Experience in the work of medical teams to assist the Republic of Serbia in the fight against the new coronavirus infection

Abstract

Based on the President of the Russian Federation’s decision, a consolidated detachment of the Armed Forces was formed to assist Serbia in the fight against the new coronavirus infection (COVID-19). The detachment included eight medical and nursing groups under the leadership of epidemiologists, each of which included an anesthesiologist (resuscitator), an infectious disease doctor (therapist), a nurse - anesthetist and worked in two directions: sanitary and epidemiological reconnaissance and sanitary - anti - epidemic (preventive) measures; providing medical care to patients with a new coronavirus infection in treatment and prevention organizations. During the work, specialized medical care was provided to 800 patients; disinfection measures were carried out at 100 facilities in 19 cities, 215 buildings with a total area of 1042 thousand m2 were treated, and 54 paved road sections a total area of 395.5 thousand m2. Besides, 28 objects with a total area of more than 200 thousand m2 were disinfected in 10 settlements of the Republika Srpska (Bosnia and Herzegovina), and about 60 thousand m2 paved road sections. The measures taken made it possible to localize the foci of the disease of new coronavirus infection in the territory of Serbia and the Republika Srpska and to reduce the mortality rate among the victims.

Substantiation of modern approaches to drug provision of children during sanatorium - resort treatment in military health resorts of Russia

Abstract

At present, the Russian Ministry of Defense pays special attention to the treatment of children in the conditions of sanatorium-resort organizations. This is largely due to the physiological and behavioral characteristics of children, as well as the fact that the health of the child directly affects the social status of the families of military personnel and the motivation for further military service. In this regard, children arriving at a military health resort should be able to receive high - quality and full - fledged medical care using all the resources necessary for this. The article substantiates promising directions for improving the drug supply of children in military sanatorium - resort organizations, identifies existing problems, and identifies ways to solve them.

To improve the classification of eye burns

Abstract

Currently, the system of the Ministry of Health of Russia has adopted the classification of eye burns, proposed in 1973 by N.A.Puchkovskaya and V.M.Nepomashy. The appearance in recent years of the concept of limbal stem cells of the corneal epithelium and its application in clinical practice has significantly expanded the possibilities of diagnostics, treatment, and visual rehabilitation of patients with eye burns. In 1997, M.D.Wagoner proposed a classification of eye burns, in which the gradation of burn severity is determined by the degree of damage to limbal stem cells. Taking into account this modern approach, a new classification of eye burns is proposed.

Criteria for the diagnosis of metabolically neutral obesity in male military personnel

Abstract

In the hospital therapy clinic of the S.M.Kirov Military Medical Academy conducted a single - center, one - stage observational sample survey of 289 men aged 38.5±5.6 years with abdominal obesity. Groups of patients with metabolic syndrome were formed (134), with criteria for metabolically healthy obesity (155). Metabolically neutral obesity was diagnosed in 86 patients with metabolically healthy obesity and plasma leptin levels <3.5 ng/ml. It was found that the maximum frequency of detection of hyperlipidemia, prediabetes, left ventricular hypertrophy and atherosclerotic changes in the carotid arteries were determined in metabolic syndrome, the minimum frequency - in metabolically neutral obesity. Military men suffering from uncomplicated obesity are shown to determine the concentration of free leptin in blood plasma. Exceeding the threshold for leptin levels >3.5 ng/ml in abdominal obesity may require more aggressive lifestyle adjustments, and possibly an early start of drug therapy.

The effectiveness of medical and psychological rehabilitation in a sanatorium when combined with family rest

Abstract

One of the directions in the development of the system for restoring service members’ health with occupational stress phenomena is the organization of medical and psychological rehabilitation. A comparative analysis of the effectiveness of medical and psychological rehabilitation of 150 service members who arrived at the sanatorium together with family members and 150 servicemen who entered without families was carried out. During their stay in the sanatorium, both groups’ servicemen received homogeneous procedures: audiovisual polysensory relaxation, aeroionotherapy, heat therapy, transcranial low - frequency magnetotherapy, somatosensory stimulation, aerobic strength physical exercises, and therapeutic swimming, as well as rational psychotherapy. To assess the results, we used functional and psychological examination data before and after medical and psychological rehabilitation. The best results were obtained when service members stayed in a sanatorium together with family members.

