MODERN APPROACH IN TREATMENT OF REFLEX COUGH
Abstract
Etiologic factors responsible for the development of reflex coughing are described. A highly effective and safe method is proposed for the management of this condition of psychogenic origin associated with hyperventilation by intradermal procain blockade Zakharyin-Head zones for the larynx in conjunction with preventive treatment of coughing attacks.

Osteoporosis and stroke are the most important risk factors for femoral neck fracture. Most fractures in post-stroke patients occur on the side of paresis and paralysis and more frequently due to a fall. To prevent such complications must be a management strategic priority for patients during poststroke rehabilitation.

THE CONCEPT OF A STEPWISE APPROACH TO TREATING NONSPECIFIC PAIN IN THE LOWER BACK
Abstract
The paper discusses the choice of an optimal mode to relieve pain syndrome on an exacerbation of chronic musculoskeletal pain in the back. It highlights the possibility of using a stepwise approach: therapy is started from the least effective doses of nonsteroidal anti-inflammatory drugs (NSAIDs) and, if required, switched to other NSAIDs, a combination of the latter with other non-drug and drug treatments, including those with myorelaxants, antidepressants, and GABAergic agents.

RATIONAL PHARMACOTHERAPY FOR UPPER RESPIRATORY TRACT INFECTIONS: THE ABILITIES OF IMMUNAL
Abstract
Acute respiratory viral infections (ARVI) belong to the most common illnesses in both adults and children. Not only ENT physicians, but also, first of all, therapists and pediatricians encounter them in the outpatient setting. The paper provides an evidence-based review of Immunal®. Russian and international experience with the drug based on Echinacea, including that in combination with ascorbic acid, suggests its high efficacy as an immunomodulator and its good safely profile when used in the combination therapy for ARVI.

DOPAMINERGIC THERAPY IN PATIENTS WITH CHRONIC CEREBROVASCULAR INSUFFICIENCY
Abstract
Chronic brain ischemia (CBI) is considered as individual cerebrovascular syndrome that manifests itself as a complex of neuropsychological and neurological symptoms. Trials have provided support for the efficacy and safety of Pronoran (piribedil), a nonergolinic dopamine receptor agonist, its effects on cognitive, psychoemotional, and motor disorders in patients with CBI. At 60 days of treatment, the drug exerted a significant leveling effect on static and gait disturbances. According to the Mini-Mental State Examination (MMSE) scale and Montreal Cognitive Assessment (MoCA) test, 68% of the patients had ameliorated cognitive and affective disorders as compared with the baseline. There was no significant improvement in Hospital Anxiety and Depression Scale scores, which was likely to be associated with the fact that a relatively small group of patients had been examined and they had only subclinical symptoms of anxiety and depression when they had been included into the study; however, there was regression of anxiety and depressive disorders and emotional normalization in 79% of the patients.

RATIONAL COMBINATION ANTIHYPERTENSIVE THERAPY WITH BISOPROLOL AND AMLODIPINE
Abstract
The current guidelines for the management of arterial hypertension (AH) underline that combination therapy is essential to achieving the proper effect in the majority of patients. Bisoprolol and amlodipine are widely used to treat AH. Bisoprolol is a /З-adrenoblocker that is more commonly used in young patients with vasospastic hypertension accompanied by higher renin levels while amlodipine, a calcium channel blocker, is given to elderly patients with volume-dependent hypertension and low renin activity. Thus, the physiological and pharmacological properties of these drugs determine the rationality of their combination; bisoprolol and amplodipine are characterized by a complementary mechanism of action and are able to minimize the side effects of each other. In addition, the combination therapy makes it possible to increase patient compliance and to ensure effective blood pressure monitoring.

Abstract
When used in the treatment of insomnia and mild to moderate nonpsychotic depression in patients with organic diseases of the nervous system (chronic brain ischemia and dorsopathy), Valdoxan promotes relief of the symptoms of depression at the early stage of the disease. In severe depressive disorders, it is expedient to use the drug longer to achieve stable remission.

CURRENT UNDERSTANDING OF PATHOPHYSIOLOGICAL ASPECTS OF GERD AND APPROACHES TO PHARMACOTHERAPY
Abstract
Heartburn is considered to be the main symptom of gastroesophageal reflux disease (GERD) which significantly affects the quality of patient life. The current studies showed that the most frequent cause of heartburn after eating was the accumulation of acid on the gastric contents surface - the formation of the socalled «acid pocket». Neutralizing the acid pocket is not an easy task even for antisecretory agents. And to prevent gastroesophageal reflux after eating is even a more complicated task. The modern and safe alginate - andantacid-based drug Gaviscon Double Action offers an effective solution to this problem by producing a double effect due to its antacid and antireflux properties.

THERAPY IN PATIENTS WITH ARTERIAL HYPERTENSION AND PAROXYSMAL ATRIAL FIBRILLATION
Abstract
The paper gives the results of a trial of the antiarrhythmic and antihypertensive efficacy of sotalol used for 12 weeks in 34 patients with arterial hypertension (AH) concurrent with atrial fibrillation (AF). If the treatment was ineffective, a combination of indapamide and perindopril was added. According to the findings, sotalol may be successfully used as a drug for antiarrhythmic therapy and effective blood pressure lowering in patients with grade I-II AH and paroxysmal AF.

Abstract
The paper considers a problem in substantiating the choice of combination drug therapy for primary arterial hypertension. It is noted that there are two types of patients with neuroreflectory and humoral regulation of blood flow. A scheme for choosing the drugs and changing their competence is proposed and substantiated in relation to patient age.

The paper considers the characteristics of headache in temporomandibular joint dysfunction. Additional neurophysiological studies (stimulation electromyography of the masculatory muscles, trigeminal evoked potentials, and wink reflex) are recommended to estimate the involvement of the masculatory muscles and trigeminal system, which allows adequate therapy to be chosen.

SPECIFIC FEATURES OF POSTINFARCTION LEFT VENTRICULAR DYSFUNCTION IN YOUNG PEOPLE
Abstract
Postinfarction left ventricle (LV) remodeling was evaluated in relation to the location of myocardial infarction (MI) in 100 males aged less than 50 years who had experienced first Q-wave MI. The patients were divided into 2 groups of 50 subjects each: A) those younger than 40 years; B) those aged 40 to 50 years. Both groups showed a significant increase in LV sizes in anterior MI after one-year follow-up and in posterior MI in subsequent years. Analysis of LV structural and geometric indicators (end systolic size, end diastolic size, ejection fraction) revealed that there were no significant differences in the indicators in relation to the location of MI in Group A and that in Group B the indicators were on the contrary statistically different in anterior and posterior MI in early and late postinfarction LV remodeling.

Abstract
The increased production of tumor necrosis factor-α and interleukin-6 in patients with chronic viral hepatitis B and C has been established to contribute to the worsening damage to the liver and the development of its fibrosis. There is evidence that the above proinflammatory cytokines may be used as an additional diagnostic criterion for the course and prognosis (the development of fibrosis) in the disease in question.
