EFFICACY OF OMEGA-3 POLYUNSATURATED FATTY ACID FOR PRIMARY AND SECONDARY PREVENTION OF CARDIOVASCULAR DISEASES: CURRENT STATE OF THE PROBLEM

Abstract

In this review, the advisability of broad prophylactic and therapeutic application of ω-3 polyunsaturated fatty acids (ω-3 PUFA) in various cardiovascular diseases (CVD) is proved. Epidemiological studies show a positive effect of ω-3 PUFAs on the risk of CVD complications. Results of numerous clinical studies evaluating the efficacy of ω-3 PUFA for the primary and secondary prevention of coronary heart disease after coronary artery stenting, heart rhythm disturbances, heart failure, and dyslipidemia are discussed. A number of studies confirm the benefits of drugs containing ω-3 PUFAs, with respect to both the degree of frequency and clinical efficacy in comparison with their other sources.

Abstract

The article discusses the problem of bacterial biofilms formed by various microorganisms, including in the organs of the gastrointestinal tract. Helicobacter pylori can form a biofilms, and plays a crucial role in the development of peptic ulcer disease. It is emphasized that the use of eradication schemes with the drugs, potentially affecting the structure of H. pylori biofilm, allows to achieve the best results of eradication therapy. This is confirmed by a long and successful use of clarithromycin, having specific effects on microbial biofilms, in the first-line eradication therapy.

Abstract

The article presents an overview of the biochemical properties of ademetionine, which has active multilateral effects on the cell metabolism. The pharmacological activity of preparations containing ademetionine as an active substance was assessed and prospects of development of new drugs were shown. In particular, ademetionine can modulate apoptosis in normal and proliferating hepatocytes, and in this respect ademetionine is potential agent for chemotherapy of hepatocellular carcinoma. Due to anti-inflammatory and regenerating properties, ademetionine may be used in intensive care units and in burns centers.

CONNECTIVE TISSUE DYSPLASIA AND MAGNESIUM DEFICIENCY

Abstract

The article discusses the problem of connective tissue dysplasia (CTD). The role of magnesium deficiency in the genesis of CTD is revealed. Disturbance of magnesium homeostasis plays a significant role in the development of diseases of all body systems, especially the nervous, skeletal, cardiovascular, and reproductive systems. Therefore, early diagnosis of CTD, particularly in children, allows to administrate proper rehabilitation therapy and to prevent disease progression. Prolonged use of drugs, directly and indirectly affecting the metabolism of connective tissue: stimulants of collagen synthesis in combination with trace substances such as magnesium orotate (Magnerot), is useful in all patients with symptoms of hereditary CTD.

Abstract

Vertebral artery syndrome (VAS) combines the complex of cerebral, vascular and autonomic syndromes, arising from the lesion of the sympathetic plexus of vertebral artery, the deformation of its wall or luminal changes. In neurological practice, the diagnosis of VAS, as a rule, is making in patients with the specific set of complaints and clinical syndromes that can be associated with degenerative lesions or anomalies of the cervical spine. The problems of diagnosis of VAS are considered, the treatment of this pathology is discussed. Data for the possible use of nonsteroidal antiinflammatory drug Airtal (aceclofenac), centrally acting muscle relaxant Mydocalm (tolperizone) and other medicines are presented.

Abstract

It examines new possibility of functional dyspepsia treatment, its ulcerous and dyskinetic variants. It was worked out fixed combinations of remedies, combining in one capsule prokinetic domperidone and proton pump inhibitor omeprazol. Such combined preparation is Omez D, which normalizes motor function of digestive tract upper part and inhibitor secretion of hydrochloric acid in stomach. It positively influences on reduction of pain syndrome in epigastrium and dyspepsia presentations, which are typical for functional dyspepsia. It is presented indications of Omez D, schemes and intake.

Abstract

Migraine is common form of headache, leading to restriction of social adaptation of patients, decrease in quality of life, and significant economic losses. The basis of many forms of migraine is a genetic predisposition; development of attack involves biochemical disturbances, disorders of vascular tone regulation, and violations of pain control mechanisms. On account of features of the pathogenesis of migraine, prophylactic treatment to prevent and jugulate attacks is required. Step-wise scheme for use of drugs of different pharmacological groups for jugulation of migraine attacks is adopted in most countries. The possibilities of using of combined drug - Nomigren, which consists of ergotamine, analgesic, antiemetic drug, and caffeine, are discussed.

VASCULAR DEMENTIA - THE PROBLEM OF GENERAL PRACTITIONER

Abstract

The article discusses the clinical significance and approaches to the treatment of cerebrovascular diseases in the physician's practice. The vascular dementia (VD), which first must be recognized by general practitioner, inevitably lead to social maladjustment, promotes a discernible decline in adherence to treatment; in this connection, the course of majority of chronic diseases worsens. The contributing factors for VD and other cognitive impairments (arterial hypertension, diabetes mellitus, obesity, etc.), possible ways of their prevention and treatment are considered. Results of clinical studies show that pentoxifylline is one of the pathogenetic action drugs which inhibit the progression of cognitive dysfunction. It is noted that long-term use of pentoxifylline allows to reduce the rate of development of VD in patients after acute cerebrovascular events, including recurrent events.