Problems of vaccine prevention of meningococcal infection in the Armed Forces

Abstract

The idea was formed that due to the favorable epidemiological situation and the possibility of preventing meningococcal infection with the help of vaccines, it is possible to achieve control of this infection. However, meningococcus showed all the classic properties of variability and adaptability. Changes in the structure of circulating meningococcal serogroups led to the formation of a non - immune stratum of the population in conditions of increasing pathogenicity of the dominant serogroups. The incidence of meningococcal infection over the past ten years in the Armed Forces of the Russian Federation can be assessed as low, but the mortality rate is high, which is directly related to the increase in the total number of asymptomatic carriers of this infection, especially in the conditions of the formation of organized military groups. The key principles of organizing vaccination against meningococcal infection are presented.

Development of the system of hygienic training and education of personnel in the Armed Forces

Abstract

The purpose of the work was to substantiate proposals for improving hygienic training and education of military personnel, which are the most important task of military hygiene. The system of these events has gone from an educational discipline for cadets and officers of the Russian army in the 1870s to the legislatively enshrined compulsory training in hygiene knowledge and a healthy lifestyle at all levels of military service and military education at the present time. Particular attention is paid to the methodological support of the learning process. The main unresolved issue is the lack of training in the basics of health protection for army and navy personnel, cadets, and students. It is proposed to introduce the military hygiene discipline into the plans of educational institutions of the Ministry of Defense. For the conscript contingent, a «Memo on the protection of the health of a serviceman» has been developed for training during the course of a young soldier (sailor).

Surgical treatment of closed testicular injury in military personnel

Abstract

The frequency of closed trauma of the scrotum and testicle, domestic and foreign classifications of closed injuries of the testicle are presented. On the example of 2 cases of closed trauma of the scrotum in military personnel, the high diagnostic value of ultrasound and Doppler studies of the damaged testicle and the vessels feeding it is shown. The choice of surgical tactics was carried out in accordance with the severity of testicular damage. Conservative therapy for traumatic injury to the testicle and scrotum is indicated for contusion and the absence of hematomas. Given the difficulties in recognizing closed testicular injuries, surgical treatment is recommended for those patients in whom testicular rupture and hematocele are determined or suspected after a scrotal injury.

The main aspects of treatment tactics in a general surgical hospital for patients with the first episode of spontaneous pneumothorax are considered. In the presence of a long - lasting unrecoverable spontaneous pneumothorax and the impossibility of transferring the patient to a specialized thoracic department, an urgent operation in the amount of diagnostic video thoracoscopy, elimination of the cause of pneumothorax, and anti - relapse treatment are indicated. The possibilities of providing elements of specialized medical care in a garrison military hospital have been demonstrated.

A clinical case of a severe course of hemorrhagic fever with renal syndrome and a pronounced leukemoid reaction

Abstract

A contract soldier, 19 years old, was admitted to the intensive care unit on the 8th day of illness with a pronounced leukemoid reaction up to 92.8ґ109 / L (a shift in the leukocyte formula to myelocytes 18%, blast cells 0.3%), hemorrhagic syndrome due to consumption coagulopathy, acute renal injury, stage III according to AKIN. Received antibacterial therapy with moxifloxacin 400 mg per day intravenously, symptomatic treatment, 2 sessions of plasmapheresis with plasma exchange were performed, 3 sessions of hemodiafiltration, replacement of donor fresh frozen plasma was 5710 ml. Antiviral therapy was not performed due to late hospitalization and decreased glomerular filtration rate. On the 15th day, in a satisfactory condition, he was transferred to the infectious diseases department. On the 74th day, he was discharged in satisfactory condition.

Low awareness of primary care physicians about this nosological form in the present time causes specific difficulties in the correct diagnosis of the disease. A competently collected anamnesis and knowledge of the disease’s clinical picture makes it possible to diagnose and carry out complex drug therapy timely.

Experience in using non - invasive hardware correction of blood pressure in the complex treatment of arterial hypertension in young people

Abstract

The experience of studying the efficacy and safety of blood pressure correction using non - invasive percutaneous electrical stimulation in young people is presented. The study involved 14 patients aged 18 to 45 years. In addition to standard antihypertensive therapy, all patients underwent blood pressure correction using the ABP-051 transcutaneous electrostimulator. The decrease in blood pressure reached average values and did not go beyond its boundaries during 14 days of observation in the hospital. Further studies are required to determine the prolonged effect of using the device to correct blood pressure.

Experience in using a medicinal composition based on selenium for local treatment of wounds

For civil aviation dispatchers, occupational stress is one of the leading risk factors for the formation of arterial hypertension «in the workplace». The article presents the combined color and light therapy and psychocorrectional music therapy results for 120 dispatchers of the complex for the prevention of arterial hypertension. According to individual methods, the simultaneous impact on the visual and auditory analyzers with pulses of light and sound, used in original rhythmic modes, gives a pronounced vasocorrective effect - a decrease in systolic and diastolic blood pressure and heart rate to the average level.
