OCCUPATIONAL ASTHMA FROM THE PERSPECTIVES OF SCIENTOMETRIC INDICATORS

Abstract

A scientometric analysis of the global array of scientific publications on occupational asthma (OA) has shown that a total of 164 915 publications on asthma, including 6661 research articles on OA, were indexed by the global abstract and citation database Scopus (http://www.scopus.com) over 20 years (1999-2019). A search in the Russian Science Citation Index revealed 63 publications on OA from 2005 to 2019, to which 126 references were made. However, the average number of citations per article is 2.0. However, a detailed analysis shows that only 23 articles are cited, while 40 are not cited at all.

CHRONIC SPONTANEOUS URTICARIA: MARKERS FOR DIAGNOSIS AND PROGNOSIS

Abstract

Chronic spontaneous urticaria (CSU) - is a disease associated with degranulation of mast cells and appearing of weals and/or angioedema during more than 6 weeks without specific trigger. Medical and social significance of CSU is connected with its wide distribution mostly among able-bodied population, significant life quality impairment and high treatment costs. Immune and pathogenetical mechanisms of CSU development can vary significantly among patients. Actual task of the modern medicine is a search for new clinical and laboratory markers of CSU which can help to determine better treatment approach, changes in disease activity and to predict treatment response.

THE EFFECT OF THYMALIN ON THE IMMUNE SYSTEM, HEMOSTASIS AND CYTOKINES LEVEL IN PATIENTS WITH VARIOUS DISEASES. PROSPECTS FOR APPLICATION IN CASE OF COVID-1

Abstract

The review evidences the data on the status of COVID-19 patients with lymphocytopenia, violation of the relationship between subpopulations of T-lymphocytes, NK-cells, T- and B-lymphocytes. The appearance of a «cytokine storm», accompanied by an increase in the concentration of pro-inflammatory cytokines, was detected in these patients. At the same time, there occurs hypercoagulation and increased concentration of fibrinogen and D-dimer. COVID-19 patients manifested the development of thrombotic microangiopathy, thrombosis, disseminated intravascular coagulation and multiple organ failure, which often resulted in death. Administration of the peptide immunocorrector Thymalin in case of acute and chronic diseases and injuries led to normalization of cellular and humoral immunity, decrease of the pro-inflammatory cytokines amount, elimination of the «cytokine storm» or inhibition of its development, and reduction of the intravascular coagulation intensity.Combined application of Thymalin and Heparin in case of various diseases with dysimmunity and hypercoagulation resulted in positive therapy outcome. The analysis of literature and the data, obtained from the study on the Thymalin clinical effect, indicate the possibility of complex therapy with Thymalin and Heparin in patients with COVID-19, thus contributing to the decrease of pathological process severity and morbidity reduction.

Abstract

ABC transporters and mainly P-glycoprotein play critical physiological and protective roles in during reproductive processes. The P-glycoprotein, encoded by the ABCB1 gene, is suggested to protect the developing embryo from drugs which are substrates of P-glycoprotein and other xenobiotic exposures. It is increasingly recognized that the activity of these transporters is affected by genetic polymorphisms. The article looks into the recent studies exploring the association between P-glycoprotein polymorphisms and the risk of fetal birth defects associated with medication use during pregnancy.

HEMATOLOGICAL INDICATORS AS PREDICTORS OF THE OUTCOME OF A NEW CORONAVIRUS INFECTION COVID-19 PATIENTS OF DIFFERENT AGE GROUPS

Abstract

In March 2020, the World Health Organization declared a pandemic of a new coronavirus infection caused by SARS-CoV-2 virus. The disease is characterized by high contagiosity and a certain mortality rate. Death is generally associated with risk factors such as the patients age, constitutional features, the presence of concomitant pathology, etc. The purpose of this study was to identify the distinctive features of hematological indices in patients with COVID-19 divided into two groups depending on the outcome of pathology (recovery/death). Statistically significant differences were found in individual erythrocytes, leukocytes and platelet indices. The detected features do not contradict the known facts and supplement the information about the pathogenesis of the new coronavirus infection.

