The foundation for stroke prevention in current clinical practice
Abstract
Ischemic stroke (IS) is a complex multifactorial disease with a now widely discussed polyfactor hereditary component and biochemical, metabolic, inflammatory, coagulation, and hemodynamic factors. Its development is also influenced by environmental conditions. The assessment of the contribution of various risk factors to the prognosis of a first and recurrent IS can serve as the basis for developing personalized programs for primary and secondary IS prevention, especially in high-risk group patients. The issues of primary and secondary stroke prevention are common for neurologists, cardiologists, endocrinologists, therapists, and, of course, general practitioners.

Epidemiological survey strategy and risk assessment in occupational medicine
Abstract
The paper considers the problem with assessment of cause-and-effect relationships in occupational pathological studies and occupational medicine, by applying methodological approaches of clinical epidemiology (the term evidence-based medicine is better known in the Russian Federation). It also discusses the issues of evidence and causation in occupational medicine, the organization and conduction of epidemiological surveys using the specialized questionnaires, as well as clinical, functional, molecular, and genetic studies. The organizational and methodical bases for the assessment and management of professional risks in industrial medicine are under discussion.

Polypragmasia in ophthalmic practice
Abstract
The paper provides a discussion of the clinical case of female patient P. aged 70 years, who applied for examination to the Advisory Department, Eye Hospital, with the previously verified diagnosis of grade 1a open-angle glaucoma, early cataract, and mild hypermetropia. During instillations of the local formulation of a ß-adrenoblocker and an а-adrenomimetic in combination with paroxetine (an antidepressant of the selective serotonin reuptake inhibitor group), the patient developed weakness, hypotension, and bradycardia (blood pressure, 100/60 mm Hg; heart rate, 58 beats/min). In this case, there was a serious adverse reaction that was significantly associated with the adverse drug interaction according to the Horn-Hansten scale).

Post-traumatic diaphragmatic hernias: diagnosis and treatment
Abstract
Post-traumatic diaphragmatic hernias (PDH) account for about 13% of all diaphragmatic hernias. Due to difficulties in the diagnosis of PDH, it is necessary to develop protocols for examination of patients with isolated and mixed chest injury, by using the results of multispiral computed tomography, the most informative method for instrumental diagnosis in such patients. General practitioners, surgeons of specialized departments should be also informed further about the diagnosis and treatment of PDH in order to form their greater alertness to this category of patients.

Use of zinc sulfate for peptic ulcer
Abstract
The incorporation of zinc sulfate into the standard treatment regimen for peptic duodenal bulb ulcer can eliminate immunological shifts occurring in this pathology and results in an increase in the peripheral blood levels of leukocytes, lymphocytes, Tlymphocytes, and B lymphocytes and a decrease in the blood levels of IgM, circulating immune complexes, and the proinflammatory cytokine tumor necrosis factor-α.

Individual rehabilitation therapy techniques for osteoarthritis
Abstract
Osteoarthritis (OA) is one of the most common joint diseases. The problem of its treatment needs to form the new approaches meeting the high safety requirements, especially in elderly people with comorbidity. A patient group of 72 people aged 38 to 76 years with a reliable diagnosis of OA and long-standing severe pain syndrome was selected. Thirty-two patients received treatment with ionized gel with hyaluronic acid and transcranial electrostimulation sessions, resulting in a reduction in pain by 56% of the baseline value on the visual analog scale, which was 2.3 times higher than that in the control group (24.3%). Upon completion of a 10-session cycle, the dose of used nonsteroidal anti-inflammatory drugs (NSAIDs) was reduced by 2 times in 58% of cases in the study group; 10% of patients completely discontinued the drugs; 4 weeks later only 25% of patients resumed taking NSAIDs at an average therapeutic dose. The use of ionized hyalurone-containing gel and transcranial electrical stimulation in patients with chronic pain syndrome in OA can be a promising rehabilitation technique at the stage of restorative treatment.

Pathogenetic communication of erectile dysfunction, anxiety and depressive disorders in hypertension
Abstract
Erectile dysfunction (ED) in patients with arterial hypertension (AH) is very common, reaching according to various authors 70%, with accompanying psychoemotional disorders aggravate the course of sexual as well as somatic disorders. Associated pathology increases the risk of premature death, worsens the prognosis and reduced social functioning of patients. The common pathogenetic mechanisms of ED, anxiety and depressive disorders and hypertension are endothelial dysfunction with decreased nitric oxide, low levels of androgens, the violation of cortico-visceral connections with the imbalance of the hypothalamic-pituitary-adrenal system and sympathicotonia. Complex pathophysiology requires prescription of adequate anti-hypertensive and psychotherapy, effective treatment of erectile dysfunction.

Hormone replacement and cardiotropic therapy in young men with hypertension, overweight, and androgen deficiency
Abstract
Eighty-five men with hypertension, testosterone deficiency, and overweight were treated. The patients achieved a significantly effective result of 6-week treatment with minimal doses of a combined drug (10 mg lisinopril and 5 mg amlodipine) and minimal doses of hormone replacement therapy (transdermal gel 5 g and testosterone 50 mg daily). Early detection and timely treatment can prevent possible cardiovascular events. This therapy has a positive impact on blood pressure, vascular bed, erectile dysfunction, left cardiac remodeling, and psychological status.
