DIAGNOSIS AND TREATMENT OF PATIENTS WITH SECONDARY ARTERIAL HYPERTENSION OF ADRENAL AND RENAL GENESIS

Abstract

A multidisciplinary approach to the diagnosis and treatment of patients with symptomatic arterial hypertension enabled us to identify 154 patients with adrenal tumors. These patients underwent adrenalectomy for following reasons: pheochromocytoma - 73 patients, aldosteronoma - 43 patients, corticosteroma - 22 patients, incidentalomas - 16 patients. After surgery, stable normalization of blood pressure (BP) occurred in 84.4 % of cases. Requirement of antihypertensive therapy was preserved in 15.6% cases, but blood pressure did not reach the preoperative levels in any of the treated patients. Among 81 patients with renal hypertension, 67 patients underwent balloon angioplasty with stent implantation. Cause of narrowing of renal artery was atherosclerotic lesion in 48 (71.6%) patients, fibromuscular dysplasia - in 18 (26.8 %) patients, and Takayasu's disease - in 12 year-old girl. In the immediate postoperative period, normalization of blood pressure was occurred in all patients who underwent surgery for renovascular hypertension.

CHARACTERISTICS OF LIPID PEROXIDATION AND ACTIVITY OF SYMPATHETIC NERVOUS SYSTEM IN PATIENTS WITH CHRONIC CALCULOUS PYELONEPHRITIS

Abstract

108 patients (89 women and 19 men) aged from 18 to 59 years with chronic pyelonephritis (CP) on the background of urolithiasis in a phase of active inflammation were evaluated. Along with the standard methods of examination, survey plan included the use of specific biochemical diagnostic methods. The latter included an evaluation of lipid peroxidation, the level of fat-soluble antioxidant α- tocopherol, fractions of phospholipids and cholesterol in the membranes of polymorphonuclear leukocytes (PMN). Asa a result of examination, The changes in the activity of lipid peroxidation and structure of the lipid phase of PMN membranes were identified. There was an increase in urinary excretion of catecholamines (epinephrine and norepinephrine), which indirectly indicates an increase in the activity of sympathetic nervous system contributing to the progression of CP.

THE USE OF NEFRODOZ IN PATIENTS WITH UROLITHIASIS WHO UNDERWENT EXTRACORPOREAL SHOCK WAVE LITHOTRIPSY

Abstract

The article presents the results of the multicenter clinical comparative open-label trial in three parallel groups, which was aimed to the evaluation of the efficacy and safety of biologically active food supplement NefroDoz after extracorporeal shock wave lithotripsy (ESWL) in patients with urolithiasis. NefroDoz was prescribed for the lithokinetic purpose. The study involved 114 patients from different regions of the Russian Federation aged from 18 to 75 years (mean age 45,56±12,49 years) with a diagnosis of urolithiasis who underwent ESWL. Patients were divided into 3 groups according to the type of treatment. Evaluation of the effectiveness was performed using data from a blood test, urine analysis, and biochemical blood assay, ultrasound and KUB X-ray. The results showed that NefroDoz has diuretic, anti-inflammatory, and lithokinetic effects, and is effective and safe drug for the patients with urolithiasis who underwent ESWL.

POTENTIALS FOR THE PHYTOTHERAPY IN PATIENTS WITH CHRONIC PYELONEPHRITIS

Abstract

We observed 66 patients with chronic pyelonephritis in the exacerbation phase who received standard treatment with antibacterial drugs based on susceptibility of causative agents. All patients were randomly divided into two groups. Patients of study group have received phytodrug prolit super septo 2 capsules 2 times a day for 4 weeks in addition to standard treatment. Patients of control group have received standard therapy only. As a result of treatment, high effectiveness of therapy and reduction of length of treatment was noted in the study group. There were no subjective symptoms of disease in all patients at the end of treatment. Reduction of leykocituria and bacteriuria, normalization of body temperature, normalization of blood analysis and restoration of intrarenal blood flow were observed earlier in the study group compared with the control group. Recurrence of disease 3 months after end of treatment was not revealed in none of the patient in the study group, whereas it was noted in 14% of patients in the control group.

