NEOADJUVANT THERAPY OF THE BREAST CANCER WITH HIGH EXPRESSION OF HER-2

Abstract

Patients with regional breast cancer with the high expression or amplification of HER-2 have a very poor prognosis. Before development of modern chemotherapeutic regimens including anthracyclines and taxanes, only 5 % of such patients survived 5 years after treatment. The article presents the analysis of few but well-planned clinical trials, which have shown that adjunction of anti-HER-2 therapy (drug Trastuzumab) to neoadjuvant chemotherapy with anthracyclines and taxanes leads to the increase of full pathomorphologic tumor regression rate from 20 to 40-65 % and reduction of risk of disease progression or death by 40 %.

Abstract

In the literature review questions of pathogenesis of uterine myoma LMM, with accent on hormonal regulation of tumours proliferation are considered. The doubtless role of a progesterone in the pathogenesis of myomas is an adequate substantiation of application antigestagenes in disease therapy. Successful clinical application mifepristone confirms the leading part of a progesterone in the control of growth of a myoma. Now medicamentous treatment of a myoma of a uterus is not ideal, nevertheless, from all spectrum of offered approaches use antigestagenes (mifepristone), considering its high efficiency and safety, it has appeared the most comprehensible.

PREPARATION OF UTERINE CERVIX FOR CHILDBIRTH

Abstract

The article presents review of the literature aimed to the evaluation of the effectiveness, safety, optimal doses, treatment schemes, and frequency of side effects of drugs used for preparation the cervix for the childbirth. Currently, the effectiveness and applicability of prostaglandins (PG) and mifepristone is proved. Among PG, PGE2 or PGE1 analogues are more often used. The most frequently used methods of use of PG are intracervical and vaginal administration. PG for cervical ripening is usually used in the hospital due to high risk of side effects. Mifepristone is drug, which simulates the natural processes of cervical ripening without clinically significant side effects. Mifepristone can be used for pre-delivery preparation in the outpatient setting. In the scientific literature, there are no comparative studies of effectiveness of mifepristone and PG, and the use of drugs that stimulate nitric oxide formation, is currently poorly understood.

Abstract

The present article is dedicated to the problem of magnesium deficiency (MD) that is caused by increasing frequency of MD among examined patients and its proved relationship with a wide spectrum of diseases and the pathological conditions associating with endothelial dysfunction, apoptosis defect, metabolic and hypertensive syndromes, pathology of connective tissue, and problems of immunity. MD has particular significance at pregnancy, since conception and its earliest terms, because magnesium is universal regulator of biochemical and physiological processes in an organism. It has been established that magnesium deficiency makes particular contribution in the development of pregnancy complications that defines an important role of preventive use of magnesium preparations since preconception period.

ANTIBIOTICS IN SOLUTAB PHARMACEUTICAL FORMULATION

Abstract

The present article is concerned with dosage forms of the antibacterial agents, allowing using preparations in the liquid state - syrups, granules or others soluble or dispersible products facilitating introduction of medicines that is especially important in children. These include Flemoklav Solutab (Amoxicillin/Klavulanat), Flemoxin Solutab (Amoxicillin) and Vilprafen Solutab (Josamycin) dispersible tablets. Fundamental difference of Solutab form is the encapsulation of active substance in microspheres that protects from adverse effect of gastric acid and enzymes. The presented data shows that "line" of antibacterial drugs in the Solutab dosage form largely overlaps antibiotic requirements for treatment of widespread bacterial infections both in adults, and in children. These drugs have usability, comfortable dose schedule, and favorable safety profile, as well as additional advantages in regard to pharmacokinetic properties and low frequency of development of microbial resistance.

Abstract

Insufficient consumption of vitamins and micronutrients leads to disorders of biochemical processes and physiological functions of organism, as well as causes serious metabolic disturbance, and abnormal formation and fetal development during pregnancy. The folic acid is major vitamin participating in synthesis of DNA, coenzymes and regulation of many processes in an organism. Sufficient intake of folic acid before pregnancy and on its early terms reduces risk of development of neural tube defects, Down's syndrome, and anomalies of cardiovascular and urinary systems, formation of palatoschisis and other defects. Recommended daily intake of folic acid for adults is 400 mcg, but during pregnancy it raises to 400-800 mcg, and during lactation - 600 mcg. As a consequence, administration of balanced diet and vitamin preparations are necessary.