HEMORRHAGIC ENDOBRONCHIAL SYNDROME IN PULMONOLOGY PRACTICE: BRONCHOSCOPY FINDINGS

Abstract

Hemorrhagic endobronchial symptoms were studied using the materials of 16,582 (100.0%) primary bronchoscopic biopsies from patients with nonspecific nontumor lung diseases. Hemorrhagic changes in the tracheobronchial tree mucosa, which were designated as hemorrhagic endobronchial syndrome, were detected in 793 (4.8%) patients. The studies revealed the following variants of hemorrhagic endobronchial syndrome: multiple petechial hemorrhages, hemorrhages from limited mucosal ruptures, and a hemorrhagic component of bronchial secretion. A combination of the hemorrhagic component of bronchial secretion and one or two other signs was found in a number of cases. A flow chart was proposed to assess the hemorrhagic component of bronchial secretion, by identifying its three grades. In most cases, the hemorrhagic endobronchial syndrome was clinically equivalent to hemoptysis or moderate pulmonary hemorrhage. No association was found between one or other variant of hemorrhagic endobronchial syndrome and specific nosological entities.

The paper analyzes the spread of latent tuberculosis infection among young people. It has been established that the use of the recombinant tuberculosis allergen test and spiral computed tomography can detect both latent tuberculosis infection and post-tuberculosis changes. Preventive treatment with antituberculosis drugs stops active pulmonary tuberculosis in persons with latent TB infection. Informing university students about getting tuberculosis and its prevention contributes to the formation of commitments to prevention studies and prophylactic therapy.

A follow-up was conducted in 55 pregnant women with type 1 diabetes mellitus (DM1), 8 of whom had a pregnancy complicated by preeclampsia. The course of the disease was com-pensated in all these patients; a control group consisted of 10 women. The data of comprehen-sive clinical and morphological (using different scanning microscopy options) studies and their comparison confirm the need to correct the placental status in the presence of maternal DM1 in all cases.

The data obtained from the SF-36 questionnaires suggest that weight loss in men and women positively affects the individual components of both their mental and physical health. Reduxin Met as a component of combination therapy for T2DM can be recommended for weight loss.

POSSIBILITIES FOR CORRECTION OF CONDITIONING AND MOTOR COORDINATION PROBLEMS IN OSTEOSARCOPENIA, BY USING DIETARY SUPPLEMENTS WITH CALCIUM AND VITAMINS D3 AND Bb

Abstract

Objective. To evaluate the effectiveness of a new comprehensive osteosarcopenia treatment program using the biologically active additive Osteomed Forte (OF) containing calcium and vitamins D3 and B6 from its effect on skeletal muscle strength and volume, coordination abilities, motor activity, and social activity levels in older age group patients with osteosarcopenia. Subjects and methods. The investigation enrolled 78 men and postmenopausal women aged 50 to 80 years who had a high risk of fractures, as well as decreased muscle strength, as evidenced by tension dynamometry. Before beginning the rehabilitation cycle, the patients were randomized into 2 groups: 1) 39 patients were prescribed OF for 12 months (a study group); 2) 39 patients who did not receive additional therapy for osteoporosis (a control group). The patients of both groups underwent an examination including bone densitometry, tension dynamometry on a Back-Check device (Dr. Wolff, Germany), functional tests evaluating conditioning and coordination motor abilities, as well as quality of life (QOL) assessment using the QUALEFFO-41 questionnaire. Results. A prospective follow-up study showed that at 12 months the study group had higher back extensor and flexor strength levels (p<0.05). The control group patients exhibited negative changes in the strength of the lateral trunk flexors (p<0.05); following one year, the strength of the flexors and right lateral flexors was significantly lower than that in the study group patients (p < 0.05). Within 12 months, in both groups the functional tests revealed no changes in their mean values, except for worse results of the one-leg standing with the eyes open test in the control group (p < 0.05). Analyzing the changes in QOL indicators after 12 months, the scores for «Pain», «Daily activity», «Mobility», «General health», «Mental status» and «General indicator» were significantly lower (better QOL) in the study group than in the control group (p<0.05). Conclusion. The investigation results showed that the complex dietary supplement OF containing calcium citrate, vitamin D3, and HDBA organic complex with vitamin Be prevents decreases in muscle strength, balance function, and QOL in patients who have osteosarcopenia and a high risk of fractures.