EXPERIENCE OF USE OF SLING URETROPEXY BY TRANSOBTURATOR METHOD IN THE TREATMENT OF WOMEN WITH STRESS URINARY INCONTINENCE

Abstract

The article presents the results of the treatment of stress urinary incontinence in women using a sling uretropexy by transobturator method - TVT-O procedure. From 2008 to 2011, 67 pa-tients underwent TVT-O surgery for stress urinary incontinence. The average age of the pa-tients was 48.1 ± 3.1 years. The examination included ICIQ-SF questionnaire survey, filling out the voiding diary, inspection of the external genitalia, perineum and urethra, cough test, cystography to assess the shape and location of the bladder, as well as measuring the posterior vesicourethral angle. For the evaluation of functional state of the lower urinary tract, uroflowmetry was performed. In the immediate postoperative period, stress incontinence was eliminated completely in 60 (89.6 %) patients; improvement was observed in 5 (7.5%) pa-tients; and there was no effect of the operation in 2 (3 %) patients. 12 months after surgical treatment, 43 (64.2 %) women were very pleased with the result of the operation, 20 (29.9%) were satisfied with the result, and only 4 (5.9%) patients were not satisfied. The results sug-gest that the TVT-O surgery is a safe and effective method of surgical correction of stress uri-nary incontinence in women. This method of treatment requires thorough differential diagno-sis between stress urinary incontinence and urge urinary incontinence, as well as taking into account the structural features of the pelvis.

Abstract

According to the results of epidemiological studies of prevalence of erectile dysfunction in the Russian Federation, the search for the determinants of risk of its development was performed. It was found that the leading risk factors for the development of erectile dysfunction, in descending order of statistical significance, include the amount of accumulation of points according to the IPSS score, arterial hypertension, Lipss quality of life index, age, diabetes, and overweight and obesity.

EVALUATION OF ERECTILE FUNCTION DURING THE NIGHT IN MEN WITH OBSTRUCTIVE SLEEP APNEA SYNDROME: NEW POSSIBILITIES OF MODERN SLEEP MEDICINE

Abstract

Currently, the relationship between the sleep and erectile function (EF) is not in doubt; therefore, evaluation of this relationship in the context of sleep medicine (somnology) is the most informative diagnostic method. The article presents the results of polysomnography with the option of simultaneous monitoring of spontaneous episodes of nocturnal erections in 51 men aged 46-55 years without complaints of violation of the EF. The main group (n = 37) consisted of patients with obstructive sleep apnea syndrome (OSAS). The violation of pattern (quantitative and qualitative characteristics) of nocturnal episodes of spontaneous erections in this group of men was revealed, which was regarded as the initial (pre-clinical) manifestation of erectile dysfunction. Thus, modern diagnostic capabilities of sleep medicine allow to effectively evaluate the EF in patients with OSAS and timely assign pathogenetically justified therapy.

LEVEL OF DNA FRAGMENTATION IN HUMAN SPERM CELLS IN VARICOCELE AND PROSTATITIS

Abstract

Varicocele and prostatitis are the most common andrological diseases, which may be accompanied by a decrease in the production of sperm cells, the deterioration of their quality and increased risk of infertility. This work was aimed to the evaluation of sperm DNA fragmentation index (DFI) and main indices of sperm fertility (concentration, motility and morphology), and the relationship between these parameters in the men of active reproductive age suffering from prostatitis or varicocele. Assessment of sperm DNA fragmentation was performed by SCSA (sperm chromatin structure assay) using flow cytometry; sperm parameters were evaluated according to WHO recommendations. It was shown that men with prostatitis (n = 9) and varicocele (n = 22) had significantly higher DFI compared with men in the control group (n = 22). Negative influence of these diseases on the concentration and the percentage of motile sperm cells in the ejaculate was revealed. These data suggest that the deterioration in the quality of semen in varicocele and prostatitis may be caused not only by pathospermia, but also, at least partially, by violation of the integrity of the sperm DNA. Evaluation of sperm DNA fragmentation can be recommended for use in laboratory diagnostics for prediction of fertility in infertile men.

EXPERIENCE OF THE USE OF DRUG UROREC: EFFICACY AND SAFETY

Abstract

The article presents the experience of use of the new superselective α-blocker urorec (silodosin ) in the treatment of 30 patients with lower urinary tract symptoms aged 49 to 69 years. Patients have received silodosin 1 capsule a day for 2 months. According to the results of study, silodosin has demonstrated high efficacy and an acceptable safety profile. It was shown that silodosin does not increase the risk of hypotension in patients taking antihypertensive medications. Therapy with silodosin had positive and statistically significant impact on subjective and objective symptoms of the disease, which was confirmed by the IPSS-QoL score and uroflowmetry. Silodosin has rapid onset of effect (3-4 days after the start of treatment); it can be used in all patients suffering from BPH and having indications to the administration of α-blockers, and does not require correction of doses of antihypertensive drugs in the case of combined appointment.