The special attention is given to the treatment of VVC in pregnant women.

Abstract

The article is dedicated to the results of two studies of use of microcellulose powder (Nasaval) and microcellulose with moly extract (Nasaval Plus). The results represent a modern trend in prevention of contact of allergens and viruses with nasal mucosa by creation on its surface gelatinoid substance. Administration of drug Nasaval before expected allergic reaction allows to reduce clinical implications of allergic rhinitis, conjunctivitis and asthmatic attack without additional treatment. Use of a drug Nasaval Plus for the purpose of prophylaxis significantly prevents occurrence of droplet infections and reinfection. Use of Nasaval Plus in the treatment of active acute respiratory viral infection promotes reduction of duration and severity of disease.

Abstract

The present article is dedicated to the case of delayed diagnosis of cystic fibrosis (CF) in 6 years old patient with recurrent polysinusitis treated in pulmonological hospital. Diagnosis of CF was confirmed by genetic research. In the represented case history, polysinusitis was the basic clinical presentation of CF, relegating to the background upper respiratory tract symptoms, but which were recognized by careful examination. It was noted that Pulmozim (Dornase alfa) has distinct positive effect, allowing to achieve complete recovery of nasal breathing, hearing, and improvement of quality of life.

AUGMENTIN ES - THE DRUG WITH THE ADVANCED PHARMACOKINETICS

Abstract

The present article is concerned with inhibitor-protected antibiotic Amoxicillin/Klavulanat (Augmentin) with high efficiency in the treatment of infections of respiratory tract, soft tissues and urinary system. Currently, the most preparations of Amoxicillin/Klavulanat were developed with component ratio 4: 1, however experimental and clinical researches have shown benefits of "advanced" preparations with higher component ratio, in particular 16: 1 (Augmentin XR) and 14: 1 - a pediatric syrup Augmentin ES. It was shown that Augmentin ES is effective in the treatment of acute otitis media, particularly in otitis caused by pneumococci with the decreased sensitivity, as well as caused by β-lactamase - producing H. influenzae and M. catarrhalis. The preparation is effective in the treatment of acute bacterial sinusitis caused by the same flora, and in the treatment of other infections, for example caused by staphilococci with higher minimal inhibitory concentration in relation to Amoxicillin.

CHANGE OF CONGENITAL IMMUNITY FACTORS IN CHILDREN WITH CYSTIC FIBROSIS

Abstract

The aim of the study was to determine the key factors of innate immunity in the 118 children with cystic fibrosis. In patients with CF identified pronounced changes in the indices of innate immunity, both in serum and at the level of the mucous membrane. In chronic sowing of the bronchial tree, Pseudomonas aeruginosa and/or Staphylococcus aureus levels of bactericidal and lysozyme activity of blood serum, C5a component of complement can be considered as markers of inflammation and provide additional laboratory diagnostic criteria of bronchopulmonary phase of the process. In acute bronchopulmonary process in children with Burkholderia cepacia observed anergy factors of innate immunity, which dramatically increases the risk of septic complications.

VACCINATION AGAINST INFLUENZA WITH AGRIPPAL S1 OF PREGNANT WOMEN

Abstract

14 pregnant women ages 18 to 38 years at 22-36 weeks of gestation in good general health and without recent or concurrent febrile illness, gave written informed consent to receive immunization with influenza A/Brisbane/59/2007 (HlNl), A/Brisbane /10/2007 (HlN2), B/Florida/4/2006 vaccine Agrippal S1. There were no significant side effects after immunization in any of the women. Antibody titers were determined to A/Brisbane/59/2007 (HlNl), A/Brisbane /10/2007 (HlN2), B/Florida/4/2006 before and after a single dose of influenza vaccine. Protective titers to all three strains of vaccine were detected in 13 pregnant women.