Abstract

The article presents the main criteria for the diagnosis of complex regional pain syndromes (CRPS); main stages of pathogenesis and clinical features of these conditions are described. Much attention is paid to methods for the treatment of complex regional pain syndromes, including the use of nonsteroidal anti-inflammatory drugs (NSAIDs). Nimesulide (Nise) is one of the selective NSAIDs considered for treatment of CRPS whose action is being implemented not only through inhibition of cyclooxygenase type-2, but also due to the ability to inhibit the production of free radicals and tumor necrosis factor α.

Abstract

Recent studies indicate a definite relationship of psoriasis and cardiovascular diseases. The article presents the results of study, which was aimed to evaluation of efficacy and safety of Co-Diroton, a fixed low-dose combination of lisinopril and hydrochlorothiazide (HCT) as compared with enalapril and hydrochlorothiazide in 83 psoriatic patients with arterial hypertension (AH) 1-2 degree and overweight . It was shown that administration of enalapril 10 mg/day, HCT 25 mg/day, or Co-Diroton 10/12.5 mg in this group of patients is safe, well tolerated and accompanied by a significant decrease in blood pressure (BP) without reducing the effectiveness of antipsoriatic therapy. Thus, Co-Diroton had significantly more antihypertensive effect than enalapril or hydrochlorothiazide as monotherapy: against the background of its use significantly more patients achieved target BP levels (83 vs 48 and 44 %).

EFFECT OF LONG-TERM ANTIOXIDANT THERAPY WITH N-ACETYLCYSTEINE ON OXIDATIVE STRESS INDICES IN PATIENTS WITH CHRONIC OBSTRUCTIVE PULMONARY DISEASE

Abstract

The dynamics of free-radical lipoperoxide processes and antioxidant activity of blood plasma in 62 patients with stable chronic obstructive pulmonary disease (COPD) of different severity against the background of prolonged use of high-dose N-acetylcysteine in the combination treatment of disease were assessed. It was noted that lipoperoxide processes, reflecting the intensity of oxidative stress, were more pronounced among patients with COPD who not received inhaled corticosteroids (ICS). On the background of long-term antioxidant treatment of COPD with high-dose N-acetylcysteine, intensity of oxidative stress has decreased, and the antioxidant effect of treatment was significantly more pronounced in patients received ICS. This was accompanied by decrease in respiratory symptoms and increase in exercise tolerance. It is emphasized that the inclusion of high-dose N-acetylcysteine (1200 mg/day) in the combination treatment of stable COPD with the aim of suppression of oxidative stress is efficient for patients receiving ICS, i.e. patients with severe and very severe COPD.

Abstract

The article presents the results of evaluation of 30 women with dysmenorrhea, which was aimed to assessment of efficacy, safety and tolerability of Neurodiclovit in the treatment of this disease. According to the authors and published data, key role in the elimination of pain syndrome in this state belongs to the nonsteroidal anti-inflammatory drugs (NSAIDs) due to the importance of cyclooxygenase mechanisms of pathogenesis of primary dysmenorrhea. The results showed that Neurodiclovit is effective drug in the treatment of primary algodismenorrhea, since it has the characteristics both of NSAIDs and vitamin B.

Abstract

Blood-brain barrier prevents the transport of drugs into the central nervous system. This limits the possibilities of therapy for many diseases. Thus, the drug transport across the blood-brain barrier remains one of the key problems of modern pharmacology. This study investigated the possibility of using blood apolipoproteins to increase of dalargin transport through the bloodbrain barrier. Antinociceptive activity of dalargin (7,5 mg/kg) adsorbed on nanoparticles with different coating was studied on outbred albino mice by the tail-flick test. Delivery of dalargin to the brain was most effective and safe in case of using nanoparticles with apolipoprotein E coating.

Use of analgesic Novigan for the treatment of tension-type headache in patients with concomitant somatic pathology

Abstract

The article is dedicated to the features of tension-type headache; data for the place of nonsteroidal anti-inflammatory drugs (NSAIDs) in treatment of pain management are presented. The possibilities of use of the new combined analgesics, such as drug Novigan, are discussed. Novigan is a combination of NSAID ibuprofen (400 mg), pitofenone hydrochloride, papaverin-like peripheral action spasmolytic, and fenpiverinium bromide, cholinolytic with central and peripheral actions. The results of randomized open comparative study aimed to evaluation of efficacy and safety of Novigan and ibuprofen (MIG 400) in 45 patients with tension-type headache and concomitant somatic pathology are presented. It was shown that Novigan is an effective drug for the treatment of cephalgic syndrome of musculosceletal origin that affects the basic mechanisms of its development. Novigan is superior to MIG 400 in the rate of development of analgesic effect, which is obviously due to the presence of spasmolytic component and pharmacokinetic characteristics of the drug.
