Pulmonary tuberculosis and malignant neoplasms

Abstract

The clinical, radiological, and laboratory manifestations of pulmonary tuberculosis were retrospectively analyzed in 38 deceased patients with malignant neoplasms (MN) (a TB/MN group). A comparison group included 134 deceased patients with pulmonary tuberculosis without MN. Progressive tuberculosis concurrent with MN was established to be more common in men over the age of 50 years. Pulmonary malignant neoplasms concurrent with tuberculosis more often developed in the areas of fibrosis with previously treated cirrhotic and fibro-cavernous forms. At the same time, tuberculosis was manifested by damage to <2 lobes of the lungs, multiple destruction of medium size (2-4 cm), moderate bacterial excretion with polyresistance and multidrug resistance of tuberculosis mycobacteria, obvious intoxication of mixed origin, and a systemic inflammatory response. The cause of death was cancer in patients with tuberculosis and MN in most cases.

Endometrial cancer: topicality of the issue and opportunities of rehabilitation

Abstract

Endometrial cancer is one of the most common gynecological cancers. The relevance of the topic is due to a rise in the number of reproductive-aged patients, which presents an acute sociomedical problem. The paper analyzes the possibilities of medical rehabilitation (diet therapy, physiotherapy, immunotherapy, etc.). It considers antitumor therapy complications that negatively affect female the life and health of women, such as sexual dysfunction, paraneoplastic fatigue syndrome, lower extremity lymphedema, radiation reactions, and postcastration syndrome, as well as methods for their correction. Methods for patient rehabilitation and early diagnosis of a relapse before its clinical manifestation are analyzed. The ultimate goal of rehabilitation is to improve quality of life in patients.

Somatic and infectious diseases as a cause of death in HIV infection

Abstract

HIV-infected patients seek medical advice in various health facilities for both infectious and somatic diseases. Moreover, treatment does not always provide success, which often depends to a greater extent on a patients functional reserves rather than the quality of medical care. The authors have analyzed all death cases in a group of HIV-infected patients to identify the prevalent disease of organs and systems. Common nosological entities that result in death in patients with HIV infection have been defined. These findings can be used to plan a load on health facilities and to develop adequate preventive measures in HIV-infected patients.

Diagnosis of tuberculosis in patients in end-stage chronic kidney disease

Abstract

In the category of persons with secondary immunodeficiency, the number of patients with end-stage chronic kidney disease is growing annually. Tuberculosis incidence and mortality among these patients are ten times higher than those in the general population. The diagnosis of tuberculosis in patients with chronic kidney disease is extremely difficult: the manifestation of the tuberculosis process does not have a characteristic clinical and radiological pattern; etiological verification rarely reaches 50%, and the possibility of immunodiagnosis of tuberculosis has not been insufficiently investigated.

Abstract

A human microbiota change is considered as one of the important factors in the pathogenesis of many socially significant diseases including the respiratory tract diseases. The aim of our research was to study the composition changes of the oropharyngeal microbiota in the patients with atopic and non-atopic phenotype of bronchial asthma. Materials and methods. There were examined 27 patients with bronchial asthma and 10 clinically healthy individuals in the study. The studies of microbiota in the oropharyngeal smear samples were carried out by the 16S rRNA gene sequencing. Results. In patients with the BA were revealed the quantitative and qualitative changes in the oropharyngeal biotope : such as the significant decrease in the families of Porphyromonadaceae, Flavobacteriaceae, class Clostridia, family Peptostreptococcaceae, genus Oribacterium (family Lachnospiraceae), class Fusobacteriia, genus Fusobacteriia, family Burkholderiaceae genus Haemophilus (family Pasteurellaceae) (p<0.05). The phenotype of the disease (atopic, non-atopic) AD did not effect on the change in the bacterial composition (p>0.05). The changes in the bacterial spectrum showed a moderate strength and the strong direct and reverse correlation with the main clinical and laboratory manifestations of BA (medical history, level of IgE, eosinophils of blood and sputum, FEV1). The bacterial diversity indexes (Shannon, Chao1 and ACE) in the patients with BA were not differ from the control group (p>0.05). Conclusion. The results of the study indicate the differences in the oropharyngeal microbiota compositions of the healthy volunteers and patients with bronchial asthma. The relationship of the change in the bacterial composition to the main clinical and laboratory manifestations of the disease confirms their significance in the pathogenesis of BA. The study ofthe bacterial composition changes of the respiratory tract under the respiratory system diseases should be continued at further.

Clinical and epidemiological features of corpus uteri cancer

Abstract

Introduction. Malignant neoplasms of the reproductive system dominate the structure of cancer incidence in women in the Russian Federation. In the structure of oncogynecological diseases, uterine body cancer (UBC) leads the way. The purpose of the study: to analyze the dynamics of UBC incidence in the Omsk region in comparison with data for the Russian Federation for the period 20022018 to determine the priorities of cancer prevention. Results: there was a moderate tendency to increase the incidence of UBC in the region (Rg. =+1.8%; p<0.001) and for the Russian Federation (Rg. =+1.7%; p<0.001). The maximum percentage was 60-69 years old (32.3%). There was a moderate tendency to increase UBC among women 30-39 years old, and in the group of women 35-39 years old there was a pronounced tendency to increase the incidence (Rg.=+5.6%; p<0.05). The article presents the epidemiological features of UBC in comparison with cervical cancer in the region: the increase in the incidence of UBC (Rg.=+1.8%) and the stability of the indicator at cervical cancer (Rg.=+0.05%), the tendency to rejuvenate both locations of cancer. Conclusion: in the Omsk region, there is a unidirectional increase in the incidence of UBC with the Russian Federation, a tendency to «rejuvenate» the disease, which dictates the need for cancer prevention in the practice of an obstetrician-gynecologist.

