CARDIOVASCULAR DRUGS AND THEIR ROLE IN PHARMACOLOGICAL CORRECTION OF ENDOTHELIAL DYSFUNCTION

Abstract

Experimental and clinical data, which demonstrate endothelioprotective properties of cerebrovascular drugs, such as inhibitors of ACE, p-adrenoblockers, statins, calcium antagonists, are summarized and represented. Having compared data from literature and taken into account the results of our experimental studies, we can arrange the drugs in the following sequence: lisinopril > simvastatin > nebivolol > bisoprolol > nifedipine, depending on the severity of endothelioprotective properties.

INNOVATIVE SEARCH FOR ANTIDIABETIC DRUGS

Abstract

It is well known that diabetes mellitus is a heterogenous disease; its main forms are diabetes mellitus type I and II. Of all diabetic patients, about 90 % have diabetes mellitus type II. Most of hypoglycemic agents administered nowadays in antidiabetic therapy of type Ii, cause development of tolerance, gastrointestinal disorders, lactic acidosis, cardiac insufficiency, fluid retention, weight gain, idiosyncrasy; they also promote atherosclerosis. These data indicate a need for searching for new groups of hypoglycemic agents and development of new hypoglycemic drugs.

Abstract

An influence of a combination of verapamil/lysinopril drugs in doses 2,5/0,5, 5/1 and 10/2 mg/kg on the blood pressure of stressed animals is presented. It is revealed that the most rational combination is verapamil/lysinopril in a dose of 5/1 mg/kg, decreasing the blood pressure by 30,1 % on the 14th day of stress action in comparison with control group of stressed animals. It is established that verapamil and lysinopril separately decrease the blood pressure less effectively, by 3,8 and 17,5%, correspondingly.

Abstract

We researched the influence of GABA derivatives on rat delivery in experimental gestosis and on the physical and mental development of their posterity. It is revealed that the compound RSPU-152 decreases the rate of still birth 8 times better in comparison with the negative control group. The compound RSPU-151 is conducive to the increase of the physical and mental development of the posterity, which is indicated by a higher muscle strength, position-finding activity, a longer latent period of entering the dark chamber in the passive avoidance test in comparison with the control group posterity with gestosis. The obtained results are comparable with the effect of sulodexide, the drug of comparison.

Abstract

Open-label parallel group randomized investigation was performed. Ivabradine (15 mg/day), verapamil SR or amlodipine were administered for treatment of postMI silent ischemic patients with stable angina and concomitant chronic obstructive pulmonary disease (COPD) or asthma exacerbations. All drugs showed equal antianginal and antiischemic activity. They did not aggravate respiratory function or clinical symptoms of concomitant asthma and COPD. All drugs were well tolerated.

The paper presents an analysis of clinical and epidemiological features of influenza morbidity in the Volgograd region in the epidemic season 2010/11. The prevalence of moderate forms of pandemic and seasonal influenza A /H1N1/ among the young and not vaccinated patients, late hospitalization of patients and a high rate of gastrointestinal disorders in patients with pandemic influenza A /H1N1/ (California 04/2009) was revealed.

Abstract

Macrophages play one of the central roles in the inflammation development obtaining pro-inflammatory M1 or anti-inflammatory M2 phenotype depending on the microenvironment. Some endogenous reprogramming macrophages factors (MRF), for example, TGF-p, SP-D, IL-4, INF-y can be found in serum. We demonstrated that decreasing and increasing of serum concentration and MRF respectively can purposefully reprogram macrophages phenotype to M1 or M2; and alveolar macrophages of two mice lines C57BL/6 and BALB/c were practically identical in their reprogramming ability. Our findings enlarge the existing data on the role of the environment in macrophages regulation and indicate new immunologic approaches to inflammatory lung diseases treatment.

Abstract

The study considers the results of investigating the volume and shape of lobes of the thyroid gland in children and teenagers of both sexes, aged 8-15 and living in the Volgograd region. We revealed a predominance of right-sided asymmetry fraction of thyroid volume in children and adolescents aged 8-15. It is shown that the shape of thyroid lobes in the investigated group of children and teenagers aged 8-15 approximated a correct ellipsoid in shape. The results confirm the validity of the formula by C. L. Brown (1981) for calculating the volume of the thyroid.

The study revealed that the composition of the biocenosis in parodontal disease includes 12 species of staphylococci, of which 1 is coagulase-positive and 12 are coagulase-negative. It was established that the predominant species of staphylococci in the association is S. aureus, which has strongly virulent properties and factors contributing to their persistence.

IMPROVING THE ORGANIZATION OF TIMELY DETECTION OF PULMONARY TB CASES IN MOSCOW CITY

Abstract

During the 12-year study (1997-2008) we evaluated the dynamics of major statistical parameters and described them using the criteria of effectiveness of timely detection of pulmonary TB cases by therapeutic-and-prophylactic institutions of Moscow, including 16 TB dispensaries. The principal method of TB detection among adults was radiology (X-ray, fluorography). To evaluate the results of case detection we proposed a complex of criteria, both external (incidence and clinical structure of pulmonary TB, proportions of patients, who died from TB, unknown to dispensaries or during the first year of follow-up), and internal ones (concordance between cases subject to X-ray/fluorography screening to the absolute amount of examined patients, the percentage of implementation of preventive screening plan and the population coverage by fluorography, the proportion of TB risk groups among the population and their coverage by X-ray/fluorography screening, the proportion of targeted populations and their coverage by X-ray/fluorography).

