Differential approach to the treatment of lower urinary tract symptoms in benign prostatic hyperplasia

Abstract

The article considers the current approaches to the treatment of benign prostatic hyperplasia (BPH). The data for etiology, clinic and diagnosis of BPH are presented. The possibilities of alpha1-adrenergic blockers and 5alpha -reductase inhibitors administration in BPH are discussed. The article substantiates methods of combination therapy using alpha1-blocker treatment (short course) and 5alpha-reductase inhibitors, and alpha1-blockers in combination with anticholinergic drugs in patients with BPH with symptoms of overactive bladder. Much attention is given to the treatment of acute urinary retention.

Abstract

The article presents current state of hypogonadism problem, in particular its age forms; the role of the hypogonadism in the development of sexual function disorders in men is presented. We discuss the relationship of hypogonadism with somatic disorders, including cardiovascular disease, type 2 diabetes mellitus, and metabolic syndrome. We consider testosterone drugs that currently use for the treatment of hypogonadism and erectile dysfunction. We particularly discuss the problem of hormonal replacement therapy safety using testosterone drugs, particularly its impact on the risk of prostate cancer development.

Immune correction in surgical practice

Abstract

The problem of immunosuppression caused by surgical infections is discussed. The results of study of immune status, effective balanced immunoprophylaxis and therapy in surgical clinic are presented. The article describes the links of the immune response, methods of immune status assessment in patients with various forms of surgical infection; characteristic of immunotropic drugs allowing to improve the effectiveness of surgical treatment, and immunocorrection algorithm in various forms of infection and immunogram types are presented. The authors recommend immunocorrection in all surgical patients both at their treatment, and during the preoperative and postoperative period.

Current potential for antibiotic prophylaxis in surgery

Abstract

The need for perioperative antibiotic prophylaxis (AP) during surgical operations is proved and acknowledged. Adequate AP not only reduces the incidence of postoperative infectious complications, but also significantly affects the most important measures of treatment effectiveness, such as the duration of hospital treatment, the cost of hospital bed-day, and mortality. The data for risk factors of postoperative wound infections, choice of antibiotic for the AP in the light of patient's "antibacterial history» are presented; particular attention is given to the peculiarities of AP in cardiac surgery.

Abstract

We analyzed the results of 3239 autopsies in three major hospitals in 2003-2006. The frequency of comorbidity in surgical patients reaches 33%, which creates additional difficulties in diagnosis and management of these patients and leads to the high frequency of divergence in diagnoses. The example of two clinical cases demonstrated the need for a unified strategy and tactics of physician and surgeon in the surgical clinic, which contributes to the correct diagnosis and choice of optimal treatment tactics.

Abstract

The modern classification of prostatitis is presented, it is noted that abacterial chronic prostatitis/chronic pelvic pain syndrome (CPPS) account for 90-95% of cases. The paper is dedicated to the diagnostic criteria for CPPS, the algorithm of assessment, current approaches to the treatment of this disease. The authors present the results of the nutritional supplement Longa-DNA administration, an essential component of which is L-arginine, in 31 patients with CPPS. Following the course of therapy, all patients noted subjective state improvement, mainly manifested in the pain relief.

Abstract

The results of the study the effectiveness of long-term treatment with α-adrenoblocker Dalfaz (alfuzosin), which included 41 patients of benign prostatic hyperplasia (BPH), are presented. The study demonstrated the high safety of Dalfaz administration in patients with urination disorders. Dalfaz has received a good mark as a safe and high effective drug for prolonged drug therapy for BPH. The study confirmed the principal possibility of permanent Dalfaz administration in patients with benign prostatic hyperplasia for many years as first-line drugs for medication correction of urination disorders in patients with BPH.

Abstract

Premature ejaculation (PE) is the most common male sexual disorder. The results of the study, including 55 men with PE and 30 healthy volunteers, whose goal was to define the role of somatosensory evoked potential (SEP) in the differential diagnosis of different forms of PE and predicting the effectiveness of treatment with selective serotonin reuptake inhibitors (SSRIs) in this category of patients are presented. At the stage of primary ambulation, the SEP evaluation is also allow to perform differential diagnosis between primary and secondary forms of PE. This will optimize the algorithm of examination and treatment of this category of patients. The subsequent SSRIs administration are clinically effective in patients who respond to the test with paroxetine, which also plays an important role in the choice of treatment.

Use of tamsulosin (Sonizin®) in the treatment of chronic abacterial prostatitis

Abstract

The results of treatment of 49 men with chronic abacterial prostatitis are presented. In the first (study) group were included 30 men from the first day of treatment received tamsulosin (Sonizin®) in a dosage of 4 mg daily during the month. The second (control) group included 19 patients who did not receive alpha-adrenoblockers. Patients in both groups have received standard treatment of pancreatitis. Excellent and good results of treatment were observed in 92.6% of patients from the study group in comparison with 66.4% of patients from control group. In the study group also noted a more pronounced decrease in the amount of symptoms. It was concluded that tamsulosin can be recommended for widespread use in the treatment of chronic abacterial prostatitis, and it is the first-line drug for patients with dominated dysuric disorders.

Abstract

The results of study of indole-3-carbinol and epigallocatechin-3-gallate (Indigal 400 mg) effects on the processes of alteration and repair of the urethra in experimental animals after surgery using tissue adhesive latex are presented. It is shown that Indigal due to antioxidant and antiproliferative properties affects the processes of alteration and repair in the pathologically altered urethra that lead to formation of cribriform elastic connective tissue with high level of capillary blood flow in this zone.