Abstract

Objective: to determine the level of therapists’ basic knowledge about the management of patients with chronic obstructive pulmonary disease (COPD). Material and methods. This study analyzed an anonymous multicenter survey within the ASCO project for the management of COPD patients. Results. The study revealed an insufficiently high level of therapists’ knowledge about the management of patients with COPD. When answering the questions, the respondents showed the smallest results in selecting laboratory and instrumental ones showing evidence of severe COPD symptoms; in indicating drugs for the basic therapy of COPD with obvious symptoms and a high risk of exacerbations; in selecting the clinical group (type) of COPD for the following criteria: 60% forced expiratory volume in one second, one exacerbation per year, mMRC-1, and CAT-9 and in choosing the appropriate statement regarding COPD treatment/control. Conclusion. An analysis of the study results revealed an insufficient level of specialists’ basic knowledge about COPD, which, in the authors’ opinion, necessitates additional educational measures in this area among the practitioners.

RESULTS OF MASTOIDOPLASTY ACCORDING TO HISTOLOGICAL EXAMINATION AND COMPUTED TOMOGRAPHY

Abstract

Objective: to evaluate the status of an osteograft after mastoidoplasty according to temporal bone histological examination and computed tomography (CT). Subjects and methods. Ninety-six patients diagnosed with chronic epitympanic antral suppurative otitis media were operated on. Coarse bone meal was used in 51 (53%) cases; finely dispersed meal was employed in 45 (47%) cases. CT of the temporal bones and histological examination of the graft were performed in the patients at 12 months after surgery. Results. Analysis of temporal bone CT data showed that the implanted bone graft was less dense than the cortical plate of the mastoid process. Nineteen implanted bone graft samples were histologically examined at 12 months after surgery. Mature bone beams of correct structure were detected in 9 (47.4%) samples. Young bone tissue elements as osteoid and uneven young osteogenic tissue growths were prevalent in 10 (52.6%) cases where finely dispersed bone meal had been used. Conclusion. Temporal bone CT allows evaluation of the status of the implanted bone material in the long-term postoperative period. The histological examination at one year after surgery indicated that when coarse bone meal versus finely dispersed one was used, the bone material underwent all stages of integration mainly through osseointegration.

USE OF TRANEXAMIC ACID IN PATIENTS WITH HEMOPHILIA A DURING KNEE ARTHROPLASTY

Abstract

Objective: to reduce intraoperative and early postoperative blood loss in patients with hemophilia A during endoprosthetics of the large joints. Subjects and methods. The investigation enrolled 29 patients aged 28 to 56 years with severe hemophilia A, who underwent knee arthroplasty. All the patients had end-stage hemophilic arthropathy with pronounced pain syndrome. The patients were divided into 2 groups. Hemostatic therapy only with factor VIII (FVIII) preparations was performed in Group 1 in the intraoperative and postoperative periods; FVIII in combination with tranexamic acid (TXA) was used in Group 2 intraoperatively and in the first 3 days. Hemostasis with FVIII was carried out according to the standard scheme, by calculating that the intraoperative level of the factor in the patients blood was at least 100%. TXA was given at a dose of 10-15 mg/kg at an interval of every 8 hours. Results and discussion. All surgical treatments yielded satisfactory or good results at follow-ups of at least 1-3 years. In the FVIII+TXA group, the intraoperative blood loss was an average of 350 ml (23 to 34%) less than that in the FVIII group. In the postoperative period, Group 1 also showed an average 25% reduction in blood loss into the drains. During early postoperative hospital stay (from 14 to 28 days after surgery), postoperative wound healing and rehabilitation were better in the FVIII+TXA group than those in the FVIII group. The patients had less marked soft tissue hematomas and swelling at the operated joint site, which allowed earlier and more effective movement developments to be initiated. No cases of thrombotic events were noted.

Ethics is a branch of philosophy that describes morality. Medical ethics in geriatrics are not that different from the general one. It operates with an ethical principles of biomedicine, which includes autonomy, beneficence, nonmaleficense, justice and fidelity. In aged population chronic pain syndromes and cognitive decline are developing much more frequent. That is leading to the serious desadaptation.