Abstract

An analysis of the antioxidant system condition by the method of spontaneous leucocyte hemoluminescence of the whole blood in patients with chronic acalculous cholecystitis in exacerbation and remission periods is proposed. We revealed a reliable increase in the maximal intensity of spontaneous leucocytic hemoluminescence of the whole blood, a decrease in the time of maximal spontaneous hemoluminescence appearance in exacerbation period and normalization of indices, which do not differ from those of the control group in remission period. We evaluated laboratory effectiveness of spontaneous leucocytic hemoluminescence of the whole blood in diagnostics of stages of chronic acalculous cholecystitis.

The objective of the study was to improve the results of Frey procedure in patients with chronic pancreatitis by the proposed method for prevention of septic complications. 16 patients were operated on by the original method. Clinical studies have confirmed the effectiveness of the method in managing underlying symptoms, clinical manifestations of chronic pancreatitis and preventing inflammatory, infiltrative and disseminated septic complications of the abdominal cavity.

One of the objectives of preventive antibiotic administration is to prevent the development of microbial resistance to chemotherapy, but its impact on biocenosis is unclear. We have shown distinct pathogenesis of resistant microflora in rats from bacterial monocultures. On the third day of the experimental chemotherapy, in biocenosis there begins a replacement of normal microflora by the resistant types and development of dysbiosis.

General plasticity of comprehension is associated with flexibility, emotional dynamism and instability, energy saturation and resistance to external influences.

Abstract

Sections of rat spleen were investigated before acupuncture and 15 minutes, 1, 2 and 4 hours after acupuncture in the field of acupuncture at points LI 4 and GV 14 by method of luminescent microscopy. It was established that after acupuncture the bioamine status of the spleen changes in 15 minutes under the impact of modification of monoamine content in the red and white pulp. The change of the content of catecholamines and serotonin in granular luminescent cells of the responsive center of the spleen is of wavy nature. Thus the correlation between the central artery and granular luminescent cells of the responsive center, the central artery and granular luminous cells of red pulp, granular luminescent cells and the background of responsive center became strongly positive.

Abstract

We have studied the conditions for isolation of analgesic drugs. We proposed a solvent for extraction, determined pH of the solution. We established the conditions for identification of analgesic drugs by using TLC, UV spectroscopy, chromatomass spectrometry, densitometry, and gas chromatography. We have shown that UV spectroscopy, HPLC and densitometry may be used for quantitative analysis. We have determined the stability of drugs in biological objects.

PECULIARITIES OF STRUCTURAL CHANGES IN THYMUS AFTER EXPERIMENTAL HYPERTHERMIA

Abstract

It is established that influence of the experimental hyperthermia (EH) on the Wistar rat organism leads to formation of accidental involution of thymus that is of temporary, reversible nature. Temporal involution of this organ corresponded to the «catabolic» phase of the posthyperthermal period (5 hours, 3 days after EH), which was manifested in a decrease of relative weight of the organ, reduction of the relative area of cortex and density of lymphocyte in all zones, especially in the internal cortical substance zone. Intensification of destructive processes at a tissue level, characters of perivascular hypostasis, migration of cellular elements in lymphatic spaces and capillaries were noted. Considerable restoration of the thymus was observed by the end of the «anabolic» phase (14 days after EH) of the posthyperthermal period.

Abstract

We studied the rate of infectious and inflammatory diseases, its dependence on thyroid hormone levels and the presence of vegetative dysfunction in 181 patients with dysplasia of the connective tissue of heart (DCTH). It is established that patients with different DCTH develop infectious and inflammatory diseases significantly more often than individuals without DCTH. In individuals with DCTH we revealed dependence between a triple rate of AVRI or chronic bronchitis, and the thyroid hormone levels as well as an association between the rate of chronic gastritis, pancreatitis, duodenitis and peptic ulcer disease, and the presence of vegetative dysfunction.

Abstract

This research is devoted to studying the possibility of adaptation of occlusal relations in orthopedic patients with partial edentia with drug-compensated diabetes mellitus, hyperacid gastritis and neurotic disorder. Occlusal splints for adaptation of the periodontium and neuromuscular system were manufactured for one group. The second group of people did not receive an occlusal splint, 72 % of them had exacerbation of pain. Those patients received a prosthesis in 6-8 weeks, after stabilization of the general state of health. Selective grinding of teeth in patients with accompanying diseases and partial edentia had to be made with supervision of occlusal splints to achieve a stable occlusion, which prevents a development of symptoms of articular muscle dysfunction.

Abstract

A prospective study of etiology of community-acquired pneumonia in 75 patients with diabetes mellitus type 2 in Volgograd was performed. Streptococcus pneumoniae (33,4 %), Klebsiella pneumoniae (13,4 %) and Staphylococcus aureus (13,4 %) were the most frequent isolates. Haemophilus influenzae, Mycoplasma pneumoniae, Chlamydophila pneumoniae, Enterobacter aerogenes were isolated from 8; 7; 9; 2,7 % and 1,3 % of all strains, respectively. Considering the database on the structure of bacterial pathogens and their local antimicrobial resistance, an algorithm for choosing antibiotics for the given group of patients is proposed. Amoxicillin/clavulanate, III- IV generation cephalosporins, carbapenems and their combination with macrolides or respiratory fluoroquinolones can be considered drugs for empiric therapy of non-severe community-acquired pneumonia; in severe pneumonia - with respiratory fluoroquinolones.
