Tamsulosin in combind treatment of patients with urolithiasis

Abstract

In administration of tamsulosine (focusin, Zentiva, Czech Republic) in combined treatment of urolithiasis concrement evacuation after extracorporeal lithotripsy occurred in 83% cases in patients with the concrement stay in the ureter for less than one month. The size of the concrement after successful lithotripsy had no significant effect on subsequent stone-eliminating therapy but long-term stay of the concrement in one place promoted inflammatory and urodynamic disturbances in the ureter treated surgically. The comparative analysis of the contractile function of the distal ureteral comparment in patients given tamsulosin showed a less active (amplitude of the contractions) and passive (tonicity) resistance to urine flow than in patients given a standard therapy. These changes were accompanied with lower pressure in the renal pelvis and suppression of enzymuria. Thus, tamsulosin addition to combined treatment of urolithiasis has a positive effect on ureteral urodynamics, concrement evacuation and functional structures of the kidney by indirectly lowering intrapelvic pressure.

Left-side venous anastomosis as a method correcting blood outflow from the kidney

Abstract

Case histories and surgical protocols of 50 patients who were treated for chronic hepatitis by creating left-side renoportal venous anastomosis (RPVA) were analysed retrospectively. Early after surgery 75% patients had microhematuria, proteinuria to 0.033-0.066 g/l, leucocyturia. At discharge from the hospital these abnormalities were not registered in the majority of the patients. Three months after operation these indices were at the preoperative level. Significant shifts in parameters of urine were associated with an anomalous condition of the left renal vein (annular, retroaortal), its compression, portal hypertension and creation of RPVA without legation of the splenic vein. In a cositive compression test RPVA was created without arrest of arterial inflow for 45 min. This can be a criterion of feasibility of left renal vein ligature if left-side RPVA cannot be performed for preservation of the left kidney. Validity of left-side RPVA use for correction of blood outflow from the left kidney is proven by functional improvement and normal side of the kidneys in long-term postoperative follow-up.

Tulozin in combined treatment of patients with acute urinary retention

Abstract

There is much evidence that tulozin promotes recovery of spontaneous urination, Qmax and is effective in combined treatment of patients with acute retention of urine caused by prostatic adenoma. Tulozin produces positive changes in the lower urinary tract symptoms. Rare occurrence of side effects enables long-term treatment and achievement of good therapeutic response. Tulozin is recommended for patients of younger age, with minimal comorbid pathology, hypotonic with orthostatic reactions, history of side effects in the treatment of other alpha-adrenoblockers, in comorbid hypertention, hypercholesterolemia, retrograde ejaculation, low potention, overactive bladder, prostatitis, after prostatic TUR, transvesical adenomectomy.

Treatment of chronic cystitis in postmenopausal women

Abstract

A randomized double blind controlled trial of intravaginal cream ovestin (estriol) was conducted in 102 postmenopausal females with chronic cystitis. The patients were divided into two groups. Group 1 (n = 52) received standard therapy, group 2 (n = 50) received standard therapy plus ovestin (daily intravaginal administration for 2 weeks, then twice a week). Final results were evaluated 1 year after therapy. Before the treatment 94% patients had subnormal levels of blood estradiol. One year after the treatment positive results were achieved in 91.3% patients of group 2 and 65.8% of group 1 (p < 0.001). In group 2 patients ovestin significantly reduced pollakiuria, nocturia, episodes of urgent urinary incontinence, painful voiding. Percentage of patients with leukocyturia diminished from 100 to 8%, bacteriuria - from 74 to 4%, with vaginal lactobacilli increased from 0 to 56%, enterobacterial contamination of the vagina fell from 66 to 12%. Vaginal pH decreased from 6.0 to 3.6. Addition of ovestin to standard therapy reduced the number of cystitis recurrences 11-fold, days of antibacterial therapy - 12.4-fold. Side effects of ovestin (vaginal pruritus) were seen in 4% patients.

Abstract

A protective effect of lithium chloride (LC) in thermal ischemia of the kidney of different duration was studied in rats. LC efficacy was estimated by functional activity of ischemic kidney in early and late reperfusion period, by damage to mitochondria of tubular epithelial cells and production of active oxygen forms (AOF) and nitric oxide (NO). LC has a marked anti-ischemic effect. In thermal renal ischemia for 40 and 60 min LC provides functional safety of the ischemic organ. In longer ischemia, 50% of the animals died. The mechanism of the protective action of LC is related to reduction of APK production, support of a high transmembrane potential of mitochondria and NO synthesis redistribution in the cells of tubular epithelium cells.

Estimation of the contractile function of the upper urinary tract by multichannel impedance ureterography before and after endoureteropyelotomy

Abstract

We studied qualitative and quantitative characteristics of the upper urinary tract (UUT) contractile function examination with multichannel impedance ureterography (MIUG) in patients given roentgenoendoscopic treatment for stricture of the ureter and pelvoureteral segment (PUS) for prognosis of the disease outcome and decision on further therapy. UUT function was studied with MIUG in 12 patients with stricture of the upper third of the ureter and PUS during and 1-2 months after endoureteropyelotomy. UUT peristalsis was assessed qualitatively (direction of peristaltic wave, rhythm of peristalsis, wave shape) and quantitatively (amplitude of ureteral contractions, frequency of peristalsis, wall tonicity). Ureteral peristalsis disorders of different severity were found in the majority of patients. UUT contractile function improved after treatment in 83% patients. The improvement concerned wave shape, peristalsis amplitude, a relief of wall tonicity, frequency of contractions). MIUG can objectively assess UUT function in patients with ureteral and PUS stricture, predict efficacy of roentgenoendoscopic treatment early after surgery. MIUG is intended for diagnosis of UUT contractile disorders undetected by x-ray, for objective control over quality of therapy, for formulating indications to pathogenetic treatment.