The experience of using of comprehensive rehabilitation program with dietary supplements Osteomed Forte in patients with a jsteoporotic distal forearm fracture

Abstract

The aim of this work was to assess the effectiveness of comprehensive rehabilitation of patients with a osteoporoticdistal forearmfracture with the inclusion of dietary supplement Osteomed Forte. Material and methods. The study group consisted of 17 men and women over the age of 50 years with a low-traumaticdistal forearm fracture within 1-60 days prior to inclusion in the study and with signs of successful fracture consolidation. Patients were assigned a rehabilitation complex lasting 20 days (3 weeks), aimed at restoring the functions and fine motor skills of the injured arm. During the rehabilitation period, as well as in the next 5 weeks of observation, patients received dietary supplement Osteomed Forte in a dosage of 4 tablets a day. Results. The use of complexrehabilitation program was associated with an increase in the quality of life and a decrease in the level of depression and anxiety in 2 months in patients with osteoporoticdistal forearm fracture. There was also an increase in serum calcium and phosphate level, as well as a decrease in the bone remodelingrate. Good tolerability of a comprehensive rehabilitation course was revealed.

HIV epidemic features in the Almetyevsk District of the Republic of Tatarstan

Abstract

Analyzing the trend in the HIV epidemic in the Almetyevsk District of the Republic of Tatarstan over 2000-2018 shows that the higher incidence rate of HIV infection stabilized, the rate of its sexual transmission and the involvement of middle-aged and older individuals in the epidemic increased. It is found that the course of the epidemic is affected by the incidence rate of tuberculosis (TB) in HIV-infected people. The rapid spread of HIV in the high TB risk group requires that phthisiatricians should pay special attention to the problems of HIV infection among the population followed up, as well as control over how anti-epidemic measures are implemented needs to be enhanced in TB facilities. The primary task is to effectively prevent HIV infection and to avoid tuberculosis cases in HIV-infected people.

Diagnosis of functional gallbladder disorder and sphincter of Oddi in clinical practice

Abstract

Recently, the study of functional gastrointestinal disorders has become an urgent task due to the prevalence of this pathology. Functional disorders of the biliary tract are known to underlie clinical symptoms both in primary functional disorders occurring as an independent pathology and in secondary dysfunctions resulting from organic pathology. It has become possible to adjust the accumulated knowledge on the clinical course of the functional gallbladder disorders, their diagnosis and treatment due to the Rome IV criteria. Nevertheless, many issues remain unsettled, including the function tests with choleretic breakfast. Such testing allows to evaluate the function of the gallbladder and sphincter apparatus of the biliary system. However, the reaction to the same choleretic breakfast can differ significantly intrapatient, which requires the development of a standard, generally accepted technique. In our study we showed the effectiveness of Chophytol at a dose of 5 ml used as a choleretic breakfast. The choice of Chophytol was due to its well- known and proved choleretic and cholekinetic effects, as well as the fixed concentration of choleretic elements. The present case demonstrates special features of the functional biliary disorders course, their diagnosis and treatment.

Prolonged persistence of viral and bacterial pathogens affects the atopy development and can lead to exacerbations of chronic respiratory tract diseases. This paper presents information about the origin, spread and clinical symptoms of coronavirus infection. Current approaches to managing patients with acute liver failure

The paper considers data on the etiopathogenesis and clinical manifestations of acute liver failure (ARF), as well as current approaches to managing patients with this condition. Despite success in intensive care, ARF is a clinical syndrome with a high mortality rate that amounts to as much as 50%. The clinical picture is most frequently represented by the development of coagulopathy, encephalopathy, and multiple organ dysfunction. The severity of the course, the likelihood of irreversibility of RF, and the etiological heterogeneity of the syndrome necessitate further elaboration of therapeutic strategies.

The paper describes the etiology and pathogenesis of periodontal diseases, caries, and dental hypersensitivity. It shows the role of oral hygiene in preventing dental diseases. Pomorin toothpaste that contains brine taken from the Pomorie Lake (Pomorie lye), is presented. Clinical trials have proven that Pomorin toothpaste is highly effective in treating gum disease and reduces the reproduction and activity of bacteria that contribute to the development of caries. Pomorin Maximum Protection toothpaste is designed to take deeper and better care of sensitive teeth. It actively affects the enamel, diminishing dental sensitivity, and is able to restore the enamel (to induce tooth remineralization). Pomorin Anti-Periodontal toothpaste is used for periodontosis and periodontitis; it prevents odontolith formation. The data presented may lead to the conclusion that the Pomorin series toothpastes are a quite effective and useful component of combination therapy for periodontopathies, caries, and dental hyperesthesia.
