti-HCV in patients and donors of blood

Abstract

During the assessment of 118 299 patients from hospital n. a. A.A.Vishnevskiy (2001-2005) anti-HCV was found out in 3,6%, HbsAg in 1,94%. By comparison with data of 1992-1994 (hospital n. a. N.N.Burdenko) the indices of disclosure of anti-HCV in in-patients fell 2 times, HbsAg 6,4 times. During this period, diverse changes of frequency of detection of markers in blood-donors took place: HbsAg - reduction, anti-HCV-elevation. During 2001-2005 reduction of frequency of detection of anti-HCV was noted in in-patients and blood donors, but reduction of frequency of detection of HbsAg only in blood donors. The frequency of detection of hepatitis C, B markers in blood donors during the last 10-15 years is correlated with the incidence of virus cirrhosis. Dynamic changes or stability of frequency of detection of hepatitis C, B markers in blood donors and in-patients during current period is correlated with the score of virus cirrhosis mortality.

Epidemiological aspects of the influenza «pandemia» during 2009-2010 years

Abstract

The article presents the preliminary data analysis of past «pandemic» and regional epidemics in the world caused by the influenza virus A(H1N1)sw1. Draw conclusions about the relative softness of the clinical and epidemiological manifestations of recent developments in connection with the lack of antigenic shift from a new strain and a significant protective role of acquired earlier population of cross-immunity of population to variants of influenza virus A(H1N1)sw1. In military units as representative risky groups are reflected all the laws of the epidemic process of influenza in the population. Consequently, the results of epidemiological surveillance in them can be extrapolated to the population and make a forecast of the epidemic in order to develop strategies and tactics to combat this infection in a large scale.

Types of damages among victims and dead persons in sea accidents

Abstract

The traumas received by victims in the largest sea catastrophes and accidents, connected with case (hull) damage, overturning and flooding of courts from 1983 to 2010 are considered. It is established that principal views of damages at rescued and victims are sharp hypothermia of light and moderate severity, combined mechanic-cold and mechanic-burn-chemical traumas, drowning. The analysis of the received combined traumas reveals prevalence of heavy damages among the rescued victims in the sea catastrophes and accidents. The size and structure of sanitary losses depends on weight of accident, remoteness from coast, weather conditions, speed of flooding of a vessel, readiness of forces of rescue and the beginning of carrying out of a rescue operation, training of crew and passengers.

The oldest military hospitals of Saint-Petersburg: round date in history

Abstract

In 17І0 in a under construction capital of the Russian state the first central military hospital for treatment the soldier of garrison has been organized. Its initial device differed from existed regimental infirmaries a little; however personal home nursing of tsar has helped development of hospital. In 1715 the hospital has been solemnly consecrated. Since 1735 in hospital have started to spend training of future doctors and with the basis in 1798 of Army medical college it became its clinical base. During teamwork the hospital became high-grade clinical base of academy, has developed and became the modern well equipped clinic-diagnostic centre.

Semicentenary anniversary of the faculty of medical officers training for Air Forces

In October the 8 in Military Medical Academy n.a. S.M.Kirov was a conference dedicated to the 50th anniversary of the Faculty of doctors for the Air Force. The issues of history of training and improvement of air doctors were considered, were mentioned the names of the principals of the faculty: B. V. Tikchomirov (1960-1964), BNKudryavtsev (1964-1972), I.I.Zhirkov (1972-1975), S.A.Bugrov (1975-1982), V.F.Zhernavkov (1982-1992), V.A.Afonin (1992-1998), I.N.Lizogub (1998-2003), D.K.Podovinnikov (2003-2008), Yu. M.Bobrov (2008-2010). Quoted the data that more than 200 graduates of the faculty became notable figures in military medicine. Progress of the students in study, sport and scientific research was noted.

The concrete results of operations and information about the staff are established.

Obviously, the initiative emanated from the publisher.