Long-term results of ureteral endoprosthesis with nitinol stents

Abstract

Ureteral endoprosthesis results are presented for 22 patients (9 males and 13 females) with long strictures of the upper urinary tract (UUT). Stenosis was caused by rectal tumor in 2 patients, by cancer of the uterine cervix in 5 patients. In a female with Burneville-Pringle disease metallic prosthesis was set in the pyeloureteral segment (PUS) of the solitary right kidney. Six patients had long strictures of the abdominal part of the ureter, 4 had PUS strictures, 3 - of the ureter of the transplanted kidney and 1 had stricture of ureterocalicoanastomosis after Neivert operation. Endoprosthesis was established by a retrograde approach in 13 cases, via percutaneous antegrade approach - in 9 cases. In postoperative period 2 patients had exacerbation of chronic pyelonephritis, they received anti-inflammatory treatment. Patancy of the ureter was reestablished in all the cases and persisted for all the follow-up period (4-62 months). Late after operation a patients with Burneville-Pringle disease developed UUT obstruction because of ureteral compression by enlarged angiolipomatous nodes under endoprosthesis. Therefore, one more nitinol stent was set in the upper third of the right ureter retrogradely. Thus, ureteral endoprosthesis in long and recurrent strictures of the ureter, ineffective plastic surgery on the UUT is an operation of choice.

Abstract

The article presents 2-year pilot results of a multicenter, randomized, controlled trial of prostamol®-UNO effects on symptoms progression, quality of life, tolerance and safety in patients with early prostatic adenoma. The drug was used in a single dose 320 mg/day for 36 months. Prostamol®-UNO efficacy in arrest of the symptoms progression and quality of life was assessed with the use of IPSS and QoL(BS) questionnaires. Measurements were also made of changes in Qmax, urine volume, residual urine, size of the prostate.

Abstract

A great variety of subjective and objective disorders related with chronic prostatitis make difficulties in its diagnosis. The proposed principle of chronic prostatitis classification identifies 33 clinically significant variants of diagnosis by combination of classification grades. The system of fuzzy decisive rules for prediction and differential diagnosis of chronic prostatitis supports decision on the level 0.9 and higher. Use of energetic characteristics of diagnostically significant biologically active points raises quality of prognosis of the onset and exacerbation of chronic prostatitis and helps to select optimal schemes of acpuncture. Application of decision support systems for urological decision on prognosis, diagnosis, prevention and treatment of chronic prostatitis improves quality of medical aid and adequate management of patients with chronic prostatitis.

Effects of exogenic and endogenic epidemiological factors on the onset of urinary bladder cancer (UBC) were studied with application of regression and correlation analyses. Evidence obtained with these two methods was almost similar. A complex of the most significant independent factors for men and women was determined. These factors underlie an original mathematic model of UBC development and relevant computer program. The method can define the risk of UBC for men and women and propose prophylactic measures.

Transposition of the "conflict" vessel in obstruction of the pyeloureteral segment in children

Abstract

We studied effectiveness of the "conflict" vessel transposition in operations for hydronephrosis due to a vasoureteral conflict in children. For 1999-2007 we operated 323 children with unilateral hydronephrosis in obstruction of the pyeloureteral segment (PUS). The "conflict" vessels (CV) as a cause of the obstruction were detected in 67 patients who were divided into two groups. In group 1 (n = 33; 49.3%) we made PUS resection and created antevasal pyeloureteroanastomosis. In group 2 (n = 34; 50.7%) we made CV transposition using a graft from adventitium of the anterior pelvic wall. The degree of postoperative reduction of the calycopelvic system was the same in both groups, but in group 2 dilation decreased more rapidly and reached maximum in the first postoperative month. Simplicity of CV transposition as a method of hydronephrosis surgical treatment, its high effectiveness allow us to recommend this method for study in randomized prospective trials.

A total of 110 children aged from 5 to 15 years with different forms of congenital obstructive uropathy and secondary pyelonephritis received standard combined treatment (n = 71) or standard treatment combination with photon therapy 3 months after discharge from hospital. Postoperative combined treatment including photon therapy promoted uneventful course of the postoperative period, normalized immunological indices, achievement of persistent remission.

Abstract

A case is reported of a 58 year old patient with a diagnosis: double giant ureterocele, secondary multiple calculi of the right ureterocele, calculus of the inferior calyx of the right kidney, calculus of the terminal part of the left ureter, double ureterohydronephrosis, chronic pyelonephritis, multiple calcifying myoma of the uterus. A combined single-stage operation (panhysterectomy, deletion of multiple calculi of the right ureterocele, deletion of the calculus of the left ureterocele, crosscut right ureterectomy with excision of ureterocele, right ureterocystoneostomy by Leadbetter-Politano, installation of the double J-stent into the right kidney, crosscut and longitudinal left ureterectomy with excision of ureterocele, left ureterocystoneostomy by Leadbetter-Politano, installation of intubating drainage into the left ureter, cystostomy) followed by extracorporeal shock-wave lithotripsy were performed in this adult patient with combined pathology of the urinary system.