CHRONOBIOLOGICAL APPROACH TO THE TREATMENT OF PATIENTS WITH ERECTILE DYSFUNCTION USING A COMBINATION OF LOCAL NEGATIVE PRESSURE AND LASER ILLUMINATION

Abstract

It is shown that the synchronization of energetic, spectral, frequency, and temporal parameters of technique with biorhythms of physiological processes in the organ, which is target of impact, and in the human body as a whole, allows to significantly improve efficiency, and achieve stable and reproducible results of treatment. The article presents the results of study including 62 patients with vasculogenic ED. The study design included the randomization of patients into 3 groups depending on the complex of the therapy with the use of combined techniques, including negative pressure and laser illumination (LLNP) as a part of combined therapy and as monotherapy. Significant increase in the peak flow velocity after a course of treatment was observed in all three groups of patients. Improvement in erectile function was observed in all groups; according to IIEF score, erectile function has increased by 22.3±0.05% in group 1, by 34±1.5% in the group 2, and by 19±1.7% in the group 3, indicating the best results of treatment in the group receiving combined therapy. Combination of LLNP with the administration of PDE5 inhibitors significantly increases the effectiveness of treatment of vasculogenic ED due to the influence of physical factors on the stabilization of hemodynamics in the main arteries of the penis. After a course of therapy, increase in systemic vascular elasticity by 39.8±1.5% was also noted. The efficiency of the LLNP methodology in the treatment of patients with vasculogenic ED is demonstrated. The best results were obtained in the group of patients treated with combined therapy, including the use of LLNP and PDE-5 inhibitor.

PROGNOSTIC VALUE OF KI-67 AND VIMENTIN MARKERS IN PATIENTS WITH METASTATIC KIDNEY CANCER

Abstract

The study was aimed to determine the role of ki- 67 and vimentin markers in choosing the tactics of surgical treatment of patients with metastatic kidney cancer. The analysis of the results of treatment of 71 patients with metastatic renal cell carcinoma was performed. The first group included 16 (22.5%) patients, who underwent complete removal of distant metastases after nephrectomy. The second group consisted of 55 (77.5 %) patients who underwent cytoreductive nephrectomy only. Immunohistochemical analyses of tissues removed were performed in all patients for the evaluation of ki-67 and vimentin expression levels. When comparing the outcomes of patients after complete removal of metastases depending on the vimentin and ki-67 expression levels (<10% and > 10%), a significant increase in the cumulative survival in the case of absence of vimentin and ki-67 expression of <10 % (log - rank test, P=0.0001) was revealed. The same pattern was noted in the group of patients who underwent nephrectomy only. In the absence of expression of vimentin and ki-67 <10%, survival rate in patients who underwent metastasectomy after nephrectomy was significantly higher compared with patients who underwent nephrectomy only (P=0.001). In the case of positive expression of vimentin and ki-67 > 10%, differences in survival rate in these groups were not observed (P>0.082). Thus, an association between the vimentin and ki-67 expression levels and the degree of spread of metastatic renal cell carcinoma was identified: the high level of expression of these markers precludes the increase of life expectancy of patients after removal of all metastases.

FIRST EXPERIENCE OF HO:YAG LASER URETHROTOMY IN THE TREATMENT OF STRICTURES IN PATIENTS WITH PROSTATE CANCER

Abstract

Strictures of vesicourethral anastomosis (VUA), urethral strictures, and bladder neck obliteration are frequent complications occurring after treatment for prostate cancer and dramatically reducing the quality of life of the patients. To date, there is no single standard treatment of urethral strictures. One of the promising methods is laser optical urethrotomy using a solid-state Ho:YAG- laser. Since 2012, we treated 12 patients with strictures of VUA, urethral strictures, and bladder neck obliteration. According urethrography, the maximum length of stricture was 4.5 cm. Treatment efficacy was assessed at 6 months after surgery objectively according urethrography, uroflowmetry, and ultrasound of the bladder with the definition of residual urine; and subjectively - by IPSS questionnaire and QoL questionnaire. After removal of the urethral catheter, all patients had recovered independent urination, decreased IPSS scores by 59,5%, IPSS-QoL score by 45.87 %, decreased residual urine volume by 89.92 %, and increased maximum urinary flow rate by 78.19 %. Intraoperative complications and early postoperative complications were not observed. Ho:YAG laser is a minimally invasive and safe tool for urethrotomy of strictures of VUA, urethral strictures, and bladder neck obliteration arising after treatment for prostate cancer. Definitive conclusions about the effectiveness of this method should be based on long-term results of comparative trials.

Abstract

The article presents the description of the clinical observation of the 36 years old patient with testicular feminization syndrome: genetic (karyotype 46 XY) and gonadal male sex, female external genitalia and the absence of derivatives of Mullerian ducts (uterus, fallopian tubes, the upper third of the vagina). At the first stage, testes were removed; then, vaginoplasty using segment of the sigmoid colon for possible sexual intercourse was performed. There were no complications. Currently, the patient's condition is satisfactory; she lives a regular sex life.
